Can you get weight loss injections without a prescription?
No: a genuine weight loss injection such as Mounjaro cannot legally be supplied without a prescription. It is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber, and it is used alongside a reduced-calorie diet and increased physical activity.1 What many people really mean is 'without a GP appointment' or 'without an NHS waiting list'. That is achievable through a regulated private pharmacy.
The distinction matters. A non-prescribed route means nobody has checked your medical history, your current medicines or whether the product is suitable. 04 · The legal position
Why there is no legal non-prescribed weight loss injection
The weight loss injections that work in clinical trials are prescription-only medicines. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose pen (KwikPen) in six strengths from 2.5mg to 15mg.1 It is supplied only after an online consultation reviewed by a GPhC-registered prescriber.
That requirement is not red tape. A prescriber assesses your BMI, medical history and current medicines before deciding whether the medicine is suitable for you.1 Without that step, nobody has checked whether the product is right for you, and nobody is responsible for follow-up if something goes wrong.
So when a website, social media account or private message advertises weight loss injections with no consultation or prescription, treat it as a warning sign. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.7 Sellers offering to supply without a consultation or prescription are, by definition, outside the regulated system that protects you.
What 'non-prescribed' usually means in practice
Some want an over-the-counter injection from a high-street shop. None exists for medicines such as Mounjaro. Some want to skip their GP, which is a different question: a regulated online pharmacy can run the consultation and prescribing for you. And some mean 'cheap and fast, no questions asked', which is exactly the profile of the unregulated market.

What the law protects you from
Prescription-only status exists so that a trained professional checks suitability first, and so there is a clear record of who prescribed, who dispensed and what you were given. When you buy outside that chain, you lose all three. If a pen turns out to be fake, contaminated or the wrong strength, there is nobody accountable and nowhere to turn. That risk is the main reason we say plainly that the regulated route is the only sensible one.
A regulated pharmacy can also be checked. The General Pharmaceutical Council keeps a public register of pharmacies and pharmacy professionals, and ours is registration 9012878.12 If a seller cannot point to a register entry you can verify yourself, walk away.
05 · Who is eligible
Who can be prescribed a weight loss injection
Eligibility is based on BMI and health, not on whether you want to avoid a doctor. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1
This is why a prescriber has to look at your answers. Two people with the same weight may have very different suitability once medical history and current medicines are considered. A prescriber assesses your BMI, your medical history and your current medicines before deciding.1 That judgement is what you pay a regulated service for, and it is the part that disappears when you buy from a non-prescribing seller.
What if I do not meet the criteria?
If your BMI is below the licensed thresholds, a prescriber can decline the order. Being turned down is a safety feature, not a failure. Ask about lifestyle support instead, and revisit the question if your circumstances change.
Do I need to see my GP first?
You can start with a free online consultation without booking a GP appointment first. The questionnaire asks about your height, weight, health conditions and medicines, and the prescriber reviews it. Be honest in your answers: they exist to keep you safe.
If you already have a GP who knows your history, sharing your plans with them is sensible, particularly if you take insulin or other diabetes medicines, where doses may need adjusting because of the risk of low blood sugar. The licence is explicit that treatment sits alongside a reduced-calorie diet and increased physical activity.1 No injection, prescribed or not, replaces that. Anyone promising that a jab alone will transform your weight without any change in eating or activity is selling something the evidence does not support. 06 · How Mounjaro works
How a genuine weight loss injection works
Mounjaro works by activating two natural gut-hormone receptors at once. Tirzepatide is the first treatment of its kind to activate both GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 Both classes are prescription-only. Neither can be replaced by an unregulated product that merely borrows the name or the marketing language.
How long it stays in your system
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That is worth knowing before you start: this is a pharmacologically active medicine with real effects, which is exactly why a prescriber should be involved.
Why a prescriber matters for a medicine like this
A medicine that changes appetite and slows stomach emptying has side effects and interactions. The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.1 Serious side effects are uncommon but need urgent attention: seek immediate medical help for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen, fever or yellowing of skin or eyes, or signs of a severe allergic reaction. A prescriber can advise on your current medicines and on how to proceed if something does not feel right. Report any suspected side effect through the MHRA Yellow Card scheme.19

Not every 'GLP-1' product is the same
You will see products described as 'GLP-1 support', 'natural GLP-1' or 'Ozempic alternatives'. Naming a mechanism in a product description does not make a product equivalent to a licensed medicine. Licensed medicines have been assessed for quality, safety and effectiveness for a defined use and dose. A product that has not been assessed that way cannot be treated as a substitute, however similar the marketing sounds.

