What GLP-1 injections do and why people look for alternatives

GLP-1 injections for weight loss work by changing appetite signals, so most people feel fuller sooner and less hungry between meals. Tirzepatide, sold as Mounjaro, is a once-weekly prescription medicine that activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.1 That dual action is what distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy, Ozempic).

This is medical weight loss with a GLP-1 injection, and it is intended for chronic weight management. It is used alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Mounjaro comes as a pre-filled multi-dose KwikPen in six strengths from 2.5mg to 15mg, and each pen holds four weekly doses.

So why do people look for an alternative? The reasons we hear most are needle dislike, cost, digestive side effects, and a worry about being on treatment long term. Others have read about people stopping Ozempic and want to understand what comes next. These are fair questions, and the honest answer is that each one has a different solution: a tablet for needle dislike, a lower dose or slower titration for side effects, and a planning conversation for cost.

It also helps to separate two meanings of 'alternative'. One meaning is another licensed medicine that does a similar job. The other is a product marketed as a natural replacement. The first group is assessed by regulators and tested in trials. The second is not held to the same standard for weight loss.

Names matter here too. Ozempic is a semaglutide brand licensed for type 2 diabetes; Wegovy is the semaglutide brand for weight management. Mounjaro contains tirzepatide, a different molecule. When people ask for an 'alternative to Ozempic', they are often really asking about Wegovy or Mounjaro, both of which are weight-management products we can discuss with you.

Before comparing options, decide what you actually want to change. If it is the weekly needle, look at oral options. If it is side effects, ask about pace. If it is cost, compare the monthly price at your likely maintenance dose. If it is the idea of staying on medicine, talk to a prescriber about your goals and timeline. Naming the real problem makes the right alternative much easier to spot, and it stops you paying for a substitute that solves nothing.

05 · Realistic weight loss

Realistic weight loss from GLP-1 injections, and what a stone means

Realistic weight loss from GLP-1 injections depends on the medicine and dose, and trial averages do not predict your own result. In SURMOUNT-1, which enrolled 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

A worked example makes this concrete. For someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. These are averages across a trial population. Some people lost more, some less, and treatment works alongside a reduced-calorie diet and increased physical activity.

The same trial reports how many people reached certain thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 That tells you a meaningful loss is common at the studied doses, but not universal.

Semaglutide has its own trial results. In STEP 1, adults on semaglutide 2.4mg (Wegovy) lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 In the first large head-to-head trial (SURMOUNT-5), tirzepatide averaged 20.2% over 72 weeks against 13.7% for semaglutide 2.4mg.15 Both are effective, licensed treatments.

Reviews of GLP-1 injections often quote dramatic outcomes. We do not use personal stories as evidence; trial data is the better guide. Higher is also not automatically better: many people settle at 5mg, 10mg or 15mg, and your prescriber guides the right pace.1

Compared with lifestyle change alone, the gap in SURMOUNT-1 (3.1%) is large, which is the clearest reason an alternative that is only lifestyle-based may not deliver the same average result. That does not make lifestyle work pointless: the medicine is designed to support it, and the people in the trial combined both.

06 · Side effects compared

GLP-1 weight loss injection side effects, and when an alternative is wiser

GLP-1 weight loss injection side effects are mostly digestive, and for most people they ease within days to a few weeks. For Mounjaro, the most common effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects, up to 1 in 10, include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.13

The trial numbers give a clearer picture. Nausea was reported by 24.6% of people on 5mg, 29.0% on 10mg and 28.0% on 15mg, compared with 8.5% on placebo. Diarrhoea was reported by 18.7% on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo. These were more common while the dose was increasing and became less frequent over time. Permanent stopping because of gastrointestinal effects was 1.9% on 5mg, 3.3% on 10mg and 4.3% on 15mg, against 0.5% on placebo.1

In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped because of side effects, compared with 2.6% on placebo.2 Hair loss was reported by 4.9% of people taking tirzepatide versus 1.0% on placebo, and injection-site reactions by 8.0% versus 1.8%.1

Side effects are most likely when starting treatment or stepping up. Speak to your clinical team rather than pushing through severe symptoms.

Practical steps that help most people: eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated and avoid lying down straight after eating. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.

Seek immediate medical help for severe, persistent stomach pain that may spread to your back (possible pancreatitis), pain in the upper right abdomen with fever or yellowing of skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea, watch for low blood sugar; your prescriber may need to adjust those medicines.13

Side effects can be the genuine reason to consider an alternative, but not always the first step. Ask your prescriber about holding the dose or slowing the pace before switching. Report any suspected side effect through the Yellow Card scheme.19 Other medicines have their own side-effect profiles, so an alternative is not automatically gentler.

