01 · The core answer

Is face change a side effect of weight loss injections?

"Ozempic face" is not a drug effect. It reflects what losing a significant amount of weight quickly can do to facial volume, and the same applies to loose skin.1 Our guides on Mounjaro weight loss face and weight loss drugs and the face cover the same ground from other angles.

In the SURMOUNT-1 trial of 2,539 adults, people on tirzepatide with diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, versus 3.1% with lifestyle changes alone. These are trial averages and individual results vary. For someone starting at 100 kg, 20.9% is about 21 kg. A change of that size can alter how the face looks, whatever the method.

03 · Dose and pace

How dose and pace relate to results

Everyone starts on 2.5mg once a week. This dose is about letting your body adjust, not weight loss itself. After at least 4 weeks it rises to 5mg, then steps up by 2.5mg at minimum 4-week intervals to a maximum of 15mg, depending on response and tolerance.1

Hair shedding and other visible effects

Temporary hair shedding is a recognised side effect, linked to rapid weight loss itself rather than a direct drug effect. It was reported by 4.9% of people on tirzepatide in the weight-management trials, versus 1.0% on placebo. For most people it settles as weight stabilises, and balanced nutrition with enough protein matters.1

Injection-site reactions were reported by 8.0% of people on tirzepatide versus 1.8% on placebo. Mild redness, itching or a small bruise normally settles within a few days.13

When to seek help

Swelling of the face or throat with difficulty breathing can be a sign of a severe allergic reaction: get medical help straight away. This is different from gradual loss of facial volume.1

Get medical help straight away for severe, persistent stomach pain that may spread to your back, or upper right abdominal pain with fever or yellowing of skin or eyes.1 Report suspected side effects via the Yellow Card scheme.19

06 · Stopping treatment

What happens when you stop

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. Nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

07 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Does Ozempic face go away?

What are the possible side effects of taking Mounjaro for 3 months?
Most are digestive: nausea, diarrhoea, vomiting, constipation and abdominal discomfort affect more than 1 in 10 people. They are most likely when starting or increasing a dose and usually ease within days to a few weeks. Hair shedding and injection-site reactions are also recognised.
Is it safe to stop weight loss injections suddenly?
Tirzepatide takes roughly 4–5 weeks to clear the system after a final dose, and nothing needs doing while it clears. Speak to your prescriber before stopping so you can plan ahead. Your clinical team can advise on diet and activity afterwards.
Can you drink alcohol on a weight loss injection?
There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. Moderation is the sensible rule.
Can I prevent facial changes while losing weight?
You can reduce the risk but not remove it. Eat enough protein, do resistance exercise and let your prescriber guide the pace of loss rather than aiming for maximum speed. Results vary between individuals, and skin commonly adapts gradually once your weight stabilises.
Who can get Mounjaro for weight loss?
It is licensed for adults with a BMI of 30 or above, or 27 to 30 with a weight-related condition, alongside a reduced-calorie diet and activity. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.
