What happens if you take Mounjaro from a pen after 30 days?
A pen used after its in-use period is outside the licence, and sterility and potency are no longer assured. The licensed rule is plain: once a pen is in use, it can be kept out of the fridge for up to 30 days (not above 30°C), after which it should be discarded.113

What should you do if your pen is past its window?
Stop using it, do not top up the dose from a second pen, and contact your prescriber or pharmacy. Give them the facts: when you first used the pen, how it was stored, and when your last injection was. They can advise on the safest way to carry on with your treatment.

03 · Why the limit exists
Why does Mounjaro have an in-use limit?
The in-use limit is the line beyond which sterility and potency are no longer assured by testing.
Keeping the cap on matters too, as it protects the pen from light.1

04 · A pen partway through use
What if the pen has been in use for less time?
If the pen has been in use for less than 30 days (not above 30°C), it is still inside the licensed window, provided it has been kept as directed.
An in-use pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days, so most holidays need no special arrangements.1 Within that window, keep the cap on, keep the pen out of direct sun, and do not leave it in a hot car or on a radiator.
Do not invent your own shorter or longer limit. Some figures circulating online come from other countries' labels and do not match the UK licence.
There is also a simple sanity check built into the schedule. Each KwikPen contains four weekly doses.1 If you inject on the same day each week, the pen should be empty by the time the in-use window ends.
05 · Unopened pens and the fridge
How long does an unopened pen last in the fridge?
An unopened pen kept in the fridge keeps until the expiry date printed on the box.
The storage rule is to keep Mounjaro pens in a fridge at 2–8°C and never freeze them.1 If a pen has been frozen, even partly, do not use it.
The in-use limit and the expiry date are separate things. The expiry date applies to a sealed pen stored correctly. The in-use limit starts when you first use the pen and runs regardless of the printed date. A pen with a distant expiry date still has to be discarded when its in-use period ends.
What if an unopened pen has been out of the fridge, for example after a delivery delay or a power cut? Do not rely on a day figure you have read online. Check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1
06 · Taking a dose late
Can you take Mounjaro a few days late?
Yes, within limits, and the limit is counted from the scheduled injection time, not from the age of the pen. The rule in the product information is: if you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1
So if you realise three days after your usual day, you can still take that dose. The next step in the rule reads: if more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
You can also move your injection day on purpose. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 If you are running late and want to reset to a more convenient day, that rule is the one to follow.
Time of day is flexible. There is no best time of day, and Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1
07 · Pen problems
What if something seems wrong after an injection?
Any unexpected effect after an injection should be discussed with your prescriber, and you can report it through the Yellow Card scheme.19 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
Injection basics: Mounjaro is injected under the skin once a week, in the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Needles, a sharps bin and injection guidance are included with every order.
08 · When it seems to stop working
Why might Mounjaro seem to stop working?
Most of the time it has not stopped working: weight loss is not linear, and the pace is slower than people expect. Plateaus of a few weeks are normal and expected as the body adapts to a lower weight.
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1 Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.2
If progress stalls, the licensed responses are practical: review food intake honestly because portions drift back up, protect protein and activity, check the basics such as sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.
Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
Trial averages do not predict your own result. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. Individual results vary.2
09 · Travel and avoiding waste
How can you avoid wasting pens or running past the window?
Plan around the weekly schedule and keep spare pens sealed in the fridge. Travel is the situation where in-use limits and storage most often collide, so it is worth being specific.
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Check destination rules for travelling with prescription injectables before you fly.1 In-use pens can be kept at a temperature not above 30°C for up to 30 days, so most holidays need no special arrangements. For hot climates use an insulated pouch.
Habits that help in everyday use: pick a fixed injection day and a time you will remember, record the date you first used each pen, and keep the cap on between doses. 10 · How it clears
What happens in your body if you stop or pause?
If you pause, the medicine does not vanish at once. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
Mounjaro is licensed for weight management in adults and for type 2 diabetes, and it is used alongside a reduced-calorie diet and increased physical activity.1 NHS access is being phased in over several years, and NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition.4
