No licensed weight loss injection in the UK is sold without a prescription. Mounjaro, Wegovy and liraglutide products (Saxenda and others) are all prescription medicines. Mounjaro in particular is supplied only after an online consultation reviewed by a GPhC-registered prescriber.1 Anything advertised as a "non prescription" version is either not the real medicine, not licensed, or being supplied outside the regulated route.
That sounds like a barrier, but the legitimate route is designed to be accessible. You complete a short health questionnaire online, a prescriber reviews it, and if the treatment is suitable it is dispensed by a registered pharmacy and delivered to your door. There is no GP appointment to wait for and no charge for the consultation. 04 · Why none are legal
Why there is no legal over-the-counter weight loss injection
No weight loss injection is licensed for sale without a prescription in the UK, because these are potent prescription medicines that need a clinical decision before use. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
The phrase "non prescription weight loss injection" therefore describes something that does not exist as a licensed product. When a listing uses it, one of three things is usually happening. The seller may be offering a product that is not the real medicine. They may be offering a genuine-looking product through a route with no prescriber involved. Or the listing may be loosely worded marketing for something that is not an injection at all, such as a supplement.
Other licensed injections follow the same rule. Wegovy (semaglutide) is a once-weekly pen with a dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least 4 weeks and reaching the 2.4mg maintenance dose from week 17. Liraglutide (Saxenda, and products such as Erly) is supplied at 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. Each of these is a prescription medicine, and each needs a prescriber to decide it is right for you.
It helps to understand the reasoning rather than treat it as red tape. Mounjaro must be used alongside a reduced-calorie diet and increased physical activity, and a prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.1 That assessment is the safety step. It is the difference between a medicine matched to you and a product taken without anyone checking whether it fits your health.

05 · Risks of unregulated sellers
The risks of buying from unregulated sellers
Buying a weight loss injection from an unregulated seller means you cannot know what is in the pen. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme precisely because falsified medicines reach patients through informal channels.7 Social media accounts, messaging apps and anyone offering to supply without a consultation or prescription fall outside that protection.
Several specific problems follow from an unverified supply:
- Unknown contents. A pen that looks right may contain a different amount of medicine, a different substance, or nothing useful.
- Unknown storage history. Mounjaro pens belong in a fridge at 2–8°C and must never be frozen.1 A product that has travelled through informal hands has no traceable cold chain.
- No clinical check. Nobody has reviewed your medicines, your medical history or whether you are pregnant or planning a pregnancy. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.
- No aftercare. If something goes wrong, there is no prescriber to call and no pharmacy responsible for the product.
You can check a pharmacy yourself. The General Pharmaceutical Council keeps a public register of pharmacies and pharmacy professionals, and a genuine UK online pharmacy will show its registration number. Signs a listing is not a regulated supply include: no consultation or health questions, a promise of specific results, pressure to pay quickly by bank transfer, no named pharmacy, and prices far below what the licensed product could plausibly cost. 06 · Over-the-counter alternatives
Over-the-counter products: what they are and are not
Over-the-counter products are not injections and are not equivalent to a prescription GLP-1 medicine. People often ask, "Is there an over-the-counter drug like Ozempic?" and "How can I mimic Ozempic naturally?" The honest answer is that nothing sold without a prescription replicates how these medicines work.
The best-known marketing term is "nature's Ozempic", applied to certain supplements. These are low-cost supplements with no clinical oversight. Dosing is not standardised across supplement products, which is itself a reason for caution. Dramatic weight-loss claims on supplement packaging should be read sceptically.
There are also fibre products, appetite-focused supplements and other items on pharmacy and supermarket shelves. A pharmacist in person is the right person to ask about what is appropriate for you, especially if you take other medicines. A sensible way to think about it: if a product is cheap and freely available, it has not been through the regulatory testing that sits behind a licensed medicine. That does not always make it harmful, but it does mean you are relying on marketing rather than evidence. 07 · How Mounjaro works
How Mounjaro works
Mounjaro contains tirzepatide, which activates two natural gut-hormone receptors at once: GIP and GLP-1. It is the first treatment of its kind to do so. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1
In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That is the central point: the medicine is a tool that makes eating less easier, not a replacement for changing how you eat and move. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1
This is also why an over-the-counter product cannot be a stand-in. Mounjaro's effect comes from a specific active ingredient acting on specific receptors, delivered at a precise, titrated dose in a purpose-built pen.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 It is one more reason a prescriber should be involved: decisions about starting, adjusting or stopping benefit from clinical guidance.
The injection itself is straightforward. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. You rotate the site with each dose, and each KwikPen contains four weekly doses.1

08 · Who is eligible
Who can get Mounjaro: licensed eligibility and the NHS picture
Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1

On the NHS, Mounjaro is available, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
NHS access to Wegovy runs through specialist weight-management services; ask your GP. If you are weighing NHS against private, the honest summary is that the NHS route is free at the point of use for those who qualify under its criteria, while the private route has a wider licensed eligibility range but is paid for. Neither involves buying an injection without a prescription.
For NHS prescriptions in England the standard prescription charge is £9.90 per item where it applies, but whether you are offered Mounjaro on the NHS depends on the eligibility rules above, not on the charge.
If you do not meet the criteria, that is useful information too. A prescriber can discuss other approaches, and a pharmacist can advise on safe over-the-counter options. What you should avoid is trying to bypass the criteria by buying from an unregulated source, because the criteria exist to identify who benefits and who might be harmed.
If you are on other medicines, tell the prescriber. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. Doses of insulin or sulfonylureas may need adjusting because of low blood sugar risk.7 These are exactly the details a consultation is there to catch.
09 · What results look like
What results look like — and how fast
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Trial averages do not predict your own result.
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days.1
Early experience differs widely. Some people notice appetite change and early loss in week one; others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13
Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That built-in review is another feature a non-prescription route cannot offer: someone is watching whether the medicine is actually working for you and whether it remains appropriate.
Beware of unregulated listings that promise specific results or specific timescales. Dramatic weight-loss claims have no place alongside a medicine whose own trial results are expressed as averages with wide individual variation. A realistic expectation, set at the start, makes it far more likely you will stay with treatment long enough to judge it fairly.

10 · Dosing and injecting
Dose ladder, missed doses and storage
Everyone starts Mounjaro on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg; higher is not automatically better, and your prescriber will guide the right pace.1
This is another reason the prescription step matters. Titration is personal, and a self-directed approach to dose can cause problems. Altering doses outside the licence means unknown dosing accuracy and no safety data.1
Missed dose. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1
Changing your day. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose. There is no best time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off.1
Storage. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the cap on to protect it from light, and never use a pen that has been frozen.1
Travel. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements; for hot climates use an insulated pouch. Unopened pens keep in the fridge until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.1
None of this is complicated, but it all assumes a genuine, traceable pen with proper labelling and guidance, which is what a regulated pharmacy supplies.
11 · Planning the cost

Planning the cost of Mounjaro means thinking in pens rather than single purchases. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. The commonest early side effects of Mounjaro are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.13 Starting at 2.5mg and stepping up no sooner than after at least 4 weeks is designed to give your body time to adjust.
Because Mounjaro is a prescription medicine, any new or worrying symptom should be raised with your prescriber. 13 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Step 3: Next-day tracked delivery
Dispensed by our GPhC-registered pharmacy in temperature-controlled packaging with needles and a sharps bin included. Orders approved by 3pm for next-day tracked delivery.
Prescription and dispatch follow clinical approval. Confirm current delivery options when ordering.
