When Do GLP-1s Start Working? A Realistic Timeline (October 2026)

Most people notice appetite changes within the first few weeks of a GLP-1 medicine, but measurable weight loss builds over months, not days. The starting weeks are an adjustment phase, and most visible progress comes as the dose steps up. Trials measured results over a long period, so judge progress by trend, not single weigh-ins. Timeline current as of October 2026.

A quick overview

What to know
before you start.

  • Some people feel less hungry soon after the first injection; others notice little until a higher dose. Both are normal.
  • Early weeks on the starting dose are for adjustment, so losses are usually modest.
  • Think in months, not days, and judge progress by the trend rather than any single weigh-in.
  • Plateaus of a few weeks are normal and are not a reason to double doses or add unlicensed products.
  • Your prescriber reviews response at set points, so you are never left to guess whether treatment is working.
In this guide

01 · The core answer

How long until a GLP-1 medicine starts working?

The honest answer has two parts. Appetite effects can begin early, sometimes after the very first injection. Weight loss you can measure builds gradually, and most of it comes after the dose has been stepped up. Tirzepatide, the medicine in Mounjaro, activates two gut-hormone receptors that help regulate appetite and slow how quickly the stomach empties.1 Most people feel fuller sooner and less hungry between meals.1

Experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.1 If you feel nothing at first, that does not mean the medicine has failed. The dose you respond to may still be ahead of you.1

StageWhat you may noticeWhat it means
First days after the first injectionPossible change in appetite; mild nausea or tiredness for some peopleCommon early effects that usually settle. A quiet start is also normal.
Starting weeks on 2.5mgSmaller portions, less food noise for some; modest early loss, part of it waterAn adjustment phase, not a test of whether the medicine works.
Dose steps from 5mg onwardMore consistent appetite control and a steadier downward trendMost measurable progress builds here, month by month.
First few months overallVisible but still modest loss compared with trial headlinesMost people are still on the lower rungs of the dose ladder.
Long term, towards the trial timescaleLarger cumulative change, with plateaus along the wayTrials measured the headline results at 72 weeks.

02 · Your next step

Is this relevant to you, and what should you do next?

If you are still deciding whether to start, the timeline matters because it sets expectations. Treatment is used alongside a reduced-calorie diet and increased physical activity.1 It is not a quick fix, and individual results vary. If you want to talk through whether it suits you, our guide on whether you should start a GLP-1 covers the decision in more detail.

If you have already started, your most useful tool is a simple log: weight on the same day each week, how hungry you feel, and any side effects. Looking at the trend over several weeks tells you far more than a single reading. The free GLPTrackr companion app is built for exactly this kind of tracking.

If you are comparing medicines, the timing differs a little by product and dose ladder. We explain the pace for when Mounjaro starts working compared with Wegovy and for how quickly Wegovy acts in separate guides.

When you are ready, the weight loss clinic explains how treatment works with us. The Mounjaro pen starts at the 2.5mg strength: your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99). Prices checked 10 October 2026. A free online consultation, reviewed by a GPhC-registered prescriber, comes first.

This page is information, not a substitute for medical advice. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. Individual results vary.1

03 · Why appetite changes first

Why appetite changes before the scales move

Appetite changes come first because GLP-1 medicines work on the signals that tell you when you are hungry and when you are full. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That is why the first thing many people report is a change in how they relate to food rather than a drop on the scales. Meals feel satisfying with less on the plate. The urge to snack between meals can ease.

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If they do not settle, or if they worry you, speak to your prescriber rather than waiting it out.

Appetite effect and weight loss are separate things. Your body needs time with a reduced intake before the trend on the scales becomes clear. Early readings also include water, so a quick drop in the first days is not a reliable signal of what comes next, and a slow start is not a reliable signal of failure.

How long tirzepatide stays in the body

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 Try to inject on the same day each week and follow the pen's instructions each time.

Side effects can be most noticeable in the first days after an injection, and they often ease as your body adapts to each dose. If you want a detailed look at the very first dose, see our guide on whether Mounjaro starts working right away.

