How appetite-reducing medicines actually work

A true appetite suppressant pill is not a stimulant that makes you forget food. The licensed medicines work through gut hormones, the natural signals your digestive system sends to your brain after eating. By mimicking those signals, they change how hungry you feel and how quickly you feel satisfied. This is why prescribers describe them as tools to support a reduced-calorie diet, not as replacements for one.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Wegovy tablets and Foundayo are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, so what is true of one should not be assumed for another.

Products sold as appetite suppressant pills are generally supplements or over-the-counter products. They are not tested against placebo in trials of the size and length that underpin the prescription medicines, and the claims on the packaging are marketing, not evidence.

Prescription medicines sit on the other side of a safety gate. A prescriber asks about your health history, your other medicines, pregnancy plans and previous experience with weight loss, because these affect which option is suitable and which are not. That gate is not an obstacle to good care; it is what makes the medicine safe to use and what separates a regulated pharmacy from a seller of unknown products.

The tablets and injections are intended for use alongside changes to eating and activity, and your prescriber will review how you are getting on at regular points.

Finally, appetite is only part of the picture. Sleep, stress, medicines you already take, and long-standing eating patterns all influence hunger. A good consultation takes those into account, and a good medicine plan includes support beyond the pill itself. That is why ongoing review matters more than which brand name is on the box.

05 · Wegovy tablets

Wegovy tablets: how they work and what trials show

Wegovy tablets are a licensed daily tablet for weight management in the UK. Wegovy tablets contain semaglutide, the same active ingredient as the Wegovy injection, taken as one tablet once a day.20 They are licensed for weight management in adults with a BMI of 30 or above, or 27 to under 30 with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity.20

The dosing ladder is gradual. Treatment starts at 1.5mg once daily and steps up to 4mg, 9mg and then the 25mg maintenance dose, with at least one month at each dose; the dose can stay at an earlier step if needed.20 That flexibility matters, because tolerability is part of the plan, not an afterthought.

The daily routine is the part people underestimate. The tablet must be taken on an empty stomach after fasting for at least 8 hours, swallowed whole with a sip of water (up to half a glass, about 120ml), and you then wait at least 30 minutes before eating, drinking or taking other tablets.20 If you have a busy morning, early commute or regular morning medicines, think honestly about whether you can keep to that every day. If a dose is missed, skip it and take the next tablet the following day.20

What the OASIS 4 trial found

In the OASIS 4 trial, adults taking semaglutide 25mg tablets for 64 weeks lost on average 13.6% of their body weight, compared with 2.2% on placebo.22 76.3% lost at least 5% of their body weight (vs 30.5% on placebo) and 59.8% lost at least 10% (vs 14.1%).22 Individual results vary, and these are trial averages, not a prediction for any one person.

The most common side effects are digestive — nausea, vomiting, diarrhoea, constipation, indigestion and stomach pain — along with headache and tiredness; gallstones and hair loss are listed as common.22 Side effects are a normal reason for a prescriber to hold you at an earlier dose, which the licence allows.

Who might prefer it

People who dislike needles often start their research here, and a tablet can feel less daunting than a pen. It can also suit someone with a settled morning routine who does not mind waiting before breakfast. Those who find a daily fasting routine hard to sustain sometimes decide a weekly option fits better, or look at Foundayo instead.

06 · Foundayo

Foundayo (orforglipron): the tablet with no food rules

Foundayo is a licensed daily tablet. Foundayo contains orforglipron, a once-daily GLP-1 tablet made by Eli Lilly, the maker of Mounjaro.21 Unlike Wegovy tablets it can be taken at any time of day, with or without food and without water restrictions; the tablet is swallowed whole.21

It is licensed for weight management in adults with a BMI of 30 or above, or 27 to under 30 with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity.21 That is the same BMI framework as Wegovy tablets, though the two medicines are otherwise different and your prescriber will check which is suitable.