07 · What results look like
What results to expect, and how fast
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg.
'How can I lose two stones in 12 weeks?'
Weight loss on Mounjaro is gradual by design, so no timescale like that can be promised. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1 The trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.
Anything advertised as rapid or dramatic weight loss should make you more cautious, not less. Fast-result claims are a hallmark of unregulated sellers.
The first weeks
Experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1
When a prescriber reviews progress
Your response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That review loop is a core safety feature that an unprescribed product cannot offer.
To set expectations realistically, remember that trial averages do not predict your own result. Some people lose more, some less, and the pace depends on dose, tolerance, diet and activity.
08 · Supplements and 'natural Ozempic'
'Poor man's Ozempic', berberine and other over-the-counter options
Over-the-counter products are not equivalent to prescription weight loss injections, and there is no over-the-counter weight loss injection in the UK equivalent to Mounjaro. You will see supplements nicknamed 'poor man's Ozempic' or 'nature's Ozempic', usually berberine-based. They are low-cost supplements with no clinical oversight.
A prescription medicine such as Mounjaro is licensed for a defined use, comes in set strengths in a pen that delivers a defined dose, and is supplied after a prescriber has checked it is suitable for you.1 A supplement has none of those features. Dosing is not standardised, and nobody reviews whether it suits your health or your other medicines.
What can I buy for quick weight loss?
The honest answer is that nothing sold for quick weight loss is a safe shortcut. Dramatic weight-loss claims on supplement packaging or social media should be treated with scepticism. Because supplements are available cheaply without assessment, they attract exactly the marketing hype that regulated medicines are not allowed to use.
If you are heavier and have health risks, the sensible route is a conversation with a prescriber about licensed options. If you are not eligible for a prescription, ask a pharmacist or GP about structured diet and activity support instead.
How can I mimic Ozempic naturally?
You cannot replicate a prescription medicine with food or supplements. What you can do is build the habits the licence itself requires alongside treatment: a reduced-calorie diet and increased physical activity.1 These work with or without medication, and they matter even more if you do start treatment.

09 · Fakes and scams
How to avoid counterfeit and unregulated weight loss injections
The biggest risk of looking for non-prescribed injections is counterfeit or unregulated product. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme, where you can report suspected fake medicines and side effects.7 Weight loss injections are a high-demand product, which makes them a target for sellers who cut corners.
Red flags to watch for
- The seller offers to supply without a consultation or prescription.
- Dramatic weight-loss claims, or promises of specific results.
- No verifiable pharmacy registration you can look up yourself.
- Sales through social media messages, marketplaces or unsolicited direct messages.
- Prices that look far below what regulated pharmacies charge.
- Pressure to pay quickly or by unusual methods.
How to check a seller is real
Look up the pharmacy on the General Pharmaceutical Council register.12 Our GPhC registration number is 9012878, and you can verify it independently on the regulator's site at pharmacyregulation.org. A genuine online pharmacy will always show its registration and will always ask you to complete a consultation first.

Why storage and handling matter

10 · Dosing, injecting, storage
How a prescribed injection is used: dosing, technique and storage
A properly prescribed pen comes with a clear dosing plan, which is another thing unprescribed products lack. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at minimum 4-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better.
Injecting
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1 There is no best time of day, so pick a day and time you will remember.

Missed doses and changing day
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other. You can change your injection day as long as at least 3 days (72 hours) have passed since your last dose.1
Storage and travel
Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the cap on to protect it from light.1 You can fly with Mounjaro: carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription. If an unopened pen has been out of the fridge, check with the pharmacy before using it.
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
12 · NHS versus private
NHS or private: the honest picture
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Why private is not 'non-prescribed'
