Any licensed site you can use comfortably and consistently is suitable. Mounjaro is injected under the skin (subcutaneously) once a week. The licensed sites are the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection.113

Your result is driven by your dose, your diet and activity, and time on treatment. In the SURMOUNT-1 trial, people on tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks; individual results vary.2

04 · Rotating your site

Rotating your injection site each week

Rotate the site with each dose. That is the one rule the licence attaches to every site, and it is more useful than agonising over which site is best.113

A simple routine keeps it effortless. You can alternate sides of the stomach week to week, or move between stomach and thigh. Whatever pattern you choose, write it down or log it in the free GLPTrackr companion app so you never have to guess where last week's injection went.

Each KwikPen contains four weekly doses, so one pen gives you four injections. A tidy rotation across a pen could look like this:

  • Week 1: left side of the stomach, at least 5cm from the belly button
  • Week 2: right side of the stomach, at least 5cm from the belly button
  • Week 3: front of the left thigh
  • Week 4: front of the right thigh

05 · Where not to inject

Stick to the licensed sites: the stomach area, the front of the thigh and the back of the upper arm (with a helper).1 Other sites are not licensed.

Within the stomach, the licence states a clear boundary of at least 5cm from the belly button, so treat that as a firm limit rather than a rough guide. If you are unsure whether a spot is acceptable, ask your prescriber before using it rather than improvising.

06 · When to inject

When is best to inject Mounjaro?

There is no best time of day. Mounjaro works the same in the morning or evening, with or without food, so what matters is injecting on the same day each week.1

Pick a day and time you will remember. Many people choose a weekend morning so that early side effects fall on days off. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1

Empty or full stomach is not a factor for the injection itself: you can inject with or without food.1

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Do not shorten the gap below that.

If you miss a dose

Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip it and take the next dose on your normal day. Never take two doses within 3 days of each other.1

Can Mounjaro be injected incorrectly?

Yes, mistakes are possible. The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

Follow the pen's instructions for the injection itself, including how long to hold it in place.

08 · Storage and travel

Storing pens and injecting on the move

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1 Keep the cap on to protect it from light, and never use a pen that has been frozen.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements because of the 30-day in-use allowance; for hot climates use an insulated pouch.1

Unopened pens should stay in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a day limit you have seen online, since those figures often come from other countries' labels.13

09 · Making it work better

How to make Mounjaro work better

The levers that matter are your dose, your diet and activity, consistency and time, not the injection site. Mounjaro is used alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 The medicine makes the diet easier to follow; it does not replace it.

In practice, that means:

  • Inject on the same day every week.
  • Follow the dose ladder your prescriber sets. Everyone starts on 2.5mg, which is about letting your body adjust rather than weight loss itself, and after at least 4 weeks the dose can rise to 5mg and then in 2.5mg steps to a maximum of 15mg.1
  • Remember that higher is not automatically better. Many people settle at 5mg, 10mg or 15mg, and your prescriber guides the pace.1
  • Protect protein and activity, and check the basics such as sleep and alcohol if progress stalls.1

10 · How fast it works

How quickly does Mounjaro start working?

Weight loss on Mounjaro is gradual by design, so think in months, not days. The first weeks on 2.5mg are an adjustment phase and losses are usually modest.1

Experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

By three months most people are still on the lower rungs of the dose ladder. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.1 The full averages in SURMOUNT-1 were measured at 72 weeks, not 12.2

For a worked example, 20.9% on the 15mg dose is about 21 kg for someone starting at 100 kg. That is a trial average over 72 weeks, and trial averages do not predict your own result.2

Plateaus

Plateaus of a few weeks are normal as the body adapts to a lower weight. Review food intake honestly, protect protein and activity, check sleep and alcohol, and ask your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.1

Food, alcohol and what your prescriber reviews

There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual.1 If you have questions about drinking with your medicines, ask your prescriber.

Treatment response is reviewed by your prescriber. NHS or private access

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment. Privately, Mounjaro is licensed for adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition, alongside a reduced-calorie diet and increased activity.1 A prescriber assesses your BMI, history and current medicines before deciding. Mounjaro should not be used during pregnancy and should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
