During sexual arousal, natural signals cause the blood vessels in the penis to relax. PDE5 inhibitors help prolong this effect, making it easier to achieve and maintain an erection. Sexual stimulation is still required. Sildenafil, vardenafil and avanafil generally have a shorter duration of action than tadalafil, which can remain effective for up to 36 hours. The medicines differ in how quickly they work, how long their effect can last and how they are taken.
What They Can Not Do
PDE5 inhibitors do not increase relationship desire or create arousal on their own. Second-Line Options: Alprostadil
Alprostadil is a treatment used when tablets have not worked or are not suitable, and it works through a completely different mechanism.
Injection, Urethral Pellet and Cream
Alprostadil is chemically identical to a naturally occurring compound called prostaglandin E1, and it acts directly on the erectile tissue rather than relying on the arousal signal that tablets depend on. It is available as a self-administered injection directly into the penis, as a small pellet inserted into the urethra, or as a topical cream, a newer route for the same medicine.

Hormone Treatment

Low-Intensity Shockwave Therapy: Where the Evidence Stands
Low-intensity shockwave therapy is an emerging treatment, available privately in the UK, and it is not yet a first-line option because the evidence base is still developing.
A 2025 systematic review covering 21 trials and 1,357 men with mild to moderate ED found some evidence of benefit, but the certainty of that evidence was limited by small studies, non-standard protocols and short follow-up periods. It is also worth knowing that focused and radial pressure-wave devices are different technologies, and evidence for one does not automatically apply to the other.
A device carrying appropriate UKCA or CE marking indicates that it meets applicable regulatory requirements, but this does not by itself establish that the device is effective for ED.