07 · Tablets and other medicines

Tablets, daily injections and other prescription medicines

The licensed alternatives to a weekly GLP-1 pen are oral GLP-1 tablets, daily injections and a handful of non-GLP-1 medicines, each decided with a prescriber.

Oral GLP-1 tablets are the closest substitute for people who dislike needles. Wegovy tablets are an oral form of semaglutide, and Foundayo (orforglipron) is an oral tablet taken as a daily medicine.2021 Each has its own prescribing information, including how and when it is taken, so follow the product leaflet and your prescriber's advice. Their trial results should not be set against Mounjaro's, because separate trials with different designs cannot be read side by side. Daily injections are another route. Liraglutide, sold as Saxenda and in products such as Erly, is supplied at a concentration of 6mg per millilitre. It is a daily injection, unlike once-weekly Mounjaro and Wegovy. If your issue is the needle itself, a daily injection is unlikely to help, but if you want to compare dosing frequency, this is the fact to weigh. Figures vary — your prescriber can advise on how it compares for you.

Other prescription medicines such as orlistat work differently from a GLP-1 medicine, and they are not interchangeable with one. Figures vary, so your prescriber can advise. Availability also differs, so a medicine you read about may not be licensed or available in the UK.

There are also injections for weight loss that are not GLP-1 medicines, such as vitamin B shots and other injectable products. These are discussed widely, but weight loss claims need evidence. 08 · Natural alternatives

Natural alternatives and the 'poor man's Ozempic'

No natural product has been shown to do what a GLP-1 injection does, and supplements marketed as 'natural Ozempic' are not equivalent. People often ask what the closest natural thing to Ozempic is, or what counts as the poor man's Ozempic. Berberine is the name most often attached to these labels, along with fibre products and various herbal blends.

The honest position is this. These are low-cost supplements with no clinical oversight, available cheaply without assessment. Their dosing is not standardised, and they are not licensed as weight-loss medicines. Dramatic weight-loss claims on product pages should be treated with suspicion, particularly where a seller promises specific results.

A supplement can also interact with other things you take, and nobody reviews your health history before you buy it. That lack of assessment is a real difference from a prescription route, where a prescriber checks your suitability, medical history and current medicines first.

Foods and habits that support fullness are a different matter. High-fibre vegetables, adequate protein, regular meals and enough fluid can make a reduced-calorie diet easier to follow. Those habits are also what treatment is meant to sit alongside, so building them now is useful whichever route you choose. They are support for a diet, not a replacement for a medicine.

09 · Sustainable weight loss

Sustainable weight loss, stopping treatment and long-term use

GLP-1 injections are designed for chronic weight management, so the sustainability question is really about what happens when treatment changes or stops. People ask why others stop Ozempic, and whether anyone keeps the weight off afterwards.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up and nothing needs doing while it clears.1 The appetite effect fades as the medicine does, so it is sensible to plan eating habits and activity before you stop rather than after. SURMOUNT-4 looked at continued tirzepatide treatment for maintenance of weight reduction, which is why prescribers discuss long-term use rather than treating a course as a quick fix.14

Common reasons people stop treatment include side effects, cost and reaching a goal. In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped because of side effects, compared with 2.6% on placebo.2 If you are thinking of stopping, speak to your prescriber first about dose reduction, a longer hold on the current dose, or a different approach.

Sustainable loss also protects the things people worry about. 'Ozempic face' is not a drug effect: it is what losing a significant amount of weight quickly can do to facial volume, on any method including diet or surgery. The same applies to loose skin. The counter-measures protect lean mass: adequate protein, resistance exercise and a pace guided by your prescriber rather than maximum speed. Skin commonly adapts gradually after weight stabilises. Temporary hair shedding is linked to rapid weight loss itself and settles for most people as weight stabilises.2

Mood is another concern. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and suicidal thoughts, and the MHRA keeps this under continuous review.17 Even so, a big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

10 · Dosing and routine

How a weekly pen works day to day

A weekly pen is a simple routine once you know the steps, which matters if needles are why you are looking for alternatives. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area at least 5cm from the belly button, the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.13 Follow the pen's instructions for the injection itself.

Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Wegovy follows a similar principle: 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least 4 weeks to reach the 2.4mg maintenance dose from week 17.18

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose and take the next one on your normal day. Never take two doses within 3 days of each other.13

Injection-site reactions are usually minor. Mild redness, itching, a small bruise or a tender lump is common and normally settles within a few days. Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging. Contact your clinical team if a reaction is spreading, hot, very painful or lasts more than a week.

