What Does a Mounjaro Pen Look Like After Two Doses? (October 2026)

After two doses, your Mounjaro KwikPen has two of its four weekly doses left, and it should still dial normally to your prescribed strength. Each pen holds four weekly doses13, so you are at the midpoint. The dose window, not the liquid level, is your guide. A pen that looks less full than expected is usually normal.

A quick overview

What to know
before you start.

  • After two doses a KwikPen has two weekly doses remaining, because each pen contains four weekly doses.
  • Once in use, keep the cap on and the pen not above 30°C for up to 30 days, then discard it.
In this guide

01 · The core answer

What your pen should look like after two doses

After two doses your pen should look like a pen that is half used: it still works exactly as it did on the first injection. Each KwikPen contains four weekly doses13, so two have gone and two remain. You turn the dose knob until the dose indicator shows your dose in the window, and you inject as you did before.

The main reassurance is built into the design. If it dials to your full dose, there is enough in it for that injection.

02 · Your next step

What to do now you are at the midpoint

Carry on exactly as before: same day each week, rotate the injection site, and keep the cap on between uses. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food1. Nothing about the second half of the pen differs from the first.

If you are on the starter strength, two doses in means you are still early in treatment. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Your prescriber guides that pace, so there is no need to change anything yourself at this point.

When you are ready to plan your next pen, our Mounjaro pen prices and consultation page shows every strength. Every later order is £25 off the standard price. If you want wider context on treatment, see our weight loss clinic overview.

03 · Checking the dose window

How to check your pen with the dose window

The dose window is the only reliable check. The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

That matters at the halfway point, because people often start to second-guess a pen once it looks different from a fresh one. A fresh pen and a half-used pen use the same dialling method, and the number you see is what you are about to inject.

Judging remaining liquid by eye is less dependable. If you are unsure, do not guess a quantity. Check the number in the window and read the patient information leaflet that came with your pen.

You can also keep a simple record. Marking each injection date on a calendar, or in a tracking tool such as our free GLPTrackr companion app, gives you a count of doses used that does not depend on how the pen looks.

04 · When the window will not turn

What if the dose window will not turn to a full dose?

That is the clearest sign a pen is finished.

After two doses you should not meet this problem, because two doses remain. If you do, something is unusual. Do not try to combine pens or top up from another pen to make up a dose. Contact the pharmacy that supplied it and tell them what the window shows and how many injections you have taken.

If a pen is faulty or damaged, tell us promptly. Our refund terms say dispensed prescription medicines cannot be returned except faulty, damaged or wrong items, which you must report within 3 days of delivery. Keep the pen and its box so the team can look into it.

If you want to see what a pen looks like when it is finished, our guide to what an empty Mounjaro pen looks like covers that stage.

05 · Why you should not stretch doses

Why you should not try to stretch the pen

Seeing a pen at the halfway point sometimes prompts people to wonder whether it could last longer. It should not be stretched.

Altering doses outside the licence means unknown dosing accuracy and no safety data.

If your worry is side effects, talk to your prescriber rather than adjusting the pen yourself.

06 · Missed or doubled doses

If you have missed or doubled a dose

If your dose count looks off, work out which injections you actually took before drawing conclusions. The rule for a late dose is simple. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1

If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

If you think you have already injected twice close together, do not inject again to "balance" it. Contact your prescriber or pharmacy and tell them when each injection happened. If you use more Mounjaro than you should, talk to a doctor immediately.

You can also change your weekly injection day if needed, as long as there are at least 3 days between two doses.1 That is the only safe way to shift your schedule.

07 · Storing a pen in use

Storing your pen between doses

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 Because a pen contains four weekly doses, a normal weekly schedule fits inside that window.

Keep the pen cap on to protect it from light, and never use a pen that has been frozen. If you travel, carry pens in hand luggage rather than the hold, which can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.

For unopened pens, keep them in the fridge until first use; they keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.

08 · The pen at other stages

Comparing your pen at other stages

The midpoint is one of several checkpoints, and reading the neighbouring stages helps you see what is normal. Our guide to what a pen looks like after one dose and the page on how a pen should look after the first dose cover the early injections.

Moving on, see what a pen should look like after three doses and what it looks like after four. If your question is about the moment of injecting, this guide to the pen straight after an injection is more relevant. For the needle itself, read about what a Mounjaro needle looks like.

Across all of these, the principle is the same: the dose window, the dose count and the leaflet are your references, and anything that does not match them is worth a call to the pharmacy.

09 · What to expect from treatment

What to expect from treatment at this stage

Two doses in is very early, and results at this point say little about the longer course. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.

Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

For context, in the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary. Think in months, not days, and judge progress by trend rather than single weigh-ins.

Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
What does a Mounjaro pen look like after two doses?
It looks like a pen that is half used and works exactly as before. Each KwikPen contains four weekly doses, so two remain. You still turn the knob until your dose shows in the window. Rely on that number rather than judging the liquid by eye, and check the leaflet if anything seems unusual.
What does a Mounjaro pen look like after four doses?
After four doses the pen is used up, and the sign is the dose window, not the appearance of the cartridge. If it will not turn to a full dose, there is not enough for an injection.
What happens if I take two doses of Mounjaro?
Do not take a second dose to compensate, and contact your prescriber or pharmacy, telling them when each injection happened. The rule for timing is that you should never take two doses within 3 days of each other. If you use more Mounjaro than you should, talk to a doctor immediately.
Should the pen still dial normally halfway through?
Yes. Halfway through, the pen should turn to your prescribed dose and show it in the window as it did at the start. If it will not, contact the pharmacy.
Can I use a Mounjaro pen for longer than a month once started?
Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. A pen holds four weekly doses, so normal weekly use fits that window. Keep the cap on and never use a pen that has been frozen.
What should I do if my pen looks different to the last one?
First check the number in the dose window and your count of injections taken. The solution inside a genuine pen is clear and colourless; do not use a pen that looks cloudy or discoloured, has particles in it, or is past its expiry. If the pen will not dial, is damaged or has been frozen, do not use it and contact the pharmacy that supplied it.
