04 · What the licence allows

What the licence allows when you want to go slower

Within the licence, your prescriber has real flexibility, and that is the legitimate route to a lower dose. This is the answer to the question as many people mean it: yes, a clinician can keep you at a lower strength when that is the sensible choice for you.

What the trial evidence says about different doses

The main UK-relevant trial evidence comes from people on 5mg and above, so it cannot tell us what to expect on 2.5mg alone. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Read that carefully. The trial reports 5mg, 10mg and 15mg. It does not give a headline figure for 2.5mg because that strength is the entry step on the ladder. So there is no trial figure to point to for the lowest strength.

There is, however, a useful message about lower maintenance strengths. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 That shows a settled 5mg dose is itself a meaningful, evidence-backed treatment level, not a consolation prize. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight, which shows the higher strengths can deliver more for some people.2

A worked example helps. For someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. Those are trial averages and your own result may be lower or higher, so treat them as context, not a forecast. The trial measured results at 72 weeks, so think in months, not days.1

The practical lesson is that the choice is rarely between the lowest strength and the highest. A settled middle strength may suit you. Note how your appetite feels between meals, how long side effects last after each injection, and how your weight trend looks over several weeks rather than days. Mention anything new that worries you. Be honest about your eating pattern and activity, because treatment works alongside a reduced-calorie diet and increased physical activity.

It also helps to say plainly what you want. If your goal is to use the smallest effective amount, tell the prescriber. If you are anxious about higher strengths, say that too. A prescriber can only tailor the pace if they know your preferences.

How Mounjaro works, and what a lower dose means

Mounjaro works by activating two gut-hormone receptors at once, and the dose controls how strongly that effect is felt. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 A lower strength produces a gentler version of this. That can be enough for some people, and it is why a prescriber may agree to hold a dose when your appetite already feels well managed and side effects are low.

Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts. Taking a slower route up the ladder gives your body longer to adapt.

The medicine stays in your system for a while. Tirzepatide has a half-life of roughly five days, so after a final dose it takes four to five weeks to clear the system, there is no way to speed this up, and nothing needs doing while it clears.1 That has a practical consequence for low-dose plans: stepping down or pausing does not cause an instant change, and effects fade gradually rather than switching off.

There is one interaction worth knowing about if you are choosing a lower dose for tolerability. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, so talk to your prescriber about contraception whatever strength you take.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Likewise, if you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so your prescriber should know about all your medicines.

Tirzepatide is licensed in the UK for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose pen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Where you settle along that ladder is a personal clinical decision, not a test of willpower.

08 · Using your pen safely

Using your pen safely while you hold a dose

If you stay on one strength for several months, a steady routine matters more than anything else. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day, because it works the same morning or evening, and what matters is the same day each week.1 Many people choose a weekend morning so early side effects fall on days off.

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection.1 Rotate the site with each dose. Follow the pen's instructions for the injection itself, and keep your needles and sharps bin to hand, as both are included with every order.

Missed doses are common when you are settled into a routine. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1 This matters for anyone holding a low dose, because doubling up to catch up is exactly what the rule prevents.

You can change your weekly injection day if needed, as long as there are at least 3 days between two doses.1

Storage keeps the pen reliable. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them.1 Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 This 30-day limit applies to a pen in use, which is relevant if you hold a strength for a while and finish pens at a steady pace.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.1 Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.

09 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
