Intravenous use is not part of the Mounjaro licence, so what follows is outside what has been tested. Mounjaro (tirzepatide) is a once-weekly prescription medicine, and its product information describes injecting it under the skin.113 Anything beyond that is outside what the manufacturer's testing and approval cover.

What to do if you think you hit a vein

Contact a clinician as soon as you reasonably can, and tell them exactly what you did. Your pharmacist, your prescribing team or the NHS online urgent advice service can all talk it through. If you feel seriously unwell at any point, get medical help straight away by calling 999 rather than waiting for a call back.

  • Stay calm and stay put for a while. Don't drive or do anything demanding until you know how you feel.
  • Note what you saw. Blood at the site, a different sensation on insertion, or a pen that behaved oddly are all useful details to pass on.

Why Mounjaro is injected under the skin

Mounjaro is injected subcutaneously, once a week, on the same day each week, at any time of day, with or without food.1 That is the route used in the clinical programme and described in the leaflet.

The pen delivers the medicine into the fatty layer just beneath the skin. This is the setting the dose ladder, the weekly schedule and the trial results were built around. The SURMOUNT-1 trial results quoted for Mounjaro, such as an average of 20.9% body weight lost on 15mg over 72 weeks, came from people using the licensed route alongside diet and exercise, and individual results vary.2

Because a medicine's dose, timing and safety profile are all tied to how it is given, changing the route changes the picture.

04 · Knowing if it happened

How do you tell if you injected into a vein?

You can't always tell whether a needle has reached a vein. The pen is designed for an under-the-skin injection.1

What people notice in practice is usually about the site: a little blood when the needle comes out, a bruise appearing later, or a sharper sensation than usual. These observations alone don't tell you where the medicine went. A small spot of blood or a bruise is a common aftermath of any injection and doesn't prove anything about the route.

So the practical approach is this:

  • If the injection felt normal and nothing seems unusual, carry on and keep an eye on how you feel.
  • If something felt clearly wrong, or you saw more bleeding than expected, mention it to your pharmacist or prescriber.
  • If you feel unwell afterwards, get medical help straight away and say that you have just injected Mounjaro.

When in doubt, ask. A pharmacist would far rather hear about a false alarm than miss a real one.

What happens if a needle goes into a vein in general?

Each medicine has its own leaflet, and the leaflet or a pharmacist is the right place for specifics.

As for healing, minor marks and bruises from a needle are usually a local matter that your pharmacist can assess. If a mark is getting worse rather than better, or you have pain, swelling or other symptoms that concern you, speak to a clinician.

Nerve damage is another frequent worry. If you notice numbness, tingling or pain that doesn't fade after an injection, tell your prescriber or pharmacist and report it via Yellow Card.19

06 · Injecting correctly

How to inject Mounjaro correctly

The best protection is good technique, every week. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection.1 Rotate the site with each dose.13

Some habits help you stay in the right place:

  • Choose a fleshy area. The licensed sites are where there is a layer of fat under the skin. Avoid spots that are very thin, scarred or already bruised.
  • Rotate steadily. Moving around within a site and between sites avoids repeated injections in the same patch.
  • Follow the pen's instructions. The leaflet explains the steps, including how long to hold the pen in place.
  • Don't rush. Many mistakes happen when someone injects in a hurry or at an awkward angle. Sit or stand comfortably, with the site relaxed.
  • Use the arm only with help. The back of the upper arm is licensed only when someone else gives the injection.

07 · Reading the pen

Dial, window and the pen's safeguards

The Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

This matters after any injection mishap too. If you are unsure whether a dose went in properly, don't try to top it up from a partly empty pen or work out a "fix" yourself. Ask your prescriber.

A well-handled pen is part of a safe injection. Keep the cap on to protect it from light, store it as directed, and never use a pen that has been frozen.1 Pens are kept in a fridge at 2–8°C. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.

08 · Next dose and timing

What about your next dose after a mistake?

Your prescriber is the right person to tell you whether and when to inject again after a possible vein injection.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can also change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 These rules show why an impulsive extra injection is a bad idea: the schedule is designed to keep doses well apart.

09 · Medicine clearance

How long Mounjaro stays in your system

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

If you are worried, the useful step is to talk to a clinician, who can check how you are and decide what monitoring, if any, makes sense.

If you take insulin or another medicine that can lower blood sugar, mention that when you call. Doses of those medicines may need adjusting alongside Mounjaro, which is another reason to involve your prescriber instead of managing it alone.

11 · When to get urgent help

When to seek urgent help

Get medical help straight away if you feel seriously unwell after any injection. That includes feeling faint, very unwell, struggling to breathe, chest pain or a racing heart; call 999 or go to A&E. Don't wait for an appointment, and tell the responder that you have injected Mounjaro and how. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

For less urgent concerns, such as a mark at the site that isn't improving or a general sense that something is off, speak to your pharmacist or prescriber the same day if you can.
