Yes, you can reach 10mg from 5mg, but the licensed route is stepwise rather than a direct leap. After 5mg, the dose can step up by 2.5mg at a minimum of four-week intervals, giving 7.5mg and then 10mg, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 So a typical path is 5mg, then 7.5mg, then 10mg.

Whether you move at all is a clinical decision. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber guides the right pace, based on your progress, your side effects and how you feel on your current pen.1

What to decide before you step up

The useful question is not whether 10mg is allowed, but whether you need it. If you are losing weight steadily, feeling in control of your appetite and tolerating 5mg well, your prescriber may be happy for you to stay where you are. If progress has stalled and side effects are mild, a step to 7.5mg is the natural next conversation.

Before any change, note your current side effects, how your appetite behaves across the week, and your recent trend rather than a single weigh-in. These make the review far more productive. 04 · How the dose ladder works

How the Mounjaro dose ladder works

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After that, the dose increases to 5mg, and can then step up by 2.5mg at a minimum of four-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly.1

The design is deliberate. Small steps give your body time to adjust to each strength before the next increase, which is why the ladder is not built for leaps. The word "minimum" matters: four weeks is the earliest a step can happen, not a target. Plenty of people stay longer on a dose because it is working well or because they want to feel settled first.

Each KwikPen contains four weekly doses, so a pen at each strength covers a four-week stretch.13 That is why a step usually coincides with starting a new pen.

05 · Why not skip strengths

Why the ladder steps rather than jumps

The ladder moves in 2.5mg steps because tolerability is the limiting factor, not the medicine's ability to work. Mounjaro slows how quickly the stomach empties and affects appetite, and the commonest early effects are mild nausea, altered appetite and tiredness in the days after each injection.1 Bigger steps risk making those effects harder to live with.

It also helps to think about what a jump would do in practice. You would move from a dose your body knows well to a markedly higher exposure in a single injection. That is a different experience from a gentle 2.5mg change, and the licence simply does not describe it as a route.

06 · Do higher doses mean more weight loss

Do you lose more weight on higher doses?

In the main trial, average results rose with dose, but the gaps are not the whole story. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The same trial reports how many people reached meaningful loss at each strength. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 So 5mg already delivered meaningful loss for a large majority of those taking it.

What does this mean for your decision? A higher dose is not a promise of a bigger result. Trial averages do not predict your own result, and the benefit of each step has to be weighed against how you feel. Someone who loses steadily on 5mg may gain little from moving up, while someone whose progress has plateaued may reasonably discuss it. Your prescriber reviews that trade-off with you.

Finally, remember pace. Weight loss on Mounjaro is gradual by design, and most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1

07 · How long on 5mg

How long should you stay on 5mg?

You stay on 5mg for at least four weeks, and after that for as long as your prescriber and you agree it suits you. There is no fixed upper limit, because many people settle at a maintenance dose of 5mg, 10mg or 15mg.1

Signs that 5mg is doing its job include a steady downward trend, a quieter appetite between meals and side effects that have eased. Signs it may be worth a conversation include a long plateau despite sticking to your diet and activity plan, or an appetite that has crept back. Neither is a failure; both are normal points to review.

Treatment response is formally reviewed too. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That review is about your tolerated dose, which is not always the highest one on the ladder.

Do not rush to climb because of what other people post online. Everyone's body adjusts differently, and the early weeks in particular vary widely: some people notice appetite change straight away, others feel little until a higher dose, and both are normal.

08 · Going back down

Can you go from 10mg back to 5mg?

Yes, a step down is possible, but it should be agreed with your prescriber rather than made on your own. That is the safe route if 10mg feels like too much.

Common reasons to discuss stepping down include persistent nausea, tiredness that does not settle, or an appetite that has become too suppressed to eat well. Tell your prescriber what you are feeling, how long it has lasted and when in the week it is worst. That detail helps them decide between holding the dose, stepping down, or changing nothing.

What not to do is improvise. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

09 · Side effects when changing dose

What to expect on 10mg and after a dose change

Expect a short adjustment after any step up. The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and these usually settle.1 Many people choose a weekend morning to inject so early side effects fall on days off.

There is no best time of day to inject. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week. Injection is under the skin: the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection, rotating the site with each dose.13

If a symptom is new, severe or not improving, contact your prescriber instead of waiting it out. If you cannot keep fluids down, contact your clinical team, as dehydration needs medical help straight away. Practical points do not change with dose. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13

10 · Dosing schedule questions

Can you change the schedule instead of the dose?

Mounjaro is licensed as a once-weekly injection, so the schedule is not something to bend.

You can change your weekly injection day if needed, as long as there are at least 3 days between two doses.13 That is the one flexible part of the routine, and it is useful if your week changes.

Likewise, a pen that has been in use should be discarded after 30 days (not above 30°C) out of the fridge, even if medicine remains. The same consistency matters when you travel: carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription.13

Staying on schedule makes each dose comparable, which in turn makes it easier for you and your prescriber to judge whether a step up, a hold or a step down is the right call.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
