01 · The short answer

Can you get Mounjaro on the NHS for type 2 diabetes?

Mounjaro can be prescribed on the NHS, but not everyone with type 2 diabetes will be offered it, and availability depends on where you live. Tirzepatide is licensed for type 2 diabetes, where it improves blood sugar control (HbA1c) and supports weight loss. NICE has a dedicated appraisal, Tirzepatide for treating type 2 diabetes.⁸

The wider NHS picture is phased. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).^{4 6} Type 2 diabetes is one of the weight-related conditions that can count towards that route.

02 · Two routes

Diabetes route versus weight-management route

The NHS can reach Mounjaro through two doors, and they are not the same. One is treatment of type 2 diabetes itself. The other is weight management, where NICE recommends tirzepatide for adults with a BMI of at least 35 plus at least one weight-related condition.⁴

Do not stop or change any diabetes medicine yourself. If you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar, and that is a decision for your clinician.

03 · If the NHS isn't available yet

The private route if you do not qualify yet

What Mounjaro is and how it works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.¹ It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹³

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.¹ This dual action distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). Most people feel fuller sooner and less hungry between meals.

05 · NHS rollout so far

Where the NHS rollout stands

NHS access to Mounjaro is phased, and full rollout is expected to take years. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.^{4 6} That means two people with the same diagnosis can get different answers depending on timing and local service.

06 · Ethnic background and BMI

Does your ethnic background change the BMI threshold?

It can. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight-related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups. Mention your background to your GP or prescriber, as it may affect how your BMI is read.

07 · Weight loss expectations

How much weight can you lose, and how fast?

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures; a sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. In SURMOUNT-1, which involved 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages and individual results vary.

As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.² Your prescriber can set realistic expectations for you.

08 · Who should avoid it

Who should not take Mounjaro

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.¹ Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.¹³

In rodent studies tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials. Tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2.¹ Always share your full medical history and medicine list. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
