This is different from oral semaglutide, which has specific instructions about taking the tablet on an empty stomach with a small amount of water and waiting before eating.
Foundayo is the brand name for orforglipron, an oral, non-peptide GLP-1 receptor agonist. What does the evidence show?
In ATTAIN-1, a phase 3 randomised trial involving 3,127 adults with obesity but without diabetes, participants received once-daily orforglipron at 6 mg, 12 mg or 36 mg, or placebo, alongside healthy diet and physical activity advice, for 72 weeks.
At week 72, the mean change in body weight under the treatment-regimen estimand was −7.5% with 6 mg, −8.4% with 12 mg and −11.2% with 36 mg, compared with −2.1% with placebo. In the 36 mg group, 54.6% of participants achieved at least 10% weight loss, 36.0% achieved at least 15%, and 18.4% achieved at least 20%.
The trial used the capsule formulation of orforglipron. The UK-authorised Foundayo film-coated tablets use different numerical strengths; the UK SmPC identifies the 36 mg capsule as equivalent to the 17.2 mg tablet formulation.
Results from a clinical trial describe average outcomes in a study population and cannot predict exactly how much weight an individual person will lose.
What commonly goes wrong with food on Foundayo?
- Reading "no food restrictions" as "eat anything". The licence requires a reduced calorie diet.
- Large portions may be harder to tolerate because Foundayo delays gastric emptying.
- Skipping meals because appetite has gone, then eating a large late dinner.
- Vomiting or diarrhoea can contribute to dehydration, particularly during treatment initiation or dose increases.
- Cutting protein along with everything else, and may make it harder to meet protein and overall nutritional needs during weight loss.
- Blaming the tablet for nausea that a fatty takeaway caused.
- Taking a second tablet after vomiting. Never.
- Using alcohol to unwind at a dose step, when side effects are already at their peak.
When should you seek help?
Contact your prescriber or pharmacist for nausea or fullness that stops you eating regular meals, for constipation that fibre and fluid do not settle, or for repeated vomiting. Seek urgent medical attention for severe persistent abdominal pain, an inability to keep fluids down, or yellowing of the skin or eyes.
