What GLP-1 pills are and how they work

GLP-1 weight loss pills are once-daily tablets that act on the GLP-1 gut-hormone pathway to support weight management. They are not the same as over-the-counter diet pills or supplements, and they are not stimulants. Two are now licensed in the UK, and both are prescription-only.2021

GLP-1 activation helps regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Tablets are an oral way of delivering a GLP-1 medicine rather than a different kind of treatment. Wegovy tablets contain semaglutide, the same active ingredient as the Wegovy injection. Foundayo contains orforglipron, a once-daily GLP-1 tablet made by Eli Lilly.2021

Two GLP-1 weight-loss tablets are now licensed in the UK: Wegovy tablets (oral semaglutide, made by Novo Nordisk), approved by the MHRA in June 2026, and Foundayo (orforglipron, made by Eli Lilly), approved in August 2026. They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only.

That last point matters when you read online lists of "best GLP-1 pills". Many mix licensed medicines with supplements, compounded products and "research" peptides sold with dramatic weight-loss claims. Only licensed products have been through regulatory review of quality, safety and effectiveness. If a seller offers any of these without a consultation or prescription, treat that as a warning sign.

It also matters that medicines work as part of a plan. Every licensed GLP-1 weight-management medicine is intended for use alongside a reduced-calorie diet and increased physical activity. Individual results vary, and trial averages do not predict your own result. A tablet does not remove the need for sensible eating, protein, movement and follow-up with your prescriber, and good services build monthly reviews around exactly that.

Finally, remember what a tablet is not. There is no tablet form of Mounjaro, because tirzepatide is injection-only in the UK. If you see a "Mounjaro pill" for sale, it is not the licensed medicine, and you should not buy it. 05 · Wegovy tablets

Wegovy tablets (oral semaglutide)

Wegovy tablets contain semaglutide, taken as one tablet once a day, and they are the oral version of a medicine many people already know as an injection.20 They are licensed for weight management in adults with a BMI of 30 or above, or 27 to under 30 with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity.

The dosing is stepped to help your body adjust. Treatment starts at 1.5mg once daily and steps up to 4mg, 9mg and then the 25mg maintenance dose, with at least one month at each dose; the dose can stay at an earlier step if needed.20 That flexibility is useful: moving up is guided by how you tolerate each step, not by a fixed deadline.

The morning routine

The routine is the part that defines this tablet. The tablet must be taken on an empty stomach after fasting for at least 8 hours, swallowed whole with a sip of water (up to half a glass, about 120ml), and you then wait at least 30 minutes before eating, drinking or taking other tablets.20 In practice this suits people with a predictable morning who can take it, wait, and then have breakfast and any other medicines.

If a dose is missed, skip it and take the next tablet the following day.20 That simple rule is worth remembering, because the strict timing can make a missed morning easy to fix incorrectly.

Moving from the injection

People on the 2.4mg weekly Wegovy injection can move to the 25mg tablet, starting one week after their last injection.20 That change should happen under prescriber supervision. Never run two GLP-1 medicines together.

What the trial showed

In the OASIS 4 trial, adults taking semaglutide 25mg tablets for 64 weeks lost on average 13.6% of their body weight, compared with 2.2% on placebo.22 76.3% lost at least 5% of their body weight (vs 30.5% on placebo) and 59.8% lost at least 10% (vs 14.1%).22 These are trial averages. Individual results vary, and the trial should be read alongside a reduced-calorie diet and increased activity.

Side effects

The most common side effects are digestive — nausea, vomiting, diarrhoea, constipation, indigestion and stomach pain — along with headache and tiredness; gallstones and hair loss are listed as common.22 Tell your prescriber about any symptom that worries you, and report suspected side effects through the Yellow Card scheme.19

Who it tends to suit

Wegovy tablets tend to suit people who prefer to avoid needles, have a steady morning routine and are comfortable with a strict fasting window. They may be less convenient if you work early shifts, take several morning medicines or find it hard to wait before drinking tea or coffee. 06 · Foundayo

Foundayo (orforglipron)

Foundayo is a once-daily GLP-1 tablet that contains orforglipron and is made by Eli Lilly, the maker of Mounjaro.21 Its main practical advantage is convenience: unlike Wegovy tablets it can be taken at any time of day, with or without food and without water restrictions; the tablet is swallowed whole.

