Which is right: Wegovy 2.4mg or Mounjaro 10mg?
Neither dose is simply "stronger"; they sit on different scales. Wegovy (semaglutide) climbs to 2.4mg, which is its licensed maintenance dose for weight management, reached from week 17 of titration. Mounjaro (tirzepatide) climbs to a maximum of 15mg, so 10mg is a mid-to-high step that many people pass through or settle at.20

Decision factors that matter more than the milligrams
Start with where you are today. If you are new to both, you would begin either medicine at its lowest dose rather than at 2.4mg or 10mg. Everyone starts Mounjaro on 2.5mg once a week, and the starting dose is about letting your body adjust.1 Wegovy likewise starts low and steps up after at least four weeks.
Next, consider tolerance. Higher is not automatically better on Mounjaro; many people settle at 5mg, 10mg or 15mg, and your prescriber guides the pace.1 Finally, consider support. 04 · How each works
How semaglutide and tirzepatide work differently
The most important difference sits in the mechanism, not the dose number. Tirzepatide, the active ingredient in Mounjaro, is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1
Semaglutide, the active ingredient in Wegovy, is a single-action GLP-1 medicine.1 That dual-versus-single distinction is what sets Mounjaro apart from semaglutide products such as Wegovy and Ozempic.1
In practice, most people on Mounjaro feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Wegovy is also prescribed to be used alongside a reduced-calorie diet and increased physical activity; neither medicine replaces those habits.
Why the milligrams are not comparable
A milligram of semaglutide and a milligram of tirzepatide are different molecules acting on different receptor combinations, so the numbers sit on separate scales. Wegovy's ladder is 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, with 2.4mg the licensed maintenance dose. Mounjaro's is 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Seeing "2.4" next to "10" tells you nothing about relative strength.
It follows that no one can say 10mg of Mounjaro "equals" a particular Wegovy dose. There is no licensed direct conversion between semaglutide and tirzepatide, which is why any change between them is made under prescriber supervision.1
Duration in the body
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 This matters if you plan a switch, because the prescriber will think about timing and the interval between medicines.
Same week, same day
Both medicines are once-weekly injections; liraglutide (Saxenda), supplied at 6mg per millilitre, is a daily injection. With Mounjaro the KwikPen holds four weekly doses, so a pen lasts a month.1 The mechanism difference is the headline; the weekly routine is similar in feel.

05 · Trial evidence
What the trials show for each medicine
The first large trial that compares the two medicines directly is SURMOUNT-5. In that head-to-head study, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15 Both are effective, licensed treatments, and the headline number alone should not decide your choice.15
Wegovy: STEP 1
In the STEP 1 trial, adults on semaglutide 2.4mg (Wegovy) lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 These are trial averages, and individual results vary.
Mounjaro 10mg: SURMOUNT-1
In SURMOUNT-1, which enrolled 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
A worked example
For someone starting at 100 kg, 19.5% is about 19.5 kg. This is straight arithmetic on a trial average, so it illustrates scale only. It is not a prediction of your own result, and it says nothing about how quickly any loss would happen.
How to read these numbers fairly
SURMOUNT-1 and STEP 1 had different designs, durations and populations. Putting 19.5% beside 14.9% as though it were a contest would be misleading. SURMOUNT-5 is the fair comparison, and it points in tirzepatide's favour on average weight loss.
Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
06 · Dose ladders
Titration: how you reach 2.4mg or 10mg
You do not begin on either of these doses; both are reached by stepping up. Wegovy pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. Titration steps up after at least 4 weeks, reaching the 2.4mg maintenance dose from week 17.18 The 2.4mg dose is the licensed maintenance dose for weight management.
Mounjaro starts everyone on 2.5mg once a week, a dose about letting your body adjust rather than weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 So 10mg is the fourth pen in the sequence.
Maintenance is individual
Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 Reaching 10mg is therefore not a goal in itself; it is simply one of the places the ladder can stop. Likewise, Wegovy's 2.4mg is the licensed maintenance dose, but the route there is gradual.
Realistic pace
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1 If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
Weekly routine on Mounjaro
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 You can change your injection day as long as at least 3 days (72 hours) have passed since your last dose.1
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day. 07 · Side effects and safety
Side effects and safety
Neither medicine is free of side effects, and the full lists are in each product's leaflet. For Wegovy, read the Wegovy Summary of Product Characteristics and patient leaflet;18 for Mounjaro, the SmPC and Patient Information Leaflet.1
Early effects on Mounjaro
The 2.5mg starting weeks are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1
Is Wegovy safer than Mounjaro?
Neither medicine can be called "safer" in general. Both are licensed, prescription-only medicines with established safety information, and the right one depends on your medical history, other medicines and tolerance. A GPhC-registered prescriber reviewing your health questionnaire is the correct way to judge suitability.
Contraception and other medicines
Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar; this is a conversation with your prescriber, not a reason to stop either medicine yourself.1
Never combine them
Mounjaro must not be combined with other GLP-1 medicines, which includes Wegovy. It must also not be combined with unlicensed "research" peptides (such as retatrutide, which has no UK licence) or compounded copies. These combinations have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign.1
Reporting problems
If you notice a new symptom, discuss it with your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme, whichever medicine you take.19 Reporting helps regulators spot patterns that individual patients cannot.

