It helps to think of the starter pen as a settling-in period rather than the main treatment. A rising number one morning, in the middle of a downward trend, is not a failure. Weighing at the same time on the same day each week gives a cleaner picture than daily checks.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Notice what the figures describe. They are results at the maintenance doses reached through the ladder, measured over a long period. They are not results of the 2.5mg starter pen over a few weeks. Reading them as "what the first dose does" would be a mistake, and it is a common one.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

A worked example makes the average easier to picture. For someone starting at 100 kg, 20.9% is about 21 kg. That is a trial average at the highest dose over the full trial period, and trial averages do not predict your own result. Some people lose more, some less, and many do well on a lower maintenance dose.

The takeaway for the starting dose question is simple. The trial shows that results are dose-related at the maintenance stage, but it does not say that you need the top dose, and it does not say the starter dose should produce large changes. Many people settle at 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1

One more point of honesty: a trial population is not everyone. People in a trial are monitored closely and follow a structured programme. In everyday life, adherence, side effects, other medicines and lifestyle all shape the outcome. Use these figures as a sense of scale, not as a forecast, and take any specific plan to your prescriber. The Mounjaro KwikPen comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Everyone starts Mounjaro on 2.5mg once a week.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Two details deserve emphasis. First, those intervals are minimums, not targets. You can stay on a dose for longer when your prescriber thinks it is right. Second, the ladder has a ceiling and you do not have to reach it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1

Each pen contains four weekly doses, so a pen lasts roughly a month at one strength.13 The first four pens, stepping through 2.5mg, 5mg, 7.5mg and 10mg, would use four pens at the standard prices, which currently total £959.96 before discounts. 09 · Injecting and missed doses

Getting the routine right matters from the very first dose. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.13

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.13 Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off, which is a sensible choice while you are getting used to the starter dose.

The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Needles, a sharps bin and injection guidance are included with every order.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 This is useful if your routine changes, for example around travel or shift work.

Why does this matter for weight loss? Consistent weekly dosing keeps the medicine steady in your system, and it makes the effects of each dose easier to read. If you swap days at random or double up after a miss, you blur the picture and risk more side effects. Sticking to the rules above also makes your review conversations more useful, because your prescriber can trust that the dose you report is the dose you took.

10 · Why not microdose

Why not microdose or stretch doses

People often ask whether they could start lower than 2.5mg, or spread a pen out, to ease in more gently. The safety position is clear.

The licensed dose ladder already starts low for tolerability, at 2.5mg. That is the gentle option, and it has been designed and tested as such.

Altering doses outside the licence means unknown dosing accuracy and no safety data.1 Trying to work around that design risks giving the wrong amount, with no way to know what you actually received.

There is also a practical argument. Trial results, including the average figures for each maintenance dose, describe what happens when people follow the licensed pathway.

So what should you do if the starter dose feels too strong? Speak to your prescriber early, rather than waiting it out or adjusting alone. And if you experience a side effect you are worried about, report it through the Yellow Card scheme as well as telling your prescriber.19

Equally, if the starter dose feels like it is doing very little, resist the urge to speed up on your own. The minimum interval between steps exists for a reason, and your prescriber decides when you are ready to move on. A quiet first month is normal, and moving up too quickly mostly adds side effects, not speed.

How tirzepatide works

Understanding the mechanism explains why dose and time both matter. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. It is also why the medicine is built up gradually. The effect on appetite and stomach emptying grows as the dose rises, and the body adapts to each step.

Slower stomach emptying also explains many of the early side effects, such as nausea and feeling full quickly. As the body adjusts at each dose, these usually settle. It is also the reason for one practical caution: Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

Another consequence is how long the medicine stays in your system. After a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 It also means that the effect of a dose change builds and fades gradually, which is one reason the minimum step interval exists. You are not meant to judge a new dose after a single injection.

Putting it together: the starter dose introduces the mechanism gently, the middle doses build the effect on appetite, and your prescriber guides you to the maintenance dose that suits you. That explains why most measurable progress builds from 5mg onward rather than from the first pen.

Whichever route you take, the principle is the same: patience through the adjustment phase pays off, because the mechanism needs time and dose to do its work.

12 · Storage and travel

Storage and travelling

Good storage protects the medicine you are relying on. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them.13 Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, useful for short trips, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.13 In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