Dosing starts low and rises with response. Treatment starts at 0.8mg once daily, then steps up to 2.5mg and 5.5mg, and then to 9mg, 14.5mg or 17.2mg depending on response and tolerability, with at least 30 days at each dose; 17.2mg once daily is the maximum.21 If a dose is missed, take it as soon as possible, but never more than one tablet a day.21

What the ATTAIN-1 trial found

In the ATTAIN-1 trial of 3,127 adults without type 2 diabetes, people taking the highest dose of orforglipron for 72 weeks lost on average 12.4% of their body weight, compared with 0.9% on placebo; the two lower doses gave 7.8% and 9.3%.23 The trial used capsules, and the UK 17.2mg tablet corresponds to the highest trial dose, so the tablet and capsule doses are not comparable milligram for milligram.23

The most common side effects are digestive — nausea, constipation, diarrhoea, vomiting, indigestion and stomach pain.23 In the trial, 10.3% of people on the highest dose stopped because of side effects, compared with 2.7% on placebo.23 Those figures are worth reading honestly: most people tolerated the medicine, and a minority stopped because of side effects.

A note on contraception

Foundayo may make oral contraceptive pills less effective: switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase.21 Tell your prescriber about contraception at the start, so the plan accounts for it.

07 · Tablets vs injection

Tablets versus the Mounjaro injection

There is no Mounjaro tablet. There is no tablet form of Mounjaro — tirzepatide is injection-only in the UK.1 What exists is a choice between tablets from other medicines and a weekly injection, and the sensible way to choose is to compare routine, suitability and evidence quality, not headline percentages.

Mounjaro is a once-weekly injection; Wegovy tablets and Foundayo are once-daily tablets.15 No trial has compared Mounjaro directly with either tablet, so results from separate trials — with different people, durations and doses — should not be set against each other as if they were head to head (SURMOUNT-5 is the trial that compared tirzepatide directly with injected semaglutide).15

The practical differences matter as much as the averages: Wegovy tablets need a strict empty-stomach routine every morning, Foundayo has no food or water rules, and Mounjaro needs one injection a week.15 A prescriber will check which option is suitable for your health, your other medicines and your routine.15

What the direct comparison does tell us

The one fair head-to-head is between tirzepatide and injected semaglutide. In the first large head-to-head trial (SURMOUNT-5), adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15 Both are effective, licensed treatments; which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone.15

For context on the semaglutide injection itself: in the STEP 1 trial, adults on semaglutide 2.4mg (Wegovy) lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 These are separate trials and should be read as such.

Choosing on lifestyle, not just numbers

Ask yourself a few practical questions. Do you mind injecting once a week, or would you rather swallow something daily? Can you fast each morning and wait before eating or taking other tablets? Do you take other medicines at fixed times? Is oral contraception part of your life? The answers often settle the choice more than a difference in trial averages.

None of these routes is superior for everyone. The right medicine is the one a prescriber confirms is suitable and that you can realistically stay on.

08 · Supplements and orlistat

Natural pills, supplements and orlistat

Natural appetite suppressants are the most heavily marketed products in this space and the least supported by evidence. "Natural GLP-1" supplements (such as berberine) are not medicines, are not regulated for this use and have no comparable trial evidence.1 Phrases like "nature's Ozempic" are marketing slogans, not descriptions of an equivalent product.

Why do they sell so well? They are low-cost supplements, available cheaply without assessment, and the packaging usually promises dramatic weight-loss claims. The gap between that promise and the evidence is exactly why we recommend caution. A product that is easy to buy is not necessarily a product that works, and a supplement's dosing is not standardised the way a medicine's is, so you cannot be sure what you are taking from one brand to the next.

Common ingredients you will see

Fibre blends, caffeine-based products, herbal extracts and vitamin-style formulations make up most of the shelf. Some may feel filling for a short time, and fibre can be a reasonable part of a balanced diet. But none is a substitute for a licensed medicine, and none has the kind of large placebo-controlled trial behind it that supports the prescription options. If you take other medicines or have a health condition, speak to a pharmacist before adding any supplement, because supplements can interact with prescribed treatment.

