How to compare Mounjaro prices fairly

The fairest comparison is the same dose, for the same four-week pen, from a pharmacy you can verify. Many headline prices are "from" prices for the 2.5mg starter pen only. That is useful for your first month but says little about what you will pay once you step up. Mounjaro comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ Most people will spend months at more than one of them, so look at every dose you are likely to need.

Start with a short checklist. Is the price per pen, and does a pen cover four weeks? Is the consultation free, or is it added at checkout? Is delivery included, and is it cold-chain? Does the price change once you are past the first order? A low advertised price can grow once these are added, which is why we say the price shown is the price paid.

Next, look at how the pharmacy treats repeat orders. Some providers advertise a low first-month price and then move to the standard price. The monthly price matters less than the total for the route you are likely to take. Treatment starts at 2.5mg and steps up through the licensed strengths, so the first months are not the same price as your maintenance months.¹

06 · Fake pens and low prices

Why a very low price can be dangerous

A price far below the rest of the market is the clearest warning sign of a fake pen. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ Saving money is no use if the product is dangerous.

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.¹² Our registration number is 9012878 and you can look it up on the GPhC register. Genuine treatment always involves a prescription and a clinical consultation, so anyone offering to supply without a consultation or prescription is not operating legitimately.

Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked.⁷ Our supply is official LillyDirect supply, direct from Eli Lilly's UK distribution.

When your pen arrives, check it. A genuine pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C or out of the fridge for more than 30 days in use.¹³ If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

Unopened pens should be stored in the fridge until first use and keep until the printed expiry date. If you are unsure about a pen that has been out of the fridge before you start it, read the patient information leaflet and contact the pharmacy rather than guessing.¹³

Ask a few questions of any seller before paying. Is there a consultation reviewed by a GPhC-registered prescriber? Is the pharmacy on the register? Will the pen arrive cold, with a batch number? 07 · NHS versus private

NHS or private: what is realistic

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 or above with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you qualify under the NHS pathway, the cost to you is the usual NHS prescription charge in England, £9.90 per item, unless you are exempt. The NHS information on tirzepatide and NHS England's rollout guidance are the places to check current criteria, since they are updated as the rollout widens.⁵

Many people ask whether diabetics can get Mounjaro on the NHS. Mounjaro is also licensed for type 2 diabetes, and NICE has separate guidance for that use.⁸ Whether it is prescribed depends on your clinical team's assessment and local pathways, so ask your diabetes team or GP rather than relying on a general answer.

Private treatment is the route for people who meet the licensed criteria but are not yet in an NHS priority group. Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹ A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.

The two routes are not mutually exclusive in time. Some people pay privately while waiting for NHS access, then move across when they qualify. If you do, tell both clinical teams what you are taking so your records and doses stay consistent.

Safe ways to spend less on Mounjaro

The safe ways to lower the cost are using discounts, comparing providers and paying monthly.

Each Mounjaro KwikPen is designed and licensed to deliver exactly four weekly doses. Use the pen as designed, then dispose of it.

A pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.¹³ That is another reason not to try to make a pen last longer than its four weekly doses.

09 · Cheaper alternatives

Cheaper alternatives and the "poor man's Ozempic"

There is no supplement that works like Mounjaro, and cheapness is not evidence of benefit. Terms such as "poor man's Ozempic" and "nature's Ozempic" are marketing labels for low-cost supplements such as berberine. They are available cheaply without assessment, have no clinical oversight, and their dosing is not standardised.

Medicines are different. Wegovy (semaglutide) is a licensed once-weekly alternative. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17, which is the licensed maintenance dose for weight management.¹⁸ Its first pen is £99.99 as a first-order price. Mounjaro and semaglutide are different medicines, and Mounjaro's mechanism is different: it activates both GIP and GLP-1 receptors, unlike single-action semaglutide.¹

Liraglutide (Saxenda, and products such as Erly) is supplied at 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. For its figures and suitability, your prescriber can advise.

Wegovy tablets (oral semaglutide) are a prescription medicine like the injections, supplied only after a consultation. Because trial results for different medicines come from different studies, you should not compare tablet results against Mounjaro's as if they were head to head.

There is no honest single answer to which weight loss pill is best. The right choice depends on your BMI, health conditions, other medicines and preferences, which is what the prescriber's review is for. Over-the-counter products and imported pills with big promises are the ones to avoid.

If price is the deciding factor, the fair way to compare is by cost per month of a licensed medicine you can verify, together with the support included. Switching between licensed medicines should be done with a prescriber, who will advise on doses.

10 · What you are paying for

What the price buys: how Mounjaro works

The price buys a prescription medicine with trial evidence, a clinical review and a supply chain you can verify. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose KwikPen.¹

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹ Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages: individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.² Trial averages do not predict your own result, and they should not be read as a promise.

The pen itself delivers exactly four weekly doses and comes in six strengths from 2.5mg to 15mg.
