How quickly, and how strongly, differs from person to person, so be wary of anyone who gives you a fixed timetable.

The reduced-calorie diet and increased physical activity that Mounjaro is used alongside are the parts of your routine that stay in your control during a break.1 Keeping them going is the most practical thing you can do while the medicine clears.

A planned break is a decision you make with your prescriber, for reasons such as side effects, a change in circumstances or simply wanting to see how you manage. An accidental gap is a travel mishap, a delayed delivery or a forgotten injection. Both end in the same place for your body, but the planned one lets you prepare.

04 · How long it takes to clear

How long Mounjaro takes to leave your system

Mounjaro takes four to five weeks to clear after your final dose. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

This is why the effect of a break does not arrive overnight. Levels fall gradually, so most people notice the change in appetite build over the following weeks rather than the next day. It is also why a single late dose is usually a minor event, while several missed doses in a row is a real pause.

The same slow clearance matters for planning. If you are thinking about a procedure, a pregnancy or a change in other medicines, ask your prescriber how the clearance period fits your situation, rather than guessing from the half-life alone. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.113

If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.113

Many people want to move their injection day after a gap. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

06 · Restarting after a break

Restarting Mounjaro after a break

Restarting after a longer break should be agreed with your prescriber, because the right dose to resume on is a clinical decision, not something to guess. Everyone starts Mounjaro on 2.5mg once a week, and the starting dose is about letting your body adjust, not about weight loss itself.111

The licensed ladder steps up from 2.5mg to 5mg after at least 4 weeks, then by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly.1 If you have been off the medicine for a long time, do not assume your body will tolerate the strength you were last on. Your prescriber can assess how long you have been off, how you tolerated treatment before and which strength makes sense now.

Early effects such as mild nausea, altered appetite and tiredness in the first days after each injection are the commonest and usually settle.1 Expect to feel some of this again if you restart on a lower dose. That is the adjustment phase, not a sign that something is wrong.

07 · Storing pens during a gap

What to do with your pen during a break

A pen you have started has a limited life even if you stop injecting. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.113

That matters if you pause part-way through a pen. Each KwikPen contains four weekly doses, but a pen in use must still be discarded after 30 days (not above 30°C), so do not hold a part-used pen for a restart weeks later.1 Keep the cap on to protect it from light, and never use a pen that has been frozen.

Unopened pens belong in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1

08 · Why not stretch or shrink doses

Why stretching doses is not a safe middle ground

It can be tempting to avoid a full break by injecting less often or in smaller amounts. This is not a licensed approach.

Altering doses outside the licence means unknown dosing accuracy and no safety data.1

There is a legitimate alternative. If the reason you want a break is side effects, raise it with the prescribing team first. A supervised dose change keeps you inside the licence, which a self-made schedule does not.

09 · Results, review and stopping

What a break means for your results

Mounjaro is a gradual treatment, and the results quoted in trials depend on staying on it. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Your prescriber reviews how you are responding. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4 A planned stop is a legitimate outcome of that conversation, and it works best when you have a diet and activity routine in place first.

Judge progress by trend rather than individual weigh-ins, because early loss includes water weight and the trend over weeks matters more than any single reading.1 The same applies after a break: expect fluctuation and look at the direction over several weeks.

10 · Planning a sensible break

How to plan a break so it goes well

A planned break goes better when you decide the reason, the length and the restart plan in advance. Here is a practical way to approach it.

  • Talk to your prescriber first. They can tell you whether a pause suits your situation and what to expect on restarting.
  • Keep your routine. The treatment is used alongside a reduced-calorie diet and increased physical activity, so keep those going while the medicine clears.1
  • Do not stockpile injections. Do not take doses early or close together to "front-load" before a break. Never take two doses within 3 days of each other.1
  • Look after your pens. Remember the in-use limit of 30 days (not above 30°C) and the fridge range of 2–8°C.1
  • Report new symptoms. Anything unexpected during or after a break should go to your prescriber, and you can report suspected side effects through the Yellow Card scheme.19

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
