In rodent studies, tirzepatide caused thyroid C-cell tumours.1 This single finding is the origin of almost every headline linking Mounjaro to cancer.

The second half of that finding matters just as much: it is not known whether this applies to humans.1 Animal findings are a signal to investigate, not a verdict. A result in rodents tells researchers where to look; it does not tell you what will happen in a person.

It helps to be precise about what the finding is. It concerns thyroid C-cell tumours, not cancer in general. If you read a claim that Mounjaro causes cancers throughout the body, check whether it names a source.

Why mention it at all if the human evidence has shown no increase? Because honest labelling means telling you everything known, including what remains uncertain.

For you, the practical translation is simple. The finding does not mean you will get cancer, and it does not mean you are safe regardless of history. It means your prescriber needs to know about certain conditions in you and your close relatives. It is also worth separating two ideas that often get merged online: a theoretical risk and an observed one. A theoretical risk comes from laboratory or animal work. An observed risk comes from people actually developing a condition at higher rates than comparable people not taking the medicine. For tirzepatide, the first has been reported and the second has not been shown in human trials.1

What human trials show, and what they cannot

No increase in thyroid C-cell tumours has been shown in human trials of tirzepatide.1

It is also worth understanding the limits of any trial. SURMOUNT-1 enrolled 2,539 adults and ran for 72 weeks.2

Why does this matter for the question you asked? Because "no increase has been shown" and "proved impossible" are different statements. The honest position is that the human evidence so far is reassuring on the thyroid, while the animal signal is the reason the precautions exist.

Where does that leave you in practice? Think about it in three parts:

  • The rodent finding exists and is disclosed in the product information.
  • Human trials have not shown an increase, and it is not known whether the rodent finding applies to people.
  • Your own history, especially any medullary thyroid cancer or MEN 2 in the family, is what a prescriber uses to personalise the decision.

05 · MTC, MEN 2 and your history

Medullary thyroid cancer, MEN 2 and your family history

If you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2, tell your prescriber before starting Mounjaro.1 They will weigh this in your assessment.

Why these two conditions specifically? MEN 2 is an inherited condition. Flag them as a precaution because of the animal finding, not because human trials showed harm.13

Note the wording: tell your prescriber, who will weigh it. That is how our consultation works: a GPhC-registered prescriber reviews your answers, not a scoring script.

What counts as a close relative? Think parents, brothers, sisters and children. If you are unsure of your family's history, say so in the consultation instead of guessing. A truthful "I don't know" is far more useful to a prescriber than a confident wrong answer, and they can advise on next steps.

Remember that Mounjaro has other reasons it may not suit you. It should not be used if you are allergic to tirzepatide or its ingredients, or during pregnancy or while trying to conceive. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, with a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems.1 Always share your full medical history and medicine list.

Eligibility itself is straightforward on paper: a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 Meeting the BMI threshold is necessary but not sufficient; suitability is the prescriber's call.

06 · Symptoms, reporting and who to ask

New symptoms: what to do and who to tell

If you notice a new or worrying symptom on Mounjaro, tell your prescriber and consider reporting it through the Yellow Card scheme.19 That advice holds whether the symptom seems related to the medicine or not. Early conversations are better than late ones.

New symptoms can have many causes and deserve a proper assessment, so your prescriber should still hear about them.

  • Severe, persistent stomach pain that may spread to your back, with or without vomiting, which may be pancreatitis: stop using Mounjaro and get medical help straight away.
  • Pain in the upper right abdomen, fever, or yellowing of the skin or eyes, which may be a gallbladder problem: get medical help straight away.
  • Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing: get medical help straight away.

07 · Heart, stroke and long-term safety

Heart, stroke, kidneys and other safety questions

If you have heart or circulation concerns, tell your prescriber. If you have cardiovascular disease, tell your prescriber, because it is relevant to your assessment and is also one of the weight-related conditions that can make someone eligible.1

What the trials do report is positive on average. Weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they explain why prescribers talk about weight management as health management.

On kidneys and liver, Mounjaro is not known to damage them directly, and no dose adjustment is needed for mild to moderate kidney or liver impairment.1 The practical risk is indirect: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys. Keep fluids up and get medical help straight away if you cannot keep fluids down. People with severe impairment need individual prescriber assessment.

On mood, the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and suicidal thoughts, and the MHRA keeps this under continuous review.17 Even so, big changes in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

08 · Mounjaro vs Ozempic safety

Is Mounjaro or Ozempic safer?

Mounjaro is the brand name for tirzepatide, a once-weekly injection made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes.1 It activates two natural gut-hormone receptors at once, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. This dual action is what distinguishes it from single-action GLP-1 medicines such as semaglutide, the medicine in Wegovy and Ozempic. Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg), and a regulated UK pharmacy will not supply Ozempic for weight loss.

If you are considering either medicine and have a personal or family history of medullary thyroid cancer or MEN 2, discuss that explicitly with your prescriber.

Results from different trials, for example SURMOUNT-1 for tirzepatide and STEP 1 for semaglutide, should not be set side by side as if they were head to head.2,9

Practical differences you can verify: Mounjaro comes in six strengths from 2.5mg to 15mg. Wegovy pens run from 0.25mg through 0.5mg, 1mg and 1.7mg to the 2.4mg maintenance dose, reached from week 17.18 Both are once weekly, and both are used alongside a reduced-calorie diet and more physical activity.

09 · Benefits weighed against risk

Weighing the benefits against the risk

A sensible decision weighs the known benefits against the uncertain risk, using your own health picture. For many adults with obesity or weight-related conditions, the benefits documented in trials are substantial, while the cancer concern is based on an animal finding that has not been shown to apply to humans.1

In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary. For someone starting at 100 kg, 20.9% is about 21 kg.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.2 Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Beyond weight, trial participants saw average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most with prediabetes returned to normal blood-sugar levels.2 That is why a prescriber considers the whole health picture, not appearance alone.

Who tends to weigh this most carefully? Anyone with a family history that touches the thyroid, and anyone who would not be comfortable with any uncertainty. Both are valid positions.

10 · NHS and private access

NHS and private access, and why the safety check is the same

Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 or above with four or more qualifying conditions.6 Full rollout is expected to take years.

That timeline is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
