01 · The core answer
Can you get weight loss medication at a BMI of 25?
Usually not through a licensed route, because the licence starts higher. Mounjaro is licensed for adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 A BMI of 25 falls under both of those lines.
That is not a technicality. The licence reflects the people studied in trials and the group for whom the benefit is judged to outweigh the risks. A prescriber assesses you against the licence.
The one genuine exception is ethnic background. For some groups the thresholds are lower. Your prescriber applies that adjustment during the assessment, not you.
02 · What to decide
The two decisions you are actually facing
The first decision is whether your BMI is the right measure for you. BMI divides weight by height squared, so it ignores muscle, build and where fat sits. If you are muscular, a BMI of 25 may overstate your risk. If you carry weight around the middle or have a family history of heart disease or diabetes, it may understate it.
The second decision is what to do about it. Better first steps are a calm look at your diet, activity and sleep, and a conversation with your GP if a condition such as blood pressure or blood sugar is involved.

04 · Ethnic background
Does ethnic background change the threshold?
Yes, it can. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight- related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups.1
This is the main reason a person with a BMI near this level might still be assessed rather than turned away at once. It does not mean automatic approval. The prescriber still looks at weight-related conditions, medical history and current medicines before deciding.

05 · What a prescriber checks
What a prescriber checks before saying yes
A prescriber assesses your BMI, medical history and current medicines before deciding whether Mounjaro is suitable.1 It is also used alongside a reduced-calorie diet and increased physical activity, not instead of them.
Beyond BMI, they will look for the conditions that bring the licence into reach: high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. A recent blood pressure reading or blood test result is often what moves a borderline case. The point is that a number alone does not decide it. A clinician reading your whole history does, and that is why a short consultation is more useful than a calculator.
06 · Safety and exclusions
Safety matters more when the benefit is smaller
Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.1 If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. It should be stopped at least one month before trying to conceive.
Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.13 In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials. Tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2.1
Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose. Mention warfarin or other narrow-margin medicines, insulin, or sulfonylureas, where doses may need reducing to avoid low blood sugar.1 Mounjaro can reduce how well oral contraceptive pills are absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.
Be wary of anyone offering to supply a prescription medicine without a consultation or prescription. Report suspected counterfeits and side effects through the Yellow Card scheme.7 Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
07 · Ozempic and Wegovy
What about Ozempic and Wegovy at this BMI?
They are single-action medicines containing semaglutide, whereas Mounjaro activates two gut-hormone receptors, GIP and GLP-1.1 Neither is a way around the BMI question. Each has its own licence set out in its product information, and a prescriber assesses you against it.
Wegovy's pens run through a dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17.18 NHS access to Wegovy runs through specialist weight-management services; ask your GP.
Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label, so a regulated UK pharmacy will not supply it for weight loss. 08 · NHS route
What the NHS offers
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions.46 Full rollout is expected to take years.
At a lower BMI, the NHS route to medication is not open. What your GP can do is check blood pressure, cholesterol and blood sugar, refer you to local weight-management support, and tell you whether a condition changes your position.
09 · Better first steps
What actually helps at this BMI
Small, steady changes outperform dramatic ones. Aim for regular meals built around protein, vegetables and fibre-rich foods, fewer liquid calories, and a realistic increase in daily movement. No single food lowers your BMI; it falls only when your overall intake sits below what you burn over time.
Strength training helps you keep muscle while you lose fat, which matters because BMI cannot tell the two apart. Sleep and stress matter too: poor sleep often drives appetite and snacking. Track what you eat for a couple of weeks, because most people are surprised by where their calories come from.

10 · Is losing weight worthwhile
Is it worth losing weight at a BMI of 25?
It can be, depending on why. If you are healthy, active and comfortable, you may have no medical reason to lose weight at all. If you have raised blood pressure, high cholesterol, rising blood sugar or a strong family history, modest weight loss can help those markers, and your GP can say how much matters in your case.
Wanting to change how you look is a personal choice, but prescription injections are not a cosmetic tool. If you find yourself preoccupied with weight despite being near the middle of the range, speak to your GP, as that is worth taking seriously in its own right.
11 · If you later qualify

If your BMI rises or a weight-related condition is diagnosed, the picture changes. Mounjaro comes as a once-weekly pre-filled KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Most people start at 2.5mg and step up under prescriber guidance.
