What the BNF cost of Mounjaro actually tells you

The British National Formulary (BNF) entry for tirzepatide gives indications, dosing and safety information, plus an NHS indicative price for each pen strength.¹¹ That indicative price is a guide to the list price the NHS is expected to pay. It is not a retail price, it is not what you pay, and private pharmacies set their own prices.

What matters for your wallet is the route. If Mounjaro is prescribed through the NHS and you pay prescription charges in England, the charge is £9.90 per item. If you are prescribed it privately, the pharmacy's own price applies. There is no single "BNF cost" that you pay in either case.

What private Mounjaro costs per month

The Mounjaro pen is priced by strength. One pen is four weekly doses, so the pen price is your monthly cost. At the starting 2.5mg dose a pen costs £169.99, and the higher strengths step up to £189.99 at 5mg, £244.99 at 7.5mg, £274.99 at 10mg, £289.99 at 12.5mg and £299.99 at 15mg. These are standard prices, and all six doses are in stock.

Planning a first year

Treatment starts low and steps up, so the cost rises over the first months. This table uses standard prices before any discount.

A new customer's first order is £40 less, and every later order is £25 less than these standard figures. For a maintenance dose of 7.5mg, 10mg or 12.5mg, your monthly cost is that dose's price: £244.99, £274.99 or £289.99 before the reorder discount.

Your prescriber decides whether and when to step up. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.

What is the NHS indicative price in the BNF?

The NHS indicative price in the BNF is a reference figure for what the NHS is expected to pay for a pack of a medicine, listed alongside its dosing and safety information.¹¹ It helps commissioners, pharmacists and prescribers compare treatments. It is not an offer price, and it does not tell a patient what they will pay.

A person reading the BNF sees a figure for a pen and assumes it is the cost to them. In practice there are three separate numbers: the list price the NHS reference uses, the NHS prescription charge a patient may pay, and a private pharmacy's retail price.

Eli Lilly raised the UK list price of Mounjaro effective September 2025, which is the price that NHS reference figures are built around.¹⁰ Private pharmacies buy at confidential terms and set their own sell prices, so private prices differ between providers even for the same dose.

Where you can see a BNF price, treat it as context, not a quote. Where you want a real price, look at the pharmacy's own live price, and check that the pharmacy is on the GPhC register before ordering.

What the BNF entry is good for

The BNF is most useful for clinical information rather than price. What you pay on the NHS

If Mounjaro is prescribed to you on the NHS and you pay prescription charges in England, the charge is £9.90 per item. That is a flat charge per prescribed item, and it is separate from the list price the NHS pays. Whether you are prescribed it at all depends on NHS eligibility and local access.

06 · NHS access and eligibility

How much does Mounjaro cost on the NHS, and who can get it?

On the NHS in England, the patient cost is the standard prescription charge of £9.90 per item for those who pay charges, but access is limited by phasing. Mounjaro is available on the NHS, but access is being phased in over several years.⁴⁶

NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.⁴ The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or above and four or more qualifying conditions.⁶ Full rollout is expected to take years.

That is why a person can meet the NICE criteria and still not be offered treatment today. Your GP or local weight-management service is the best first call.

Why people compare NHS and private routes

The NHS route is the lowest-cost option for those who qualify and are offered a place. The private route is for adults who meet the licensed criteria but who do not want to wait for the rollout to reach them. Neither route is right for everyone, and the honest answer is to ask your GP what is available locally before deciding.

What NHS and private treatment share

Both routes use the same medicine, taken once weekly, alongside a reduced-calorie diet and increased physical activity. Both involve assessment by a prescriber before treatment starts. The difference is who pays for the pen and how quickly you can start, not what the pen is.

Do not assume a private route changes the clinical rules. A prescriber still looks at your BMI, medical history and current medicines before deciding whether Mounjaro is suitable for you, and may decline to prescribe.

07 · Private licensed eligibility

Who can be prescribed Mounjaro privately?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.¹ Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.

These licensed criteria are wider than the NICE criteria for NHS access, which is the main reason some people look at private supply at all. A private prescription does not lower the clinical bar, though. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.

What the assessment looks at

What does the cost of Mounjaro pay for in terms of results?

The cost pays for a once-weekly treatment that, in trials, produced substantial average weight loss alongside diet and exercise. In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.²

These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.² At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.²

A worked example

For someone starting at 100 kg, 20.9% is about 21 kg, and 15% is about 15 kg. That is straight arithmetic on the trial averages, not a prediction for you. Trial averages do not predict your own result, and your prescriber will review your progress at regular check-ins.

What the weekly medicine is

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

09 · How tirzepatide works

How does Mounjaro work, and why does it matter for cost?

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.¹ Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

The weekly rhythm and the half-life

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.

Each KwikPen is designed and licensed to deliver exactly four weekly doses. That is why one pen equals one month of treatment, and why the pen price is the monthly cost.

Why one pen is a month

Pens are sold by strength rather than by dose count because each pen carries four weekly doses at one strength. When your prescriber steps you up, you move to the next strength pen. You do not buy partial pens or top up existing ones.

Using the pen as designed

Follow the pen's instructions each week. In use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away; do not use a pen that has been above 30°C or frozen.¹³ Our pharmacy team can answer storage questions by phone or email during support hours.

10 · Counterfeit pens and verification

How do I avoid fake Mounjaro when looking for a low price?

Only buy from a pharmacy you can verify on the GPhC register, and never from social media, beauty salons or unverified websites. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

Genuine treatment always involves a prescription and a clinical consultation. Anyone offering to supply without a consultation or prescription is not operating legitimately, whatever the price.

What a genuine pen looks like

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C or out of the fridge for more than 30 days in use.¹³

11 · Three months, plateaus and duration

What can you expect after three months, and how long can you stay on Mounjaro?

Three months is within the early titration period, so the cost and the result are both still changing. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices before discounts. The SURMOUNT-1 results are measured over 72 weeks, not three months, so they do not tell you what to expect at 12 weeks.²

Individual results vary, and trial averages do not predict your own result. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

If weight loss slows or stops

If your weight loss stalls after about three months, speak to your prescriber rather than changing the dose yourself. Your reviewer can look at your current dose, how treatment is going, your diet and activity, and any side effects.

13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