None of this means that a faster feeling equals a better outcome. People differ in how sensitive they are to the medicine, and the dose that suits you is found step by step with your prescriber.

04 · The first weeks

The first weeks: what is normal on the starting dose

The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.1

The 2.5mg strength is not designed to be the dose that delivers your full result. It exists so your body can get used to the medicine before the dose rises. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 If you want a closer look, our page on when the 2.5mg dose starts working goes into this stage in detail.

Water weight and early readings

Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 This is why daily weighing can mislead. Weight moves up and down with salt, hydration, digestion and timing. A weekly weigh-in under the same conditions, read as a trend over several weeks, tells a much more useful story.

What you might feel

  • Feeling full sooner at meals, or finishing smaller portions without effort.
  • Less preoccupation with food between meals, for some people.
  • Mild nausea, altered appetite or tiredness in the days after an injection.
  • Sometimes very little at all, which is also a normal start.

When to get in touch

Mild effects that settle are expected. If you have effects that are severe, that do not ease, or that you did not expect, contact your prescriber or pharmacist. For severe, persistent stomach pain, chest pain, fainting, swelling of the face, lips, tongue or throat, or difficulty breathing, get medical help straight away. You can also report suspected side effects through the Yellow Card scheme.19

One practical point: do not judge the medicine on the first fortnight. Wait until you have had the chance to settle into a routine, and until your dose has been reviewed. A slow beginning is not a verdict, and neither is a fast one.

05 · Month by month

Month by month: how the dose ladder shapes your timeline

The timeline is set largely by the dose ladder, because the dose rises in steps rather than all at once. Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in a full range of strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

That means the first couple of months are spent on the lowest strengths. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures: in SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2

A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure — the dose you respond to may still be ahead of you.2

Maintenance is individual

Not everyone needs to reach the top strength. Some people settle on a middle dose that controls appetite well and is easy to tolerate. Your prescriber reviews this with you during follow-up, and repeat orders re-check suitability. There is no auto-shipping, so every order is a chance to review how you are getting on.

How Wegovy compares in shape

Wegovy follows a similar stepped pattern with its own ladder: 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 The principle is the same for both: the early doses are for adjustment, and the effective level comes later. You can read more on our Wegovy pen page.

For a quick overview of general pacing across the class, see our guides on how fast GLP-1 medicines work and how soon you can expect effects.

Planning for months rather than weeks protects you from disappointment. If you expect a steady staircase rather than a slide, each plateau and each slow week is easier to read for what it is.

06 · What trials show

What the trials show about pace and size of results

The largest tirzepatide weight trial shows results that build over more than a year, not weeks. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 They are not a prediction of what will happen to you, and they were measured at the end of a long period, not at the start.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

A worked example

For someone starting at 100 kg, 20.9% is about 21 kg. It does not tell you how quickly it would happen, and it is not a promise of your own result.

How to read trial numbers fairly

Averages hide a wide spread: some people lose more than the average, some less. A figure measured at 72 weeks should not be divided into weekly or monthly rates. Weight loss on Mounjaro is gradual by design, and trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.1

It is also worth knowing that stopping treatment changes the picture. Continued treatment was examined in a separate trial on maintaining weight reduction.14 Your prescriber can talk you through what that means for long-term planning.

07 · Signs it is working

How to tell if a GLP-1 is working

The clearest sign is a downward trend over several weeks, but there are earlier and quieter signs too. You do not need to wait for a large number on the scales to know the medicine is doing something.

Signs you can notice early

  • You feel full sooner and finish smaller portions.
  • You feel less hungry between meals, and thoughts about food take up less space.
  • Cravings feel easier to manage.
  • Your weekly weigh-in drifts in the right direction across several readings.

Signs you can notice later

Clothes may fit differently before the scales show much. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises — but they are why prescribers talk about weight management as health management rather than appearance.

That is also why a check-up, blood pressure reading or blood test can be a useful measure of progress alongside the scales. Ask your prescriber which measures matter for you.