Constipation is very common on Mounjaro for weight management (it may affect more than 1 in 10 people), because food moves through the gut more slowly and many people eat and drink less. Fluids come first, then fibre from vegetables and whole grains and daily movement. If it persists, a pharmacist can recommend a suitable laxative. Fatigue is also common, most noticeable in the first weeks and after dose increases. Keep meals balanced, with protein especially, stay hydrated and keep gentle activity going.

Storage matters for a pen. Unopened pens go in the fridge until first use and keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge for up to 30 days (not above 30°C). If you are unsure about an unopened pen left out, check the patient information leaflet and ask the pharmacy.

Many people find this routine simpler than expected. If it still sounds like a barrier, an oral option may be the better conversation to have, and the consultation is the place to have it.

NHS, private treatment and what it costs

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. What are some alternatives to Ozempic for weight loss?
Licensed alternatives include Wegovy, which is semaglutide authorised for weight management, and Mounjaro, which contains tirzepatide. Oral options such as Wegovy tablets and Foundayo exist for people who dislike needles, and daily liraglutide injections are another route. Supplements marketed as natural alternatives are not equivalent and carry no comparable trial evidence. A prescriber can advise which licensed option fits your health history, tolerance and budget.
Is Wegovy cheaper than Ozempic?
Ozempic is licensed for type 2 diabetes, while Wegovy is the semaglutide brand licensed for weight management, so they are not interchangeable choices for weight loss. A regulated UK pharmacy will not supply Ozempic for weight loss. For Wegovy, your first pen costs £99.99 on a first order, and you can see current dose prices on the Wegovy pen page. Ask a prescriber which product is appropriate for your situation before comparing cost.
What can GLP-1 help with besides weight loss?
GLP-1 medicines were first developed for type 2 diabetes, and tirzepatide is licensed in the UK for both weight management in adults and type 2 diabetes. Tirzepatide improves the way the body responds to insulin, which is part of why it is used for diabetes. Tirzepatide has also been studied in obstructive sleep apnoea with obesity (SURMOUNT-OSA). A prescriber can explain whether any of these uses apply to you.
What is considered the poor man's Ozempic?
The phrase is a marketing label, usually attached to low-cost supplements such as berberine. It is not a clinically recognised substitute for a prescription medicine. These are low-cost supplements with no clinical oversight, available cheaply without assessment, and their dosing is not standardised. They have no trial results comparable with SURMOUNT-1 or STEP 1. If cost is the concern, speak to a prescriber about licensed options and compare monthly prices at the dose you will actually use.
What's the closest natural thing to Ozempic?
Nothing natural is a proven equivalent. Dietary habits can support fullness: fibre from vegetables and whole grains, adequate protein, regular meals and enough fluid. These help a reduced-calorie diet feel easier, but they are not a replacement for a medicine. Supplements sold as natural Ozempic have no comparable evidence, no prescriber review and no standardised dosing. If you want a medically supported route, a free consultation will tell you whether you are suitable for treatment.
Why are people stopping Ozempic?
People stop GLP-1 medicines for several reasons, commonly side effects, cost and reaching a goal. In SURMOUNT-1, 4.3% of people on tirzepatide 5mg, 7.1% on 10mg and 6.2% on 15mg stopped because of side effects, compared with 2.6% on placebo. If you are considering stopping, talk to your prescriber first about holding or reducing the dose, because tirzepatide takes roughly 4–5 weeks to clear the system after a final dose.
Has anyone kept the weight off after stopping Ozempic?
Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average. Habits built during treatment, such as balanced meals, protein, resistance exercise and regular activity, are what you carry forward. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
Can GLP-1 injections be sustainable for long-term weight loss?
They are intended for chronic weight management, used alongside a reduced-calorie diet and increased physical activity. In SURMOUNT-1, people on tirzepatide lost on average 15% to 20.9% of body weight over 72 weeks, compared with 3.1% with lifestyle changes alone, though individual results vary. Sustainability depends on tolerating treatment, affording it, and building habits that last. What are the side effects of GLP-1 weight loss injections?
The most common effects, affecting more than 1 in 10 people on Mounjaro, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Most are digestive, appear after starting or stepping up a dose, and ease within days to a few weeks. Get medical help straight away for severe persistent stomach pain, upper right abdominal pain with fever or yellowing skin, or signs of a severe allergic reaction. Report suspected side effects through the Yellow Card scheme.
Can weight loss injections help me lose just one stone?
Trial results are given as percentages of starting weight, so the amount lost scales with where you begin. For someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg, but those are averages at the studied doses and individual results vary. A prescriber will judge whether medicine is appropriate for a modest goal, so be open about it in your consultation.