Orlistat as a licensed alternative

Orlistat is the licensed tablet in this group. Other licensed options work differently: orlistat (tablet) blocks some dietary fat absorption and produces smaller average losses, and Wegovy and Saxenda (liraglutide) are other licensed injectables.1 Orlistat does not reduce appetite as such; it acts on dietary fat absorption. For some people that is acceptable, for others it is a reason to look elsewhere. Your prescriber or pharmacist can explain how it would fit.

Saxenda is a daily injection, unlike once-weekly Mounjaro and Wegovy.

The "stacking" trap

Some sellers suggest combining several products for faster results. That is the point at which risk rises sharply. Mounjaro must not be combined with other GLP-1 medicines, unlicensed "research" peptides (such as retatrutide, which has no UK licence), or compounded copies.1 These combinations have no safety data and no regulated supply chain.1 If something promises faster results outside a licensed pathway, that is the warning sign.1

09 · Safety and side effects

Safety, side effects and who should not take them

The licensed tablets share a pattern of side effects, mostly digestive, and a handful of situations where they are not suitable or need extra care. Reading these before a consultation helps you give your prescriber a useful history. Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.

Side effects to expect

For Wegovy tablets, the most common side effects are digestive — nausea, vomiting, diarrhoea, constipation, indigestion and stomach pain — along with headache and tiredness; gallstones and hair loss are listed as common.22 For Foundayo, the most common side effects are digestive — nausea, constipation, diarrhoea, vomiting, indigestion and stomach pain.23 In general, these tend to be most noticeable when starting or stepping up a dose, which is why gradual titration exists.

You can also report suspected side effects through the MHRA Yellow Card scheme. Never ignore a new symptom because you assume it is normal for the medicine.

Contraception

Mounjaro and Foundayo can affect how well the oral contraceptive pill works. For Mounjaro, the reason is stated plainly: Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 This applies to the pill only — implants, coils and injections are unaffected.1

For Foundayo, the advice is its own: Foundayo may make oral contraceptive pills less effective: switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase.21

Pregnancy and breastfeeding

Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.1 If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.1 Current prescribing information indicates minimal transfer of tirzepatide into breast milk; discuss breastfeeding with your prescriber so the decision reflects your circumstances.1 For the tablets, discuss pregnancy and breastfeeding plans in your consultation so the prescriber can advise on that specific medicine.

PCOS and other conditions

Mounjaro is not licensed specifically for PCOS, but many women with PCOS meet the licensed weight-management criteria, and weight loss itself is a first-line recommendation that can improve insulin resistance, cycles and symptoms.1 Mention PCOS and any fertility plans in your consultation, because improving weight can restore ovulation.1

Why a prescriber matters

Your other medicines, health conditions and plans all affect suitability. A prescriber reviewing your questionnaire is not a formality; it is how unsuitable treatment is caught before it reaches you. If something is unsuitable, the order is refunded automatically.

NHS, your GP and the private route

Can you get weight loss tablets from your GP? Your GP is the right first conversation, and they can discuss what the NHS offers locally, but access depends on where you live and how your health needs are assessed. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label.1 The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg).1 So "Mounjaro vs Ozempic" for weight loss is really Mounjaro vs Wegovy, and a regulated UK pharmacy will not supply Ozempic for weight loss.1 If a website offers it for weight loss, treat that as a red flag.

Switching between medicines

Switching is a prescriber decision, not a do-it-yourself conversion. There is no licensed direct conversion between semaglutide and tirzepatide.1 Switching is done under prescriber supervision: typically the previous medicine is stopped, a washout interval observed, and Mounjaro started at a dose your prescriber judges appropriate — often the 2.5mg starting dose, sometimes adjusted based on your history.1 Never run two GLP-1 medicines together.1

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 That matters if you are planning a pregnancy or switching medicine, which is another reason to involve your prescriber early. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Spotting unsafe sellers

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
