01 · The answer

Can you get 5 injections out of a Mounjaro pen?

No, you cannot get a fifth injection from a Mounjaro pen. Each KwikPen contains four weekly doses, and that is how the product is licensed, labelled and dispensed.1

That design is the safety feature. When four doses have been taken, the pen has done its job. If you try to force more out of it, you move outside what the pen was built and tested to do.

02 · Cost

What a pen costs and what it covers

One pen covers four weekly doses, so four weeks of treatment at your current strength. The standard price of the Mounjaro pen at 2.5mg is £169.99, and 5mg is £218.99. The stronger pens are £279.99 at 7.5mg, £290.99 at 10mg, £319.99 at 12.5mg and £329.99 at 15mg. All six doses are in stock.

Broken down by week, that is £42.50 at 2.5mg and £54.75 at 5mg. A new customer's first order is £40 less with code FIRST40, at any dose, and every later order is £25 off the standard price (code DOSE25).

Planning the full course helps too. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year (13 pens) costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg, again before discounts.

03 · Getting a pen

How ordering a pen works

Ordering has three steps. First, complete a free online consultation, a short health questionnaire with no charge and no obligation. Second, your answers are reviewed by a GPhC-registered prescriber; payment is taken when you place the order, and if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. 04 · How the pen works

How the KwikPen dial works

The Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

The right move is to start your next pen on your usual injection day, not to combine what is left with another injection.

05 · Timing and missed doses

Timing your doses: gaps, missed doses and 5 days apart

Mounjaro is injected once a week, on the same day each week, at any time of day, with or without food.13 There is no best time of day: what matters is keeping to the same day. Many people choose a weekend morning so early side effects fall on days off.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on as usual. If more than 4 days have passed, skip the missed dose and take your next one on the normal day. Never take two doses within 3 days of each other.1

That rule answers whether it is okay to inject 5 days apart. A gap of 5 days is more than 3 days, so it does not break the minimum-spacing rule, but it does change your schedule. You can change your injection day if at least 3 days (72 hours) have passed since your last dose.1 Do this deliberately and tell your prescriber.

The same ceiling answers whether you can inject two doses of 5mg: you cannot take two doses within 3 days of each other. Doubling up is not a way to catch up.

06 · Stepping up and breaks

Moving to 5mg and taking breaks

Everyone starts Mounjaro on 2.5mg once a week, and the starting dose is about letting your body adjust rather than weight loss itself. After at least 4 weeks the dose rises to 5mg. So starting 5mg after only 3 weeks is earlier than the licensed schedule allows.111

From there the dose can step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on your response and tolerability. Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.1

What about a break? Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear your system. There is no way to speed this up, and nothing needs doing while it clears.1 Whether a short break is right for you, and what dose to restart on, is a decision for your prescriber, so speak to them before pausing.

07 · Is 5mg strong?

Is 5mg of Mounjaro strong?

5mg is the first step up from the 2.5mg starting dose and is a recognised maintenance dose. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide with diet and exercise lost on average 15% of their body weight on 5mg over 72 weeks, compared with 3.1% with lifestyle changes alone. At 72 weeks, 85% of participants on 5mg had lost at least 5% of their body weight.2

These are trial averages, and individual results vary. For someone starting at 100 kg, 15% is about 15 kg. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

08 · Slow progress

Not losing weight after 4 weeks?

Slow progress after 4 weeks is common and is not a reason to alter your dose. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.

Some people notice appetite change and early loss in week one, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so judge progress by the trend over weeks, not single weigh-ins. Trials measured results at 72 weeks, so think in months, not days.2

Your response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.1

09 · If your pen seems short

If your pen seems to give fewer than four doses

If you only seem to be getting three doses, stop and check before the next injection. Each pen is meant to contain four weekly doses, so fewer is worth raising with the pharmacy.13

  • Do not make up the difference.
  • Keep the pen. Do not throw it away, so the pharmacy can look at it.

Dispensed prescription medicines cannot be returned except for a faulty, damaged or wrong item, and you need to contact us within 3 days. So raise any concern promptly.

10 · Storage and pen life

Storage, pen life and counting your four weeks

Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the cap on to protect it from light, and never use a pen that has been frozen.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.13

Travelling is straightforward: carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription. Hold luggage can fall below freezing.

11 · Who it is for

Who Mounjaro is licensed for, and NHS access

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used with a reduced-calorie diet and increased physical activity, and a prescriber assesses your BMI, medical history and current medicines.1

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions. Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment.45

What happens if I stop taking Mounjaro for two weeks?
Missing doses for two weeks goes well beyond the missed-dose rule, which allows a late dose only within 4 days (96 hours). Tell your prescriber, who will advise on restarting and which dose to use. Because tirzepatide clears slowly, over roughly 4 to 5 weeks, your prescriber will want to know how long the gap was.
Is it okay to take Mounjaro 5 days apart?
Five days is more than the 3-day minimum, so it does not break the spacing rule, but you should keep to a consistent weekly schedule. You can change your injection day if at least 3 days (72 hours) have passed since your last dose. Never take two doses within 3 days of each other.
I'm not losing weight on Mounjaro after 4 weeks. What could be causing this?
Four weeks is early. The first weeks on 2.5mg are an adjustment phase, and many people see modest losses until 5mg onward. Early changes include water weight, so judge the trend over weeks. Your prescriber reviews response, and if you have lost less than 5% after 6 months on the highest dose you can tolerate, they will discuss next steps.
