No universal end date applies to Mounjaro prescriptions. What stops a prescription is an individual decision: you reach a point where you and your prescriber agree to change course, a side effect makes it unsuitable, or you no longer meet the reasons for taking it.

How long can Mounjaro be prescribed for?

The trial evidence gives the useful context: SURMOUNT-1 followed adults for 72 weeks, and people on 15mg lost on average 20.9% of body weight, against 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2

SURMOUNT-4 is the most relevant study for duration. People who continued tirzepatide after 36 weeks lost a further 5.5% on average, while those who stopped regained on average 14% over the following year.14 That is why many people ask for a long-term plan rather than a short course, and why stopping should be a decision made with your prescriber.

04 · Planning the cost

Planning the cost if you stay on treatment

If you expect to stay on Mounjaro, plan in 4-week pens. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year of 13 pens costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. At 7.5mg a pen is £279.99, at 10mg £290.99 and at 12.5mg £319.99.

06 · What happens when you stop

What happens when you stop Mounjaro

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year.14 People who continued lost a further 5.5% on average.

A worked example makes the number concrete. For someone who weighs 100 kg when they stop, 14% is about 14 kg regained on average over a year. That is a trial average, and your own result will differ, but it shows why a planned exit matters.

The medicine does not vanish the day you stop. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. 07 · Planned exit, not stretching

A planned exit, not stretching pens

Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.

Talk to your clinical team rather than self-adjusting.

A sensible exit conversation covers three things:

  • whether a lower licensed weekly dose would hold your progress;
  • which diet and activity habits are already in place;
  • what you will do if appetite returns as the medicine clears.

Your prescriber tailors the plan to you.

08 · NHS and private access

NHS and private access: why some doctors say no

If your GP has declined to prescribe Mounjaro, the likeliest reason is how the NHS is phasing access. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4

The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions. Full rollout is expected to take years.6 Many people who meet the licensed criteria therefore choose regulated private treatment rather than waiting.

The licence is wider than the early NHS groups. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1

09 · BMI and stopping

At what BMI do you stop Mounjaro?

There is no BMI at which you must stop. The BMI figures in the licence, 30 or above, or 27 to 30 with a weight-related condition, describe who can start treatment.1

Reaching a lower BMI is therefore not an automatic cut-off. Whether to continue, lower the dose or stop at that point is a clinical conversation. A prescriber will weigh your BMI, medical history and current medicines, along with how you feel and what you want to maintain.

Bear in mind what the stopping evidence says: weight regain was seen in SURMOUNT-4 among those who stopped, so reaching a goal weight is often a reason to plan maintenance rather than simply end treatment.14 Your response is also reviewed: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

10 · Side effects and quitting

Some people stop because of side effects. In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped treatment because of side effects, compared with 2.6% on placebo.2

For any new symptom, discuss it with your prescriber and report it through the Yellow Card scheme.19 Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, and for signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

11 · Habits that protect progress

Habits that protect your progress either way

Whether you continue or stop, the habits you build on treatment matter. Increased physical activity is part of the licence, not an optional extra. Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle.

Start where you are and build. On injection days with early side effects, lighter movement still counts. Exercise also protects against regain if you later stop treatment.

The same goes for eating. Mounjaro works alongside a reduced-calorie diet, and trial results reflect that combination, not the medicine alone. Individual results vary.2 Building habits you can keep without the injection gives you firmer footing for a planned exit.

13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