What the formal review looks like

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Note that this applies after time on the highest tolerated dose, not simply after the first weeks of treatment.

First signs of weight loss

The earliest are usually behavioural: smaller appetite, easier portion control and a slightly looser feel to waistbands. Scale changes follow. Because early readings include water, treat the first few weigh-ins as noise and look for a pattern across weeks.

Keep notes on how you feel, not just numbers. A short weekly record of appetite, energy, side effects and weight gives your prescriber far more to work with at a review than a vague impression.

08 · Not losing weight

Why you might not be losing weight yet

If the scales are not moving, the most likely explanations are timing, dose or a plateau, not that the medicine has stopped working. Check each in turn before drawing conclusions.

Timing and dose

If you are still on the starting strength, the dose may simply be too low to deliver much effect. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Slower early progress does not predict failure — the dose you respond to may still be ahead of you.2

Plateaus are normal

Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Within the licence, the responses are: review food intake honestly (portions drift back up), protect protein and activity, check the basics (sleep, alcohol), and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1 A plateau is not a reason to double doses or add unlicensed products.1

A short checklist

  • Are you on the correct dose for your stage of treatment, and injecting on a regular weekly rhythm?
  • Have portions crept back up now that appetite feels easier to ignore?
  • Are you eating enough protein and staying active?
  • Are sleep and alcohol working against you?
  • Have you given the trend enough weeks to show itself?

When to talk to your prescriber

If you have been on the highest tolerated dose for a sustained period and the trend is still flat, speak to your prescriber. They will use the formal review criteria and your own history to decide what comes next. Our prescribing team offers ongoing monthly reviews and follow-up assessments for exactly this situation.

09 · Working out and prep

Working out on a GLP-1 and preparing to start

Treatment is used alongside a reduced-calorie diet and increased physical activity, so exercise is part of the plan rather than an optional extra.1 Trial results of weight loss came with diet and exercise, not instead of them.2

Activity once you have started

When plateaus occur, the licensed response includes protecting protein and activity.1 In practice, that means keeping regular movement going even when appetite is low, and not letting eating fall so far that you feel weak. Start with activity you enjoy and can repeat. If you feel dizzy, unusually tired or unwell around workouts, ease off and speak to your prescriber.

Many people find the first days after an injection are when tiredness or nausea is strongest, so it can help to plan lighter activity around then and heavier sessions when you feel well. Listen to your body rather than forcing a routine.

Preparing before your first injection

  • Complete the free online consultation honestly, including your medical history and any other medicines you take.
  • Choose a regular day of the week for your injection that fits your routine.
  • Read the patient information leaflet and the pen's instructions before your first dose.13
  • Set up a simple tracking routine, such as the free GLPTrackr app, and note a starting weight.
  • Think about meals: smaller, regular, protein-containing portions are easier when appetite shrinks.
  • Store pens in the fridge until first use, as the leaflet explains.

Needles, a sharps bin and injection guidance are included with every order, so you do not need to buy anything extra to begin.

Speeding things up safely

Within the licence there is no shortcut to pace: the dose steps are set by your prescriber, and exceeding them is not safe. What you can do is stay consistent with weekly injections, keep to your plan on food and activity, protect sleep, and attend your reviews. That gives the medicine the best chance to do its work at its own pace.

If you are weighing up other options, our guide on when orlistat starts working explains how that treatment differs.

10 · Fastest, NHS and ordering

Is there a fastest-working GLP-1, and how do you get started?

Both Mounjaro and Wegovy rise in steps, and in both cases the effective dose comes after the starting weeks. Be wary of any seller who promises specific results or a rapid start.

NHS and private routes

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Whichever route you take, the pace of results is set by the medicine and dose, not by who supplies it. Choosing a regulated pharmacy matters for safety. We are a GPhC-registered pharmacy (registration 9012878), and our Mounjaro comes through official LillyDirect supply, direct from Eli Lilly's UK distribution.