It is licensed for weight management in adults with a BMI of 30 or above, or 27 to under 30 with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity.21 As with every licensed GLP-1 medicine, it is prescription-only and you need a consultation reviewed by a prescriber.

Dosing

Treatment starts at 0.8mg once daily, then steps up to 2.5mg and 5.5mg, and then to 9mg, 14.5mg or 17.2mg depending on response and tolerability, with at least 30 days at each dose; 17.2mg once daily is the maximum.21 The top dose is therefore a ceiling, not a target. Many people settle where they get a good response with side effects they can manage.

If a dose is missed, take it as soon as possible, but never more than one tablet a day.21 This is more forgiving than the Wegovy tablet rule, which tells you to skip a missed dose.

Contraception

Foundayo may make oral contraceptive pills less effective: switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase.21 Discuss this with your prescriber before you start, and say if you are planning a pregnancy.

What the trial showed

In the ATTAIN-1 trial of 3,127 adults without type 2 diabetes, people taking the highest dose of orforglipron for 72 weeks lost on average 12.4% of their body weight, compared with 0.9% on placebo; the two lower doses gave 7.8% and 9.3%.23 The trial used capsules, and the UK 17.2mg tablet corresponds to the highest trial dose, so the tablet and capsule doses are not comparable milligram for milligram.

That caveat is important. The trial used a different form of the medicine, so you should not translate trial doses directly into the UK tablet strengths beyond what the product information says. Results are averages, and individual results vary.

Side effects

The most common side effects are digestive — nausea, constipation, diarrhoea, vomiting, indigestion and stomach pain.23 In the trial, 10.3% of people on the highest dose stopped because of side effects, compared with 2.7% on placebo.23 If side effects are troubling, speak to your prescriber before stopping, because staying at an earlier dose may be an option.

Who it tends to suit

07 · Pills vs injections

GLP-1 pills versus injections

Whether a pill or an injection is better depends on your priorities, and no trial has compared Mounjaro directly with either tablet. Mounjaro is a once-weekly injection; Wegovy tablets and Foundayo are once-daily tablets.2021

Results from separate trials, with different people, durations and doses, should not be set against each other as if they were head to head. SURMOUNT-5 is the trial that compared tirzepatide directly with injected semaglutide.

The one direct comparison we have

In the first large head-to-head trial (SURMOUNT-5), adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15 Both are effective, licensed treatments; which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone.

This comparison is between two injections. It does not tell you how a tablet performs against either, so use it to understand the injectable choice rather than to rank pills.

Practical differences

The practical differences matter as much as the averages. Wegovy tablets need a strict empty-stomach routine every morning, Foundayo has no food or water rules, and Mounjaro needs one injection a week.2021 Some people dislike needles and will always prefer tablets. Others find a weekly injection easier than remembering a tablet every single day.

Think about the following when you decide:

  • Do you want a daily habit, or a once-weekly task?
  • Can you fast and wait before breakfast and other medicines?
  • Are you comfortable with a pen injection, with needles and a sharps bin supplied?
  • Do you take other medicines that affect timing?
  • Would you rather stay with a medicine you already know?

How Mounjaro works differently

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide.1 It is a mechanism difference, not a promise about your individual result.

If you are already on an injection

People on the 2.4mg weekly Wegovy injection can move to the 25mg tablet, starting one week after their last injection.20 Moving between other GLP-1 medicines is done under prescriber supervision. There is no licensed direct conversion between semaglutide and tirzepatide, typically the previous medicine is stopped, a washout interval observed, and Mounjaro started at a dose your prescriber judges appropriate.

