The first thing to establish is whether the pen is in use or unopened, because the rules differ. Mounjaro pens are meant to be kept in a fridge at 2–8°C and never frozen.1 The one built-in flexibility applies to a pen you have already started: once in use, it can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13
So a pen you started this month and left in a bag, a drawer or on a kitchen counter, away from heat, is usually within the licensed allowance. A pen that is unopened is a different case: it is meant to stay in the fridge until first use. If it has been out, the safest route is to ask the pharmacy rather than rely on a number you saw online.
Do not panic and do not improvise. Do not inject a pen you are unsure about, do not try to compensate by taking extra medicine later, and do not put an out-of-date or doubtful pen back into circulation. Write down roughly how long the pen was out, where it was, and whether it was near a radiator, in a car or in direct sun. That information is what a pharmacist needs to give you a sensible answer.

02 · The honest answer
Is Mounjaro still safe and effective if it was not refrigerated?
Within the licensed allowance, yes: the storage conditions are set by the manufacturer so that a pen in use remains suitable for its full life. Outside the allowance, nobody can promise what the pen contains, and that is the honest position. When a pen has spent longer or hotter than the tested conditions, sterility and potency are no longer assured, which is why the instruction is to discard rather than gamble.
You will see confident answers online that give a precise number of days for unopened pens. Be cautious with them. Figures circulating online often come from other countries' labels, and UK labelling is what applies to a UK pen. For an unopened pen, the right source is your patient information leaflet and the pharmacy that supplied it.
There is also no reliable way to tell by eye whether a pen is fine. Do not rely on a visual check for a pen that has been mishandled. If the pen looks wrong in any way, or you simply do not trust it, do not inject it.

04 · The in-use pen allowance
The in-use pen allowance explained
The most important storage fact for day-to-day life is the in-use allowance. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1
In practice, the allowance means you do not have to rush back to the fridge each time you inject. Many people take the pen out, inject, and then return it. Others keep the current pen in a cool, dry place at home or in a travel bag. Either approach is within the licensed conditions as long as the temperature stays below the limit and the pen is not kept beyond its in-use life.
The allowance is counted from the point the pen is first used, not from the day it was delivered. The allowance applies to the temperature condition stated, so it is not a licence to leave a pen in a hot car, on a sunny windowsill or beside a heater. Heat is a different risk from simply being out of the fridge. If a pen has been somewhere genuinely warm, treat it as a question for the pharmacy rather than assuming the allowance covers you.
The pen is also designed to hold four weekly doses, so if you inject on the same day each week, a pen is typically finished well inside the allowance. A pen that is still holding medicine long after you would expect is worth a closer look: check whether you have skipped doses and whether the dial will still deliver a full dose.
After the allowance runs out, discard the pen, even if medicine appears to remain. Finally, keep the cap on between doses. The cap protects the pen from light, and that matters whether the pen is in the fridge, in your bag or on the counter.13
05 · Unopened pens out of the fridge
Unopened pens left out of the fridge
An unopened pen is meant to stay in the fridge until you first use it, and it then keeps until the expiry date printed on the box.1 The generous in-use allowance does not automatically transfer to a pen you have not started, and that distinction trips many people up.
So what should you do if a sealed pen was left on a doorstep, in a courier van or on a kitchen worktop? Check with the pharmacy before using it, rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, so a number quoted confidently in a forum may not apply to a UK KwikPen.
When you contact the pharmacy, have these details ready: the strength on the box, the batch number and expiry date printed on it, roughly how long the pen was out, where it was kept, and whether it was in sun or near heat. If the pen arrived with a delivery, tell us when it arrived and what the packaging looked like. If the pharmacy advises against using the pen, do not try to rescue it by putting it in the fridge and hoping. A pen with an uncertain history is replaced, not rehabilitated. 06 · Frozen and hot pens
Frozen pens, hot pens and light
Cold storage is not the only risk: a pen can be harmed at either extreme. The rule for freezing is blunt. Never freeze Mounjaro, and never use a pen that has been frozen.1 That is why a pen pushed against the back wall of an overcooled fridge, or left in an airline hold, should be treated with suspicion.
Frozen pens are a particular concern on journeys. Hold luggage can fall below freezing, so pens should travel in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription.1 If you suspect a pen has frozen, the instruction is to discard it. Heat is the opposite problem. The in-use allowance is written for temperatures below the stated limit, and for hot climates the advice is to use an insulated pouch.1 A pen left in a parked car on a sunny day is a different situation from a pen in a cool handbag. Light is the quieter risk. Keep the pen cap on so the pen is protected from light, whether the pen is in the fridge, in a bag or on a shelf.13 Taking the cap off only when you are ready to inject is a small habit that costs nothing.
For pens in use there is a clear boundary, there is a clear rule for freezing, and for unopened pens the instruction is to check with the pharmacy. Anything that does not fit neatly belongs in that last category.
If you are the sort of person who likes to double-check, keep a small card or note in the fridge with the date you opened the current pen and the injection day. It is a low-effort way of avoiding the most common storage mistake, which is simply losing track of how long a pen has been in use.
07 · Does it still work?
Does Mounjaro still work if it was not refrigerated?
The answer to whether Mounjaro still works is yes when the pen has stayed within its licensed storage conditions, and uncertain when it has not. The tricky part is that a pen mishandled outside those conditions may not look any different, and you may not feel an obvious change after injecting.
Tirzepatide is the active ingredient. It activates two natural gut-hormone receptors at once, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. If a pen has lost potency, you would not get the effect the dose is meant to deliver, but because early weeks are gradual anyway, you may not notice quickly.
That is exactly why we do not tell people to inject and see. Results on Mounjaro are gradual by design, and you judge progress by trend over weeks and months, not by a single weigh-in or a single injection. If you use a doubtful pen, a slow week could be caused by the pen, the dose, a plateau or ordinary variation, and you would have no way of telling which. Trial figures describe average results from properly stored medicine alongside diet and activity, and individual results vary, so they are not a yardstick for a pen of uncertain history.
If you have injected from a pen and then realised it had been stored incorrectly, do not worry unduly, but do tell the pharmacy or your prescriber. Watch for the usual early effects, which are mild nausea, altered appetite and tiredness in the first days after an injection and usually settle.1 Get medical help straight away for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen, fever or yellowing of the skin or eyes, or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13

