Using a Mounjaro pen after 30 days means you are injecting from a pen that has gone past its licensed in-use window. The guidance is clear: once a pen is in use, it can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.113

Nobody can tell you exactly what would happen to your particular pen, because it has not been tested beyond that window. The medicine could be weaker than the dose you dial, or the pen could be less safe to use. Neither problem is visible.

That is why we do not suggest 'trying it and seeing'. There is no reliable way to check at home, and the downside of a pen that has lost strength is a dose that does less than you expect. If you are worried you have done this, there is nothing to panic about on its own: stop using the pen and arrange a replacement.

Think about how you got here too. A pen holds four weekly doses, so on a normal weekly schedule it is finished well inside the in-use window. If a pen is lasting longer than that, something has changed: missed doses or a paused dose. Mention this to your prescriber so they can review your plan instead of leaving you to improvise.

03 · Why the limit exists

Why Mounjaro has an in-use limit

The in-use limit exists because the manufacturer has tested the pen only within a defined window. Once you begin using a pen, it is no longer sealed and cold. It is handled, carried, exposed to room temperature and pierced by a needle. The licence reflects what has been tested under those conditions.

Outside that window the testing stops. That does not mean the medicine turns harmful on a set day. It means the assurances about sterility and potency are no longer there. For a prescription injectable, that is reason enough to discard.

  • Strength: the dose you dial may not match what is delivered, because potency is outside the tested window.
  • Sterility: a pen used repeatedly over a long period has more opportunities to be contaminated, and nothing has been tested beyond the limit.
  • Appearance: a normal-looking pen proves nothing, so there is no visual check you can rely on.

Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be thrown away.1 Keeping it in the fridge does not make an in-use pen last indefinitely. 04 · Storage rules that matter

Storage rules that protect your pen

The storage rules are short: fridge before first use, never frozen, cap on, and a limit once in use. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

Many people ask whether Mounjaro needs to be refrigerated at all. Yes, until it is in use. After that, the pen can sit out not above 30°C for up to 30 days, which is helpful for short trips, but it is allowed rather than required. Many people simply keep the in-use pen in the fridge between doses, and that is fine. What matters is that the total in-use period does not go beyond the limit.

Heat is the other risk. The out-of-fridge allowance applies not above 30°C. A hot car, a sunny windowsill or a beach bag is not covered by the allowance. Our page on Mounjaro over 30 degrees explains what that means for warm weather and holidays.

Common mistakes we would encourage you to avoid:

  • Leaving the pen on a radiator shelf, window ledge or in direct sun.
  • Putting the pen at the very back of the fridge where it could freeze.
  • Taking the cap off and leaving the pen uncovered, which exposes it to light.
  • Losing track of the date you first used the pen.
  • Keeping a part-used pen 'just in case' after your dose has changed.

A simple habit helps: note the date of your first injection from a new pen on the box or in your phone, and work out the discard date at once.

05 · Unopened and out-of-date pens

Unopened pens and expiry dates

An unopened pen is judged by its printed expiry date, not by the in-use limit. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.13

The two limits are different, and they are often confused. The in-use limit starts at your first injection from a pen. The expiry date is fixed on the box and applies to a pen you have not started. People sometimes read the in-use limit as if it applied to a spare pen sitting in the fridge, which is not how it works.

What if an unopened pen has been left out of the fridge? Check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, so a number you have read in a forum may belong to a different market and may not apply to a UK pen. We would rather you ask the pharmacy that supplied it than guess.

What about a pen past its printed date? Do not use it. Expiry means the manufacturer no longer stands behind the pen's strength and sterility. Our guide to using expired Mounjaro covers this case in full.

How can you tell if Mounjaro has gone bad? You cannot reliably do so by looking. A pen can appear clear and unchanged while sitting outside its tested window. The dependable checks are the printed expiry date, the date you first used the pen, and whether it has been frozen or exposed to heat. If any of those raise a doubt, replace the pen.

06 · Dosing and pen questions

Dosing questions that come up with old pens

Most 'old pen' situations come from a dosing question, so it helps to know the licensed rules. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Each KwikPen contains four weekly doses.1

Missed a dose. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 A missed dose is a reason to follow this rule, not a reason to keep an old pen going for longer.

Injecting more often. People sometimes ask whether it is safe to inject every few days. Never take two doses within 3 days of each other. If you want to move your injection day, you can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Making a pen last longer. Altering doses outside the licence means unknown dosing accuracy and no safety data. Spreading a pen across a long period is exactly how people end up past the in-use limit.

07 · How long it stays in your system

How long Mounjaro stays in your system

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

This matters for old-pen worries in two ways. First, if you injected from a pen past its limit once, you do not need to do anything dramatic: the medicine will clear over the usual time, and you can return to your normal schedule with a fresh pen. Tell your prescriber so it is on record. Second, a gap in treatment is not the same as having no medicine in you. While a replacement pen is arranged, your prescriber is the right person to advise on your own schedule.

If you notice anything unexpected after using an old pen, speak to your prescriber, and consider reporting it through the Yellow Card scheme.19 Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

Travelling with an in-use pen

You can fly with Mounjaro, and the in-use allowance is designed for trips. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch.13

Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Check destination rules for travelling with prescription injectables before you fly.

The risk on holiday is not usually the flight. It is the gradual slide past the in-use limit: a pen started before you leave, a few weeks away, and then a pen back home that has quietly passed its date. Write the first-use date on the box before you go. If your trip is long enough that a pen could pass its limit while you are away, ask your prescriber or pharmacy in advance how to take supplies with you.

09 · Getting back on track

Getting back on track with a fresh pen

Replacing a pen and carrying on is usually straightforward, and progress is judged over months, not weeks. Weight loss on Mounjaro is gradual by design. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.2

That is reassuring if you have had a short disruption. A plateau is also normal: weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight. A plateau is not a reason to double doses or add unlicensed products.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. Missing a dose or replacing a pen does not change that review process, so keep your appointments and tell the team what happened.
