In that trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are averages across a large group.
The trial also reports how many people crossed particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Again, these describe the end of a long treatment period.
It is important not to turn these numbers into a stopwatch. A trial average over 72 weeks tells you that weight loss is substantial for many people over more than a year of treatment. It does not tell you the loss in your first month, and it does not tell you that you will match the average. Trial averages do not predict your own result. Some people lose more, some lose less, and the spread around any average is wide.
The comparison group matters too. People in the trial who relied on lifestyle changes alone lost far less on average. That gap is the part of the result attributable to the medicine working alongside diet and activity. It is also a reminder that Mounjaro is not a standalone fix. The trial design paired tirzepatide with a reduced-calorie diet and increased physical activity, and that is how the medicine is licensed to be used.
If you are tempted to compare yourself with social media posts about the first fortnight, remember that those posts are selected, unverified and rarely include the people who saw little early change. SURMOUNT-1 does not measure how soon weight loss starts.

You may read online about specific signs that your body is burning fat, such as particular smells, sweating or changes in urine. A better guide is the combination of reduced appetite, a gradual downward trend on the scales over weeks, and how your clothes fit.
Loose skin and changes in how body fat feels are another common question. There is no established timeline for when fat feels softer or firmer, and it varies by person, by how much weight is lost and by how quickly. If you are concerned about changes in your body, a conversation with your prescriber or GP is the right route.
Finally, be alert to signs that something is wrong rather than right. Starting Mounjaro: NHS and private routes
How soon you start losing weight also depends on how soon you can actually start treatment, and that depends on access. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

10 · Expectations and safety

Realistic expectations are the best protection against disappointment in the early weeks. Mounjaro is an effective licensed treatment, but it works alongside a reduced-calorie diet and increased physical activity, and individual results vary.2 The trial figures are averages over 72 weeks of treatment, so the right mental picture is a gradual process across many weeks, not a rapid early drop.
Be wary of dramatic weight-loss claims, especially those tied to very short timescales. Sellers and social media posts often exaggerate speed to attract attention. A promise of a particular result by a particular date is a warning sign, not a reassurance. Genuine providers talk about averages, individual variation and the need for diet and activity.
Safety comes first. Mounjaro can interact with other treatments. For example, Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, so discuss contraception with your prescriber.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so tell your prescriber about every medicine you take.1 These points matter in the early weeks, when your body is adjusting.

Storage is part of safe use. Store unopened pens in the fridge at 2–8°C until first use, and they keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. 11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
