Can I Go From 10mg to 2.5mg Mounjaro? (October 2026)
Going from 10mg straight down to 2.5mg is not a standard step on the licensed Mounjaro ladder, and it is a decision for your prescriber, not something to do alone. 01 · The core answer
Can you go from 10mg to 2.5mg Mounjaro?
You can only do it if your prescriber agrees it is right for you, and it is not a routine move. The licensed ladder starts everyone on 2.5mg and then steps up by 2.5mg at a time, depending on how you respond and tolerate treatment.1 Nothing in that ladder describes a jump from 10mg back to the starting dose.
What the licence does allow is flexibility. Your prescriber can hold a dose longer, or step down within the licence, if side effects are the concern.1 A step down might be a single strength lower, such as 7.5mg, rather than a return to the very start. That judgement depends on your symptoms, your history and how long you have been on treatment, so it has to be made by someone who has your details.

03 · Why people ask
Why people ask about a big step down
The most common reason is side effects at a higher dose. Nausea, a very strong drop in appetite and tiredness can all be harder to manage as the dose rises. It is natural to think that going back to the beginning will make things easier.
Other reasons are different. Some people have reached a weight they are happy with and wonder whether they need such a high strength. Others have had a gap in treatment, perhaps through illness, travel or a delayed order, and worry about restarting at their old dose.
All of these are reasonable questions, and the answer differs for each. The licensed approach is for the prescriber to guide the dose, because they can weigh your response and tolerance together.11 A general rule from a website, however well meant, cannot do that.
It also helps to say what is going wrong in specific terms. Note when symptoms start after the injection, how long they last, and whether they are easing or getting worse. That detail lets a prescriber choose between holding the dose, stepping down a little, or looking at other causes.
04 · What the licence says
What the licensed dose ladder says
Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
Those increases are minimums, not targets. Your prescriber can move more slowly than the fastest pace, and can hold you where you are. That is exactly why a person on 10mg who is struggling is not forced to choose between pushing on and stopping.
Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 If 10mg is working and tolerable, there may be no reason to change it. If it is not, a modest step down is a more natural conversation than a return to 2.5mg.

05 · Using a 10mg pen
Can you take 2.5mg from a 10mg pen?
No. A Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window, and you confirm the dose by the number in the window, never by counting clicks.13 Each pen contains four weekly doses, and the pen will not let you dial past the medicine left inside.
If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13 That is why questions about how many clicks give a lower dose from a stronger pen have no safe answer. The pen is designed around fixed doses, not partial ones.
Trying to pull a smaller amount from a stronger pen also takes you outside what has been tested. Taking smaller or more spread-out doses than the pen is designed to deliver is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1 It means unknown dosing accuracy and no safety data.
Likewise, combining lower-strength doses to build a higher one is not a licensed approach. If you need a different strength, the safe route is a new pen of that strength, prescribed for you. 06 · After a gap in doses
Missed doses and breaks in treatment
A short delay has a clear rule. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
Longer breaks are different. What to restart on after several missed weeks is a clinical decision. Contact your prescriber or pharmacist before the next injection, tell them how long it has been and what dose you were on, and let them advise. Do not simply resume at the old dose or drop to the lowest one on a guess.
You can change your injection day if needed, as long as there are at least 3 days between two doses.1 If a rearranged schedule is behind the gap, that is worth mentioning too.
Finally, if you have stopped altogether, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears.
07 · Staying on 2.5mg
Can you stay on 2.5mg and still lose weight?
You may lose some weight, but 2.5mg is designed as an adjustment dose, not a long-term plan. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1
Experience at the start differs widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1
Trial figures help frame what higher doses can do, though they are averages. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

08 · Judging your progress
Judging progress before you change a dose
Weight loss on Mounjaro is gradual by design, so a single slow week is rarely a reason to change anything. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1
Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Notice the wording: the highest tolerated dose. Tolerability is part of the plan, which is why stepping down for side effects is a recognised conversation rather than a failure.

09 · Safety and reporting
Side effects, reporting and NHS access
If a symptom worries you, speak to your prescriber, pharmacist or GP promptly rather than waiting to see whether a lower dose helps. For any serious symptom, get medical help straight away. You can also report suspected side effects through the MHRA Yellow Card scheme.19
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
