The single biggest lever you control with your prescriber is the pace of your dose increases. Everyone starts Mounjaro on 2.5mg once a week. That starting dose is about letting your body adjust, not about weight loss itself.1 It is tempting to see it as wasted time. It is not: it is how the treatment is built to be tolerated.

After at least 4 weeks the dose increases to 5mg. From there it can step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Note the word minimum. Four weeks is the earliest a step can happen, not a target you must hit. If you are tolerating a dose well and your progress is steady, staying there is a legitimate choice.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 In SURMOUNT-1, the average loss was 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, against 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2 The same trial found that 85% of people on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight at 72 weeks.2 So a lower dose still produced meaningful loss for most participants.

A useful way to think about dose is as a balance between benefit and tolerability. A dose you can tolerate and stay on, month after month, with a diet and activity routine you can keep, will serve you better than a higher dose you dread. Equally, if you have plateaued at a dose and your prescriber feels you could safely benefit from a step up, that conversation belongs in your monthly review rather than being decided alone.

05 · Build a weekly routine

Build a weekly routine you will actually keep

Consistency is the cheapest improvement available: pick one day each week and inject on it every time. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day. It works the same in the morning or the evening. What matters is that you can remember the day, so choose a day and time that fit your life and stick to them. Many people choose a weekend morning so that early side effects fall on days off.

Rotate the injection site with each dose. What if you miss a dose? Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

What if your week changes? You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 A holiday, a shift pattern or a new job are all reasons you might move it. Make the change deliberately, once, rather than drifting from day to day.

Things go wrong occasionally. 06 · Food and activity

Mounjaro is licensed to be used alongside a reduced-calorie diet and increased physical activity, and that is where much of your own influence lies.1 The SURMOUNT-1 results were produced in people taking tirzepatide alongside diet and exercise, and treatment works alongside them, not instead of them.2 If a page tells you the injection alone will do the job, it is overselling.

Tirzepatide helps regulate appetite and slows how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals.1 That makes a reduced-calorie diet more achievable than it was before. The sensible response is to use that window to establish habits: regular planned meals, food you actually enjoy, and a way of eating you could imagine keeping for years.

On activity, the licence says increased physical activity, so the aim is to do more than you do now, gradually. Walking more, taking stairs, a class you enjoy, anything you can repeat week after week. A modest routine you keep beats an ambitious one you abandon after a fortnight.

Resist the urge to treat the medicine as permission to skip meals. Very low intake is not what the treatment is designed for, and if you find you can barely eat, or you feel unwell, tell your prescriber rather than pushing through. 08 · Side effects

The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.1 Knowing that in advance makes them easier to ride out.

Planning helps. Choosing an injection day when you can take things easy, such as a weekend morning, means the days after the dose fall when you have flexibility. Eating when you are genuinely hungry, rather than out of habit, and paying attention to how full you feel are the sort of everyday adjustments many people find natural once appetite changes.

Do not fix side effects yourself by altering the dose. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

Store and travel with your pen properly

Handling your pen correctly protects the medicine you have paid for. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

For unopened pens, store them in the fridge until first use; they keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.

When your order arrives, it comes in temperature-controlled, discreet packaging. Put the pens into the fridge promptly unless you are about to start one. Only one pen is in use at a time, and the in-use clock starts when you begin using it, so note the date you start each pen on the carton or in your tracking app.

Travelling is straightforward with some planning. You can fly with Mounjaro. If your injection day falls during travel, you do not necessarily need to skip: you can inject abroad as normal if you have the pen with you. Why does this matter for results? A pen that has been frozen or stored badly is a pen you cannot safely use, and replacing it means an interrupted schedule and extra cost. No. There is no best time of day: Mounjaro works the same in the morning or the evening, with or without food. What matters is injecting on the same day each week, at a time you will remember. Many people choose a weekend morning so that early side effects fall on days off. If you need to change your injection day, make sure at least 3 days (72 hours) have passed since your last dose.
What should I do if I miss a dose?
Take the missed dose as soon as you can, provided it is within 4 days (96 hours) of the scheduled time, then carry on with your usual injection day. If more than 4 days have passed, skip that dose entirely and take your next one on your normal day. Never take two doses within 3 days of each other. If you are unsure, contact the prescribing team before injecting.
How long does it take to see results on Mounjaro?
It varies. Some people notice an appetite change and early loss in the first week; others feel little until a higher dose, and both are normal. Most measurable progress builds from 5mg onward, month by month, and trials measured results at 72 weeks. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether to continue, adjust or stop.
How do I store Mounjaro so it stays usable?
Keep pens in the fridge at 2–8°C and never freeze them. Once in use, a pen can stay out of the fridge not above 30°C for up to 30 days, then it should be discarded. Keep the cap on to protect it from light, and never use a frozen pen. Unopened pens keep until the printed expiry date; if one has been out of the fridge, check with the pharmacy before using it.
