If you did hold several pens at once, storage would become your responsibility, so it is worth knowing the rules. Unopened pens are stored in the fridge until first use, and they keep until the printed expiry date.13 For any question about how long an unopened pen can be out of the fridge, check the patient information leaflet and ask the pharmacy rather than guessing a number of days.

Once a pen is in use, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.

At six months you are typically past the lower rungs of the ladder, but the trial headline figures were measured at 72 weeks, not 26. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks.2

For context on the longer picture, at 72 weeks 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Beyond the scales, weight loss on tirzepatide came in trials with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management.

11 · NHS and private routes

NHS or private: where six months of Mounjaro comes from

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6

On the NHS, supply is prescribed through your clinical team in the usual way, and a prescription charge of £9.90 in England may apply per item unless you are exempt. 12 · Counterfeit and bulk-buy risks

Counterfeit risks when people try to stock up

The biggest danger of hunting for a six-month supply is ending up with a fake pen. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation, so any seller offering to supply without a consultation or prescription is a warning sign, however attractive a bulk offer sounds.7

Do not use a pen that looks cloudy or discoloured, has particles in it, or has been frozen. If anything seems wrong, contact the pharmacy before injecting, and report any suspected falsified product or side effect through the MHRA Yellow Card scheme.7

Reselling is another trap. Buying leftover pens from individuals, including people who stocked up and changed plan, means you cannot verify the storage history, the batch or the dose. A pen's cold chain cannot be proven after the fact, and a pen with an unknown history is not worth the saving.

The safer rule is simple: small, verified, reviewed orders from a registered pharmacy. If a deal is only available as a large bundle and you cannot confirm the seller's registration, walk away.

13 · Stopping or pausing safely

What if you stop before you have used what you bought

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

People stop for many reasons: side effects, a change in health, reaching their goal, or cost. If you become pregnant, stop the medicine and tell your prescriber promptly; Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you stop and then restart after a gap, your prescriber decides where to resume. You should not assume you can restart on a strength you held in stock. A pre-bought set of pens at higher strengths could be unsuitable after a break, and a prescriber needs to reassess.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Can I get six months of Mounjaro on the NHS?
NHS supply is managed by your clinical team, not bought in bulk. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.4 Ask your GP what is available locally.
What happens if I stop Mounjaro after buying several pens?
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system, and nothing needs doing while it clears.1 In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, so stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. If you restart after a break, your prescriber decides the starting dose, so pens you already hold may not be suitable. Dispensed prescription medicines cannot be returned except where faulty, damaged or wrong, which is why ordering in stages lowers your risk.
