The trend has two main motivations. The first is cost: a smaller dose looks as if it should make a pen last longer. The second is tolerability: people worry about nausea or a loss of appetite that feels too strong, and hope a smaller dose will soften it. Both are understandable. Neither is a good reason to alter a medicine whose dosing was designed and tested at specific strengths.

It is also promoted heavily by wellness businesses and telehealth platforms that benefit from selling a lower-cost-looking product. When a seller offers dramatic weight-loss claims with a tiny dose, treat the claim as marketing rather than medicine. No approved clinical trial supports micro dosing for meaningful long-term weight loss or metabolic health, so any promised result has nothing behind it.

It helps to be clear about the vocabulary. Micro GLP-1 is not a drug class, a product or a clinical term. A GLP-1 medicine is a prescription treatment with a licensed dose range.

For Mounjaro specifically, the licensed medicine is tirzepatide, a once-weekly injection made by Eli Lilly and licensed in the UK for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, called a KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 That is the entire set of strengths designed, tested and approved. Anything described as smaller than 2.5mg is outside the licence, and anything made by splitting a pen is not what the pen was built to do.

03 · Pen design and dosing

Why the pen design does not allow micro doses

The Mounjaro KwikPen is built to deliver set weekly doses, not fractions. You set the dose by turning the dose knob until the dose indicator shows the correct dose in the window. That single instruction removes most of the mechanical routes people imagine for micro dosing, because a click-counting approach is explicitly not how the pen should be read.

Altering doses outside the licence means unknown dosing accuracy and no safety data.1 Those last two phrases matter. Unknown dosing accuracy means neither you nor a clinician can tell how much medicine you actually received. No safety data means nobody has studied what happens when it is done this way.

Injection basics still apply

The licensed method is simple. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 There is no best time of day: Mounjaro works the same morning or evening. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off.

Spreading doses out is the second half of the micro dosing idea, and it runs into the same wall. The schedule is weekly. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, never taking two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That flexibility is the licensed way to fit treatment around your life. It is not a mechanism for spacing doses further apart or for deliberately reducing exposure.

This starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

05 · Evidence gap

What the evidence shows, and what it does not

The evidence for GLP-1 and GIP medicines comes from trials of full licensed doses, not micro doses. The best-known tirzepatide result is SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Notice what that trial tested: specific strengths, given on the licensed schedule, over a long period. It did not test fractions of a pen, irregular gaps or homemade dilutions. That means none of those percentages can be borrowed to justify micro GLP-1. Doing so would be like quoting the result of one exercise programme to defend a different one that nobody has measured.

For readers who like a worked example: for someone starting at 100 kg, 20.9% is about 21 kg. That example describes the trial average on the top dose, not a prediction for you and not a promise for any lower dose. Trial averages do not predict your own result.

06 · Sourcing and compounding risks

Compounded, split and unregulated supply

Micro GLP-1 offers are often tied to compounded or relabelled products, and that is where the highest risk sits. Compounded versions are made outside the process that produces a licensed pen, and the strength, purity and sterility of what you receive are harder to verify. The risks include contamination, dosing errors and unclear labelling. A tiny dose from an unverified vial is not safer than a known dose from a licensed pen; it is simply less visible.

The MHRA publishes guidance and counterfeit medicine alerts, and runs the Yellow Card scheme for reporting side effects and suspect products.7 If you have already taken something unusual, or you suspect a product is not what it claims to be, you can report it through the Yellow Card scheme.19

Red flags to walk away from

  • A seller offering to supply without a consultation or prescription.
  • Dramatic weight-loss claims, or any promise of specific results.
  • Products described as micro or mini versions of a prescription medicine without a licensed strength.

07 · Mechanism and clearance

How GLP-1 medicines work, and why dose matters

GLP-1 is a natural gut hormone, and GLP-1 medicines copy part of its action. That answers a common question: GLP-1 is not the same thing as Mounjaro, but Mounjaro acts on GLP-1 receptors. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

That dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1

Why a long half-life changes the micro dosing logic

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Changing amounts or gaps at random makes your levels less predictable, not more.

The licensed schedule takes this into account. A micro dosing plan invented from a social media post cannot make use of that reasoning because it does not know your starting point.

Stomach emptying and other medicines

Slowed stomach emptying is part of how these medicines work, and it is also why they can reduce absorption of oral contraceptives. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If this applies to you, tell the prescriber during your consultation. Likewise, if you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar, which is another reason to have a clinician involved rather than self-adjusting.7

08 · Natural options and other medicines

Mimicking GLP-1 naturally, pills and other routes

Nothing sold as a natural GLP-1 booster is a substitute for prescribed treatment. Products marketed as nature's Ozempic or similar are low-cost supplements with no clinical oversight, and their dosing is not standardised. If you want to help your body regulate appetite, the proven foundation is the one the licence itself relies on: a reduced-calorie diet and increased physical activity.1 A GP or pharmacist can help with the practical side.

Do the GLP-1 pills work?

Oral GLP-1 options exist and have their own product information. Wegovy tablets contain oral semaglutide, and Foundayo is a separate once-daily tablet medicine, each with its own prescribing information.2021 Their trial programmes are separate from Mounjaro's, so their results should not be set side by side with tirzepatide as if they were head to head.

Injections other than Mounjaro

Wegovy (semaglutide) is a once-weekly pen with a dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg. Titration steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17, and 2.4mg is the licensed maintenance dose for weight management.18 Liraglutide, supplied at 6mg/ml, is a daily injection, unlike once-weekly Mounjaro and Wegovy.

NHS or private?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults who meet specific criteria, and the primary-care rollout began in June 2025 with the highest-need groups first.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. Ask your GP about your own eligibility.

Can you get a GLP-1 medicine if you are not overweight?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes, and a prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, since figures circulating online often come from other countries' labels.1 This is a good example of why unverified advice is a problem: a number that sounds precise may be for a different product in a different market.

Do the GLP-1 pills actually work?
Licensed GLP-1 tablets exist, including Wegovy tablets and Foundayo, and each has its own UK product information describing how it should be used. Their trial results come from separate programmes, so they should not be compared with Mounjaro's results as if head to head.

What is the downside of GLP-1 medication?
The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, which usually settle. Other downsides are practical: it is a long-term commitment, results vary, it needs a reduced-calorie diet and more activity alongside it, and the medicine stays in your system for four to five weeks after a final dose. Serious side effects are uncommon; get medical help straight away for severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction. Your prescriber reviews response and tolerability, and the patient leaflet lists every known side effect.
Can I get a GLP-1 medicine if I am not overweight?