08 · Safety and side effects

Side effects and safety of GLP-1 pills

The side effects of GLP-1 pills are mostly digestive, and they appear in both licensed tablets. For Wegovy tablets, the most common side effects are digestive — nausea, vomiting, diarrhoea, constipation, indigestion and stomach pain — along with headache and tiredness; gallstones and hair loss are listed as common.22

Pills compared with injections

A common question is whether pills cause fewer side effects than injections. Different trials used different people, doses and durations, so you cannot lay one side-effect rate against another and draw a conclusion. What can be said is that the pattern is similar: digestive symptoms are the most common side effects of both licensed tablets and of Mounjaro.

For injected semaglutide, the STEP 1 trial reported that gastrointestinal side effects were reported by 74.2% of people on semaglutide 2.4mg, compared with 47.9% on placebo.9 That figure comes from a different trial and form, so it is context rather than a head-to-head comparison.

Practical ways to cope

Your prescriber can advise on managing digestive symptoms. The dose steps exist partly for tolerability, and for both tablets the dose can be held at an earlier step rather than pushed upwards. For Wegovy tablets, the dose can stay at an earlier step if needed.20 For Foundayo, the higher doses depend on response and tolerability.21

Contraception and pregnancy

Foundayo may make oral contraceptive pills less effective: switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase.21 Always discuss contraception, pregnancy plans and breastfeeding with your prescriber before starting any GLP-1 medicine. Mounjaro, by comparison, must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.1 Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed: switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.

When to seek help

Tell your prescriber about any new or worrying symptoms. If you suspect a side effect, you can report it through the Yellow Card scheme, which is run by the MHRA.19 If a symptom is severe or you feel seriously unwell, get medical help straight away rather than waiting for a review.

Other medicines

Because tablets have their own interaction rules, always tell the prescriber about everything you take, including over-the-counter products. For Wegovy tablets, that includes anything you would normally take in the morning, as you must wait before taking other tablets.20 A suitability check is part of every consultation, and repeat orders re-check suitability rather than shipping automatically.

09 · NHS versus private

NHS or private: how to get a GLP-1 pill

Access to GLP-1 weight-loss medicines on the NHS is being phased in, and for tablets you should ask your GP or a specialist weight-management service about current availability. Mounjaro is available on the NHS, but access is being phased in over several years.46

NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46

10 · Pills to avoid

Weight loss pills and claims to avoid

Many products labelled as the "best GLP-1 pills" are not medicines at all, and avoiding them protects both your health and your money. "Natural GLP-1" supplements (such as berberine) are not medicines, are not regulated for this use and have no comparable trial evidence. They are low-cost supplements with no clinical oversight, and their dosing is not standardised.

Supplements and "nature's Ozempic"

Online adverts sometimes claim a supplement works like a prescription GLP-1 medicine. Treat that as marketing, not evidence. A supplement has not been through the trials that sit behind the licensed tablets, so you cannot assume it will do anything comparable. Orlistat

Other licensed options work differently. Orlistat (tablet) blocks some dietary fat absorption and produces smaller average losses than the GLP-1 medicines. It is a genuine licensed option for some people, but it is a different kind of treatment, and your prescriber can advise on figures and suitability.

Ozempic

Ozempic is semaglutide licensed for type 2 diabetes, so it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg), plus the two licensed tablets. A regulated UK pharmacy will not supply Ozempic for weight loss.

Research peptides, compounded copies and stacking

Mounjaro must not be combined with other GLP-1 medicines, unlicensed "research" peptides (such as retatrutide, which has no UK licence), or compounded copies. These combinations have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign. The same logic applies to taking a GLP-1 tablet alongside an injection: never run two GLP-1 medicines together.

Counterfeit and unregulated sellers

Counterfeit medicines are a real risk wherever there is demand. Second, think about needles. Many people assume they will hate injections and find the pen easier than expected. Needles, sharps bin and injection guidance come with every order, and the pen is used once a week. Others are firmly happier with tablets, which is perfectly reasonable.

