What Happens If You Inject Mounjaro Into Muscle? (October 2026)

Mounjaro is licensed only for injection under the skin (subcutaneously), so an injection that lands in muscle is outside how the pen is meant to be used. Do not inject again to make up for it. Watch for the usual side effects and contact your prescriber or pharmacist for advice. Seek immediate medical help for severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction.13 Current as of October 2026.

A quick overview

What to know
before you start.

  • Mounjaro is a once-weekly injection given under the skin, in the fatty layer, never deliberately into muscle.
  • If you think you hit muscle, do not take a replacement dose. Ask your prescriber or pharmacist what to do.
  • Expect the same side effects as any dose, mostly digestive, and know the serious warning signs that need immediate medical help.
  • The licensed sites are the stomach, the front of the thigh, or the back of the upper arm if someone else injects.
  • Report any unexpected reaction to your prescriber and through the Yellow Card scheme.
In this guide

01 · If it has just happened

What to do if you think you injected Mounjaro into muscle

Stay calm, do not repeat the injection, and treat that dose as given. Mounjaro is injected under the skin once a week, so the dose you took is your dose for the week.13 A second injection is not a fix. You should never take two doses within 3 days of each other.1

Next, contact the prescriber who reviewed your treatment, or a pharmacist, and describe what happened. Tell them the dose, the site you used and how the injection felt. They can advise you on what to watch for and on your next scheduled dose. If you are a customer of ours, the prescribing team can talk it through with you as part of your aftercare. You can reach support Monday to Friday, 9am to 5pm, by phone on 0333 207 0413 or by email at support@everydaymeds.co.uk.

Most people can carry on with their normal routine while they watch for symptoms. Keep water within reach, eat small, slow meals, and note anything unusual. Mounjaro slows stomach emptying, so a heavy meal can sit uncomfortably whichever way the injection went in.13

02 · When to get help

Warning signs that need urgent medical help

Serious side effects are uncommon, but they need immediate attention whether or not the injection went in correctly. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis). Get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems). Seek immediate medical help too for signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.13

If you also take insulin or a sulfonylurea, watch for low blood sugar: shakiness, sweating or confusion. Your prescriber may need to adjust those medicines.1 If you feel faint, have chest pain or a racing heart, get medical help straight away.

03 · What next

Getting back to a normal routine

After a mistake, the sensible goal is a calm return to your usual weekly day and correct technique. Your prescriber decides whether anything about your next dose needs to change. If you want broader context on injection errors, our guide to injecting Mounjaro in the wrong place covers the general picture. You can also read about the vein question separately. For treatment questions, our weight loss clinic hub brings the guides together, and the Mounjaro pen page explains how the free consultation and prescriber review work.

04 · Why under the skin

Why Mounjaro goes into fat, not muscle

Mounjaro is injected under the skin because that is how the medicine is licensed and how the KwikPen is designed to be used. The product information describes a subcutaneous injection once a week, on the same day each week, at any time of day, with or without food.13 The pen's needle and dosing mechanism are built around that single route.

Mounjaro goes into the layer of fat just beneath the skin. This is the subcutaneous tissue. It is a routine route for many once-weekly injectable medicines. The leaflet gives no instruction for any other tissue, so staying with the licensed route is the only approach the instructions support.

Tirzepatide is the active ingredient. It activates two natural gut-hormone receptors, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 It has a half-life of about five days, which is why a single weekly injection is enough. After a final dose it takes roughly four to five weeks to clear the system.1 Nothing needs doing while it clears, and there is no way to speed it up.

05 · Licensed injection sites

Licensed injection sites and how to rotate them

There are three licensed injection sites: the stomach area, the front of the thigh and the back of the upper arm. On the stomach, inject at least 5cm from the belly button.13 Rotate the site with each dose.

Each site suits different people. The abdomen is the easiest to see and reach for most. The thigh is a common alternative if you prefer to sit down and look at the pen. The upper arm is only suitable when someone else injects, because it is hard to reach with the pen yourself and a poor angle raises the chance of a technique error.

If you are slim or have little fat in one area, choose a site where you can comfortably gather a little skin and fat. Reading the leaflet's step-by-step instructions before each dose takes less than a minute and helps you stay consistent. Our order pack includes needles, a sharps bin and injection guidance.

Love handles and stretch marks

Many people ask about areas outside the three named sites. Anywhere other than the stomach, thigh or upper arm is outside the licence. Our guides on injecting Mounjaro in love handles and injecting Mounjaro in stretch marks explain this in more detail. For a stretch mark that falls within the stomach or thigh, the safest approach is to choose a nearby area of ordinary skin and rotate. If you are unsure whether a spot is suitable, ask your pharmacist or prescriber.