08 · Travel and delivery
Travelling with Mounjaro and receiving deliveries
Travel is where most unrefrigerated pens come from, and the good news is that ordinary holidays rarely cause a problem. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, and ideally pack them in a cool bag with the prescription label or a copy of your prescription.1
In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch.1 If you are taking an unopened spare pen, remember it is meant to stay in the fridge, so plan where it will be kept at your destination. A hotel room fridge is the usual answer, but make sure it does not freeze items placed against the back panel.
Check the destination's rules for travelling with prescription injectables before you fly. Carry your needles and a sharps solution too, and keep everything together with your prescription paperwork. A cool bag is a small accessory purchase and is more useful than any fancy gadget.

09 · What to do after a mistake
What to do after a storage mistake
The best response to a storage mistake is calm and methodical. Begin by working out which pen you have: in use or unopened. Then note how long it was out, where it was and whether heat or freezing was involved.
If the pen is in use, within its allowance and kept cool, carry on. If it has passed the in-use limit, discard it. If it is unopened and has been out of the fridge, check with the pharmacy before using it, and do not rely on a remembered day limit. If it has been frozen, discard it.
The next question is timing: what if you now have to wait for a replacement and miss your injection day? The licensed rule is clear. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
That last point matters because the temptation, after a gap, is to catch up. Do not. Extra doses raise the risk of side effects and are not part of the licensed approach. You can also change your weekly injection day if needed, as long as there are at least 3 days between two doses, which can help line a replacement pen up with a sensible day.1
Tirzepatide has a half-life of about five days, so a short gap does not mean the medicine disappears at once; after a final dose it takes four to five weeks to clear the system.1
If you have injected and feel unwell, get medical help straight away for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen, fever, yellowing of the skin or eyes, or signs of a severe allergic reaction. If you also take insulin or a sulfonylurea, watch for low blood sugar, such as shakiness, sweating or confusion, and talk to your prescriber about adjusting those medicines.13 Report suspected side effects through the Yellow Card scheme.19
10 · Building a safe storage routine
Building a storage routine that prevents the problem
A simple routine removes nearly all the risk. Unopened pens go in the fridge at 2–8°C. They are never frozen. They keep until the expiry date printed on the box. Once you start a pen, you have a flexible window, but the clock is running, so note the date.1
Choose a consistent injection day and time. There is no best time of day: Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week. Many people pick a weekend morning so early side effects fall on days off.1 A fixed routine also gives you a natural moment to check the pen: is the cap on, is the strength right, is it within its in-use time?
Inject under the skin on the stomach area at a sensible distance from the belly button, the front of the thigh, or the back of the upper arm if someone else injects you, and rotate the site each time. Follow the pen's instructions for the injection itself.
As you move up the dose ladder, you will change pens. Everyone starts on 2.5mg, and after at least four weeks the dose can rise to 5mg, then step up by 2.5mg steps at minimum intervals up to a maximum of 15mg, depending on response and tolerance. A pen of a new strength is a good moment to reset your storage habits: put the new box in the fridge, move the old finished pen to your sharps bin, and record the new start date.
Keep a short list of what to do on delivery day. Take the parcel in promptly, check the strength against your prescription, put the pen in the fridge, and mark the next injection day. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