08 · Switching between them
Switching between Wegovy and Mounjaro
There is no licensed direct conversion between semaglutide and tirzepatide, so a switch is always a prescriber decision. Typically the previous medicine is stopped, a washout interval is observed, and Mounjaro is started at a dose your prescriber judges appropriate. That is often the 2.5mg starting dose, though it is sometimes adjusted based on your history.1 Never run two GLP-1 medicines together.1
What this means if you are on Wegovy 2.4mg
Being on the maintenance dose of Wegovy does not mean you can start at 10mg on Mounjaro. A person who has been on semaglutide 2.4mg may still be asked to start low, because the two medicines act differently and your body has to meet tirzepatide first. Your prescriber will weigh up your history and tolerance.
Reasons people consider switching
Common reasons are wanting a dual-action medicine, a plateau in response, or tolerance. The head-to-head SURMOUNT-5 result, 20.2% for tirzepatide against 13.7% for semaglutide 2.4mg over 72 weeks, is part of why the question arises, though averages do not predict your own outcome.15 Others consider the opposite direction, from Mounjaro to Wegovy, for reasons such as availability or prescriber preference; the same rule applies, because a prescriber must supervise it. 10 · NHS and private
NHS or private: how access differs
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6

11 · Other options
Tablets, Ozempic and "cheaper alternatives"
Ozempic
Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg). So "Mounjaro vs Ozempic" for weight loss is really Mounjaro vs Wegovy, and a regulated UK pharmacy will not supply Ozempic for weight loss.1
Tablets
There is no tablet form of Mounjaro; tirzepatide is injection-only in the UK. Two GLP-1 weight-loss tablets are now licensed: Wegovy tablets (oral semaglutide, made by Novo Nordisk), approved by the MHRA in June 2026, and Foundayo (orforglipron, made by Eli Lilly), approved in August 2026. They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only.2021 Their trial results should not be compared with Mounjaro's as if head to head.
Other licensed options
Orlistat (tablet) blocks some dietary fat absorption and produces smaller average losses. Wegovy and Saxenda (liraglutide) are other licensed injectables, and liraglutide is a daily injection, unlike once-weekly Mounjaro and Wegovy.1
Supplements
"Natural GLP-1" supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence. They are available cheaply without assessment, which is a safety point rather than a benefit, and dosing is not standardised. Do not treat them as a substitute for a licensed medicine.
Storage and travel, briefly
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the cap on to protect it from light.1 You can fly with Mounjaro: carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription, and check destination rules first.1 Unopened pens keep until the printed expiry date. If an unopened pen has been out of the fridge, check with the pharmacy before using it.

12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