Third, think about your health history. A prescriber will check which option is suitable for your health and your other medicines. It is tempting to compare percentages. Resist it. OASIS 4 studied Wegovy tablets over a set period, ATTAIN-1 studied orforglipron over a different one, and the injection trials ran over other periods again. Different people, durations and doses mean the averages are not directly comparable. Only SURMOUNT-5 supports a tirzepatide versus semaglutide comparison, and it concerns injections.

What you can say fairly is that each licensed option has trial evidence of meaningful average weight loss alongside a reduced-calorie diet and more activity, with individual results varying. Your own result will depend on your starting point, the dose you reach, how you tolerate it and how consistently you take it.

Think about the long game

Weight management with medicine is a long-term plan. Ask what happens if you need to change medicine, how switching works and what support you will have. Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Which GLP-1 pill is best for weight loss?
There is no single best GLP-1 pill. The licensed UK options are Wegovy tablets and Foundayo, and both have their own trial results, dosing rules and eligibility checks. Wegovy tablets need an empty stomach and a wait before eating, while Foundayo can be taken at any time. A GPhC-registered prescriber will check which suits your health, your other medicines and your daily routine before supplying either.
Do oral GLP-1 pills work?
Yes, the licensed tablets have trial evidence of average weight loss alongside diet and activity. In the OASIS 4 trial, adults taking semaglutide 25mg tablets for 64 weeks lost on average 13.6% of their body weight, compared with 2.2% on placebo. In ATTAIN-1, the highest dose of orforglipron for 72 weeks gave 12.4% versus 0.9% on placebo. Individual results vary, and averages do not predict your own result.
Do oral GLP-1 work as well as injections?
Results from separate trials, with different people, durations and doses, should not be set against each other as if they were head to head. SURMOUNT-5 compared tirzepatide with injected semaglutide only. The practical differences matter too: a daily tablet routine versus one injection a week. A prescriber can help you choose.
Which GLP-1 is best for weight loss, pill or injection?
It depends on you rather than a single winner. Pills suit people who dislike needles and can manage a daily routine; Wegovy tablets need a strict empty-stomach routine, whereas Foundayo has no food or water rules. Mounjaro needs one injection a week. Your prescriber will advise on suitability.
What are the side effects of GLP-1 pills compared to shots?
Both mainly cause digestive effects. Wegovy tablets commonly cause nausea, vomiting, diarrhoea, constipation, indigestion and stomach pain, plus headache and tiredness. Foundayo's most common effects are nausea, constipation, diarrhoea, vomiting, indigestion and stomach pain. Injections have a similar pattern. Because trials differ in people, doses and duration, their rates should not be compared directly. Tell your prescriber about troublesome symptoms, and report suspected side effects through the Yellow Card scheme.
How much does the semaglutide oral pill cost?

Which weight loss pill works best?
It is not possible to name one honestly, because success depends on the person, the dose reached and how consistently the medicine is taken. The licensed GLP-1 tablets, Wegovy tablets and Foundayo, each have trial evidence, while orlistat produces smaller average losses and supplements such as berberine have no comparable trial evidence. A prescriber can advise which licensed option fits your health and routine.
Is there an over-the-counter GLP-1 weight loss pill?
No over-the-counter pill is a licensed GLP-1 medicine. Supplements marketed as natural GLP-1 products, such as berberine, are not medicines, are not regulated for this use and have no comparable trial evidence. They are low-cost supplements with no clinical oversight, and dosing is not standardised. If you want a clinically supported option, start with a consultation reviewed by a GPhC-registered prescriber.
How to use GLP-1 oral liquid?
The GLP-1 weight-loss medicines licensed in the UK as oral treatments are tablets, not liquids. Wegovy tablets are taken once daily on an empty stomach after fasting for at least 8 hours, swallowed whole with a sip of water (up to half a glass, about 120ml), and you then wait at least 30 minutes before eating, drinking or taking other tablets. Foundayo tablets can be taken at any time of day, with or without food and without water restrictions, and are swallowed whole. Be wary of any oral liquid sold without a prescription, and follow your prescriber's instructions.
