01 · The core answer
Can you take Mounjaro if you have hypoglycemia?
You may be able to, but the decision belongs to a prescriber who knows your history. Mounjaro is a prescription-only medicine, supplied only after a consultation reviewed by a GPhC-registered prescriber.1 A history of low blood sugar is exactly the kind of detail that review is there to weigh up.
The phrase "hypoglycemia" covers very different situations. Some people have diabetes and have low readings because of their treatment. Others have no diabetes and notice shaky, hungry spells between meals. A few have a specialist diagnosis. The same medicine can be reasonable in one case and unsuitable in another.

What to do before you order
Write down when your lows happen, what you had eaten, and every medicine you take. Bring that list to the consultation. If a specialist or diabetes nurse manages your condition, ask them first.
Mounjaro is licensed for weight management in adults and for type 2 diabetes.1 If weight is your goal and you are comfortable with the assessment, you can start with the Mounjaro pen and free consultation. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

03 · Insulin and sulfonylureas
Insulin and sulfonylureas: the combination that needs care
The medicines you take alongside Mounjaro matter more than almost anything else. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines. That is different from saying the medicines cannot be combined. They can be, with a plan.113
Do not reduce or stop a diabetes medicine yourself. Any change should come from the clinician who prescribes it, who will know your readings. If you are unsure whether a tablet counts as a sulfonylurea, check the leaflet or ask a pharmacist.
Tirzepatide acts on two gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 Because the medicine works on insulin response, anyone already treated for blood sugar should have the combined effect reviewed rather than guessed at.
04 · Low sugar without diabetes
Hypoglycemia without diabetes
Yes, people without diabetes can have low blood sugar, and it deserves a proper explanation before starting any new medicine. Only a clinician who reviews your history can identify the cause.
If you have unexplained shakiness, sweating, sudden hunger or confusion between meals, see your GP. Do not assume a weight-loss injection will fix it or that it is harmless. Treating the underlying cause comes first.
If you notice a new symptom after starting, tell your prescriber and report it through the Yellow Card scheme.19
05 · Eating patterns and safety
Eating patterns, appetite and blood sugar
Mounjaro makes most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 For someone prone to low blood sugar, that effect deserves thought. A smaller appetite can mean longer gaps between meals, and that is the moment to talk to your prescriber rather than push through.
It is used alongside a reduced-calorie diet and increased physical activity, not as a reason to eat very little.1 Regular meals are a sensible default if lows are a worry. If you want a tailored plan, a registered dietitian can build one around your medicines and your history, which is better than a generic hypoglycemia diet from the internet.
Questions like "what should you avoid if you have hypoglycemia?" have individual answers. Common prompts to raise with a clinician include long gaps without food, heavy alcohol use and unplanned exercise.
06 · Who should not take Mounjaro
Who should not take Mounjaro
Mounjaro is not suitable for everyone, and the prescriber decides after reviewing your health questionnaire. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.13
Tell the prescriber about every condition and medicine, including ones that seem unrelated. Mounjaro can also reduce absorption of oral contraceptives, because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mention contraception in your consultation.1
07 · Ozempic and other options
What about Ozempic or Wegovy?
The same questions apply to other GLP-1 medicines. Semaglutide, sold as Wegovy and Ozempic, is a single-action GLP-1 medicine, whereas tirzepatide activates both GIP and GLP-1.1 Neither is automatically safe or unsafe if you have hypoglycemia. The prescriber still needs your history and your medicine list.

08 · Stopping and monitoring
Monitoring, aftercare and stopping
Treatment is reviewed over time, not decided once. Our aftercare includes ongoing monthly reviews, dose titration and follow-up assessments by the prescribing team, plus side-effect support. Repeat orders re-check suitability, and nothing ships automatically.
If you have low-sugar symptoms after starting, such as shakiness, sweating or confusion, contact your prescriber or your diabetes team. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Mounjaro has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

What Mounjaro can do for weight
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary.
Benefit has to be weighed against your own risks. For someone with a history of low blood sugar, a supervised start, honest reporting and a clear plan matter more than the headline numbers.
On the NHS, Mounjaro is available but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 Many people who meet the licensed criteria choose regulated private treatment rather than waiting.
