03 · Rodent findings
Why the warning exists
The warning exists because of animal data. In rodent studies tirzepatide caused thyroid C-cell tumours.1 Regulators require a medicine's label to carry findings like this, even when it is unknown whether they apply to people.
The same product information states that no increase has been shown in human trials. Both statements are true together: an animal signal and no human signal so far. The patient information leaflet repeats the precaution, so it is worth reading before your first dose.13
04 · Who should tell us
Who should tell their prescriber first
Two histories need to be mentioned: medullary thyroid cancer in you or a close relative, and the inherited condition MEN 2.1 Your prescriber will weigh this in your assessment.
Tell them about it during the consultation, together with your full medical history and medicine list. Other conditions are also reviewed before treatment. For example, a history of pancreatitis or gallbladder problems calls for extra care. You can read about pancreatitis and Mounjaro separately.
If you have a family history but are unsure what type of thyroid cancer a relative had, ask your GP or the relative's clinic if you can. Giving the prescriber accurate detail helps them judge suitability.
05 · Monitoring and symptoms
Monitoring, new symptoms and reporting
Routine thyroid monitoring is not required for most people on Mounjaro.1 That means no scheduled thyroid scans or blood tests simply because you are taking it.
If you notice anything new or lasting, tell your prescriber. For severe, persistent stomach pain, swelling of the face or throat or difficulty breathing, get medical help straight away. You can also report a suspected side effect through the MHRA Yellow Card scheme.19

06 · Putting risk in context
Putting the risk in context
A balanced decision weighs benefit as well as risk. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2
In trials, weight loss also came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures. These are trial-average outcomes, not promises.
None of this cancels out a precaution that applies to you. It is the reason the decision belongs with a prescriber who sees your whole history. After a final dose, tirzepatide takes roughly 4–5 weeks to clear the system.1
07 · Beyond thyroid
Other safety points worth knowing
Thyroid is only one part of the safety picture. Mounjaro is not suitable for everyone: you should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