06 · Technique tips

Right technique: reducing the chance of a muscle injection

Good technique mostly comes from reading the pen's instructions and not rushing. Always follow the pen's instructions for how to place, hold and release the injection, and for how long to keep it in place. The leaflet is the reference for these details.13

A few practical habits help:

  • Choose a licensed site and rotate it with every weekly dose.
  • Sit or stand in a position where the skin of that site is relaxed, not tensed.
  • Use a clean area of skin, and avoid sites that are bruised, tender or damaged.
  • Keep the pen cap on between doses to protect it from light.1

If you have never injected before, ask your pharmacist or prescriber to talk you through it before your first dose. Many people find the first injection the hardest, and confidence grows quickly.

07 · Side effects to expect

Side effects after any injection

Most Mounjaro side effects are digestive, and they can occur after any correctly given dose. The commonest are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.113 Side effects are most likely when starting treatment or moving up a dose. For most people they ease within days to a few weeks as the body adjusts.13

This matters because after a suspected muscle injection it is easy to blame any symptom on the mistake. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Local soreness or a small mark at the injection site is also listed as an injection-site reaction. If that is all you notice, it is in keeping with the leaflet.

Whether your symptoms are more intense than usual is a question for your prescriber, not for guesswork. Describe the timing, what you felt and how long it lasted. Do not use online posts to judge what is normal, because they cannot account for your dose, your health or your medicines.

Serious side effects

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis). Seek immediate medical help too for pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13

Reporting a problem

If something unexpected happens, you can report it yourself through the MHRA Yellow Card scheme as well as telling your prescriber.19 Reports help regulators monitor medicines in everyday use.

08 · Out of date pens

Out-of-date pens, storage and other injection mistakes

Never inject from a pen that is past the expiry date printed on the box. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.1 Once a pen is past that date, sterility and potency are no longer assured, so do not use it. Contact the pharmacy that supplied it for advice and a replacement route.

For storage, keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.1

Other injection errors

Injection mistakes are not limited to the tissue layer. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next on your normal day.1

09 · Other GLP-1 injections

Semaglutide and other GLP-1 injections

People often ask the same question about semaglutide, and the principle is the same: use the injection route in the product information. Wegovy pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, with the dose stepping up after at least 4 weeks.18 Each medicine has its own leaflet and its own instructions, and they are not interchangeable with Mounjaro's.

The injection technique for one pen does not carry over automatically to another. If you switch products, read the new leaflet from the start and ask your prescriber to check you are comfortable with the new pen. Liraglutide products such as Saxenda are supplied at 6mg/ml and are daily injections, unlike once-weekly Mounjaro and Wegovy, so the routine is quite different.

If you inject Wegovy into muscle by mistake, the sensible response is the same: do not repeat the dose, and ask your prescriber or pharmacist.

10 · Contraception and diet

If you take the oral contraceptive pill, Mounjaro slows stomach emptying, which can reduce how well the pill is absorbed. The advice is to switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only; implants, coils and injections are unaffected.17

Day-to-day habits help you feel steady between injections. No food is banned, but very fatty, fried or very sugary food and large portions tend to sit heavily because the stomach empties more slowly.1 Drinking enough also matters: reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.1

You can change your injection day if you need to, as long as at least 3 days (72 hours) have passed since your last dose.1 Tell your prescriber if the mix-up has left you unsure when to take the next injection.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
What happens if you inject Mounjaro into muscle?
Mounjaro is licensed only for injection under the skin, so a muscle injection is outside how the pen is meant to be used. Do not take another dose to compensate. Watch for the usual side effects, mostly digestive, and contact your prescriber or pharmacist. Get medical help straight away for severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction.
Do you inject Mounjaro into fat or muscle?
Into fat. Mounjaro is injected under the skin, into the fatty layer just beneath it, once a week on the same day. The licensed sites are the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site each dose, and follow the pen's instructions.
Why do you inject Mounjaro into fat?
Because that is how the medicine is licensed and how the KwikPen is designed to be used. The product information describes a subcutaneous injection once a week, and it gives no instruction for any other tissue. Your prescriber can explain more about your own treatment, including any question about technique or injection sites.
Should I take another dose if I think I injected into muscle?
No. Treat the dose as given and do not repeat it. The missed-dose guidance states you should never take two doses within 3 days of each other. Contact your prescriber or pharmacist, describe what happened and ask how to handle your next scheduled injection. Do not alter your dose yourself.
What happens if you inject out-of-date Mounjaro?
You should not do it. Pens keep until the expiry date printed on the box, and after that sterility and potency are no longer assured. If you have already injected from an expired pen, contact your prescriber or pharmacist for advice, watch for side effects, and use an in-date pen for your next dose.
How do you know if an injection went into muscle?
You cannot always tell. Technique and body shape both play a part. If the injection felt unusual, or you are worried, tell your prescriber or pharmacist what you did and where. Check for the usual side effects and the serious warning signs, and take your next dose in a licensed site, following the pen's instructions.
Can I report a mistake or side effect?
Yes. Tell your prescriber first, so they can advise you on your treatment. You can also report side effects yourself through the MHRA Yellow Card scheme. Reports help regulators monitor medicines used by many people. If you have signs of pancreatitis, gallbladder problems or a severe allergic reaction, seek immediate medical help instead.
