01 · The short answer

Can you get Mounjaro on the NHS if you have PCOS?

You can get it on the NHS only if you meet the NHS weight-management criteria, not because you have PCOS. Mounjaro is licensed for weight management in adults and for type 2 diabetes, and is not licensed specifically for PCOS.1 A PCOS diagnosis does not by itself open the door.

That said, many women with PCOS do meet the licensed weight-management criteria, and weight loss is a first-line recommendation for PCOS that can improve insulin resistance, cycles and symptoms.11 So the practical question is whether your BMI and health conditions fit. 03 · Private option

If you do not qualify on the NHS yet

If you do not meet the current NHS phase, regulated private treatment is the other route. The licensed criteria are a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 If you are from a South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean background, thresholds are usually reduced by 2.5 kg/m².4

Why weight matters in PCOS

Weight loss is a first-line recommendation for PCOS and can improve insulin resistance, cycles and symptoms.11 That is why many women ask about Mounjaro, even though it is not licensed for PCOS itself.1 Eligibility is assessed on the standard BMI criteria rather than on the PCOS diagnosis.

06 · What results look like

What the trial results show

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 For someone starting at 100 kg, 20.9% is about 21 kg.

These are trial averages and individual results vary. SURMOUNT-1 was not a PCOS study, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Progress is gradual: the first weeks on 2.5mg are an adjustment phase, and most measurable progress builds from 5mg onward.1

07 · Pregnancy and fertility

Fertility, pregnancy and PCOS

Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.13 This matters in PCOS, because improving weight can restore ovulation, so tell your prescriber about any fertility plans at consultation.11

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Whether you apply through the NHS or privately, share your full medical history and medicine list.

08 · Who should avoid it

Who should not take Mounjaro

You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.13

Tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials.1

09 · Starting dose and pace

How treatment starts

Everyone starts on 2.5mg once a week, which is about letting your body adjust rather than weight loss itself. After at least 4 weeks the dose rises to 5mg, then can step up by 2.5mg at minimum 4-week intervals to a maximum of 15mg weekly.1 Higher is not automatically better.

Which is better for PCOS, Mounjaro or Ozempic?
Neither is licensed specifically for PCOS. Ozempic is semaglutide licensed for type 2 diabetes only, not a UK weight-loss medicine; the licensed weight-management options are Mounjaro and Wegovy. Mounjaro activates two gut-hormone receptors, GIP and GLP-1, while semaglutide (Ozempic, Wegovy) is single-action. Your prescriber can discuss which licensed option suits your health history and goals.
