Pick a day that fits your week, so you do not forget a dose or double up by mistake. How Mounjaro's effect builds over time

Mounjaro's effect builds gradually because the dose is increased in steps. Treatment starts on the lowest strength and moves up, so higher doses are reached only after the early weeks.

The pen comes in the strengths 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 If you step up after at least 4 weeks on each dose, the first four pens (2.5mg to 10mg) cost £959.96 in total at standard prices. A new customer's first order is £40 less, and every later order is £25 less.

This staged approach is also why a slow first month is not a sign that the medicine is failing. The starter dose is designed as an introduction. Judging Mounjaro on the first pen alone is a bit like judging a course on its first lesson.

What the SURMOUNT-1 trial shows

In the SURMOUNT-1 trial, average results were larger at higher doses over a long period. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

The same trial reported that at 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

For a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. That is simple arithmetic on an average, not a promise.

Can you lose weight on 2.5mg of Mounjaro?

The 2.5mg pen is the starting strength, and its role is to begin the titration rather than to deliver the full effect.

How much you notice differs from person to person, and a quiet first month does not predict how you will do later.

Do not be tempted to stay on a low dose to avoid side effects, or to jump ahead to a higher dose to speed things up. Both decisions belong with your prescriber, who reviews your progress and tolerability each month.

How many hours or weeks until Mounjaro kicks in?

Mounjaro starts acting on appetite after the first injection, but the noticeable weight effect takes weeks and the larger average results took many months.

Many people describe the first sign as feeling full sooner at meals or less hungry between them, which matches how the medicine works.1 That tends to build as the dose rises, not arrive all at once.

The trial ran for 72 weeks, and that is the timeframe on which the headline averages are based.2

If you are several weeks into treatment and notice nothing, speak to your prescriber before changing anything. They can check that you are taking the pen correctly, and whether a dose step is due.

08 · What helps it work

How to speed up weight loss on Mounjaro

You cannot safely speed Mounjaro up, but you can make sure nothing is holding it back. The trial results were achieved alongside a reduced-calorie diet and increased physical activity, so these are part of the treatment, not extras.2

Practical steps that fit this:

  • Take the pen as directed, on a regular routine, and never take extra doses to catch up.
  • Use the appetite effect to eat smaller, balanced meals rather than relying on the injection alone.
  • Build activity into your week gradually.
  • Attend your monthly reviews and be honest about side effects, since a dose you cannot tolerate is not a dose you can stay on.

For what to avoid while on Mounjaro, read the patient information leaflet in full and discuss alcohol, other medicines and contraception with your prescriber.13 Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

09 · Pen and injection basics

Storage, cold injections and the pen

Mounjaro pens are stored in the fridge until first use, and unopened pens keep until the printed expiry date. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.1

10 · Feeling hot and new symptoms

Does Mounjaro make you feel hot?

Feeling hot is not on the list of side effects in the UK Mounjaro patient leaflet.13

Get medical help straight away for severe symptoms such as severe, persistent stomach pain, swelling of the face, lips, tongue or throat, or difficulty breathing, rather than waiting for a review.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · Stopping and NHS access

Staying on treatment, stopping and NHS access

Effectiveness depends on continuing treatment, and if you stop, the medicine takes roughly four to five weeks to clear your system. There is no way to speed this up, and nothing needs doing while it clears.1 That clearance time is why appetite effects do not vanish overnight, but it does not mean the benefit is permanent.

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