How ordering works

  • Complete the free online consultation: a short health questionnaire, with no charge and no obligation.
  • A GPhC-registered prescriber reviews it. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
  • Our registered pharmacy dispenses it, with same-day dispatch when ordered by 3pm.
  • Delivery is free next-day tracked delivery in temperature-controlled packaging. Saturday delivery is £14.99.

New customers save £40 on their first order with code FIRST40, and an automatic £25 reorder discount applies to every future order (code DOSE25). Klarna and pay-monthly options are available, with no subscription. The Mounjaro pen page shows current prices, and all doses are in stock.

Once treatment begins, our team provides ongoing monthly reviews, dose titration and side-effect support. Support is available Monday to Friday, 9am to 5pm, by email, account portal chat or phone on 0333 207 0413.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Do you lose weight the first week on GLP-1?
Some people do, but many do not, and neither is a problem. Some people notice appetite change and early loss in the first week, while others feel little until a higher dose. Early loss includes water weight, so a single reading is unreliable. The starting weeks are an adjustment phase, and the trend across several weeks tells you far more than any early weigh-in.
How long does it take a GLP-1 to kick in?
Appetite effects can appear within the first few weeks, and sometimes sooner, but measurable weight loss builds over months. The starting dose is an adjustment phase, and most measurable progress builds from 5mg onward on Mounjaro, month by month. Trials measured their headline results at 72 weeks, so it is sensible to think in months, not days, and to judge progress by the trend rather than individual weigh-ins.
How can you tell if a GLP-1 is working?
Look first for appetite changes: feeling full sooner, less hunger between meals and less preoccupation with food. Then watch the trend over several weekly weigh-ins taken under the same conditions. Waist size, blood pressure, cholesterol and blood-sugar measures also improved on average in trials. Your prescriber reviews your response formally, including after sustained time on the highest tolerated dose, so you are not left to judge it alone.
Why am I not losing weight on a GLP-1?
The commonest reasons are that you are still on a low starting dose, you are on a plateau, or you have not given the trend enough weeks. Plateaus of a few weeks are normal as the body adapts. Review food portions honestly, protect protein and activity, check sleep and alcohol, and ask your prescriber whether a dose step-up is appropriate. Do not double doses or add unlicensed products.
How can I speed up weight loss on a GLP-1?
There is no licensed shortcut, and you should never change doses yourself. The dose rises in steps set by your prescriber. What helps is consistency: inject on the same day each week, keep to a reduced-calorie diet, stay active, protect protein, sleep well and limit alcohol. Attend your reviews so your prescriber can decide whether a dose step-up is appropriate.
What are the first signs you are losing weight?
The earliest signs are usually changes in behaviour rather than numbers: smaller portions feel satisfying, you think about food less, and cravings feel easier to manage. Clothes may fit a little differently before the scales show much. Scale changes follow, but early readings include water, so look for a pattern across several weeks rather than reacting to a single weigh-in or a daily fluctuation.
Is there a fastest working GLP-1?
No licensed medicine is fastest for everyone. Mounjaro and Wegovy both step up in stages, with the effective dose coming after the starting weeks. Be cautious of anyone promising specific results or a rapid start, and talk to your prescriber about which medicine suits you.
Can I work out while taking a GLP-1?
Yes, and increased physical activity is part of the treatment plan alongside a reduced-calorie diet. Start with activity you enjoy and can repeat, and protect protein intake. Tiredness or nausea can be strongest in the days after an injection, so you may prefer lighter sessions then. If you feel dizzy, unusually tired or unwell when exercising, ease off and speak to your prescriber or pharmacist.
What should I do to prepare before starting a GLP-1?
Complete the consultation honestly, including your medical history and other medicines. Read the patient information leaflet and the pen's instructions, choose a regular injection day, and note a starting weight so you can track the trend. Store pens in the fridge until first use. Needles, a sharps bin and injection guidance are included with every order. Plan for steady progress over months rather than days.
What kills the most belly fat?
In trials, weight loss on tirzepatide came with average improvements in waist size, alongside diet and exercise, though individual results vary. If you are concerned about weight around your middle, speak to a prescriber about a safe, sustained plan rather than relying on quick fixes or unlicensed products.
