How weight loss injections work in the body

Weight loss injections work by copying natural gut hormones that signal fullness and slow digestion. Tirzepatide, the active ingredient in Mounjaro, activates the GIP and GLP-1 receptors at once. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That is the central idea: the medicine changes appetite signals, and you still do the work of eating differently and moving more. It does not burn fat directly, and it does not act as a stimulant.1

The dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic). Wegovy is the weight management brand of semaglutide, and its licensed maintenance dose is 2.4mg once weekly.1 Liraglutide products belong to the GLP-1 family too, but they are given daily rather than weekly.

Why weekly versus daily matters

A once-weekly pen means a single injection day to remember, whereas a daily product means building the injection into every day. Some people value the simplicity of weekly dosing, particularly on busy or travelling weeks. Others prefer the rhythm of a daily routine. Why there is no instant effect

These medicines do not work like a painkiller. The body adjusts to them over weeks, which is why every course begins at a low dose and builds. Tirzepatide stays in the system for a while after the final dose, taking roughly four to five weeks to clear, and there is no way to speed this up. Nothing needs doing while it clears.1

What the trials tell us, and what they do not

The SURMOUNT-1 trial measured tirzepatide results over 72 weeks, and the STEP 1 trial studied semaglutide in a similar population.2 Those were separate studies with different designs, so you should not read their headline numbers against each other as if one beat the other. Trial averages also do not predict your own result. Your response depends on dose, tolerance, diet, activity and the rest of your health picture. SURMOUNT-5 was the first large head-to-head trial of tirzepatide and semaglutide, and a prescriber is the best person to help you weigh the options in your own case.

05 · Mounjaro in detail

Mounjaro: the once-weekly tirzepatide pen

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

The dose ladder

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose rises to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1

Type 2 diabetes context

Mounjaro is also licensed for type 2 diabetes, where it improves blood sugar control (HbA1c) and supports weight loss. Wegovy versus Mounjaro

The honest answer to "which is better, Wegovy or Mounjaro?" is that it depends on the person. 07 · Non-injectable options

Non-injectable weight loss medication

Non-injectable options do exist: licensed GLP-1 tablets such as Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) are available as film-coated or oral tablets.2021

Why people ask about tablets

The most common reason is simply needle reluctance. Pen needles are very fine and many people find the weekly injection easier than expected, and every order includes needles, a sharps bin and injection guidance. If you still prefer tablets, tell the prescriber, who can discuss what is licensed and suitable for you.

The "poor man's Ozempic" question

"Poor man's Ozempic" and "nature's Ozempic" are marketing phrases for low-cost supplements, with berberine the best-known example. They are not licensed medicines, and they do not carry the same oversight or evidence base as prescription treatments. Dosing is not standardised and quality varies between sellers. Be wary of dramatic weight-loss claims attached to them. Anything sold as a shortcut that skips assessment deserves scepticism, and the MHRA publishes counterfeit medicine alerts for a reason.7

Over-the-counter and other products

Fibre products, over-the-counter capsules and similar items are available cheaply without assessment. Realistic pace of weight loss

Weight loss on injectables is gradual by design, so it is not realistic to expect a large change within a few weeks. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1

Can you lose a lot of weight in a short time?

People often ask whether they can reach a specific target in a couple of months. Promising a specific number would be misleading. The licensed dose ladder is built around slow, tolerable increases, and altering it to speed things up is not supported. Early loss on any plan includes water weight, so the trend over weeks matters more than any single weigh-in.1

What the starting weeks feel like

09 · Safety and what to avoid

Safety, interactions and what to avoid

The main things to avoid on Mounjaro are altering the dose, doubling up after a missed dose, freezing the pen, and keeping your prescriber in the dark about other medicines. Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose. Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin, or sulfonylureas (where doses may need reducing to avoid low blood sugar). No adjustment is needed for paracetamol.1

Contraception

The medicine can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Discuss your contraception with the prescriber before you start, and read the MHRA guidance.7

Missed doses

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

Questions about lasting harm

People ask whether these medicines can cause permanent damage. Mounjaro is not known to damage kidneys or liver directly; the practical risk is indirect, because severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys, so keep fluids up and get medical help straight away if you cannot keep fluids down. Your prescriber can talk through the risks that apply to you. Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.

Reporting problems

If you have a new symptom, speak to your prescriber, and report suspected side effects through the MHRA Yellow Card scheme.19 Reports help regulators spot issues early, and you do not need to be certain the medicine caused the problem to make one.

10 · NHS versus private

NHS access versus private treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46

Asking your GP

Start by asking your GP whether you qualify under the current phase and how local services are organised. The NHS describes the medicine on its tirzepatide page.5 NHS access to Wegovy runs through specialist weight-management services; ask your GP. A prescription from the NHS carries the standard England charge of £9.90 per item unless you are exempt, though that does not apply in every UK nation, so check locally.

When private treatment makes sense

Private treatment suits people who do not yet fall into an NHS group, who prefer not to wait, or who want faster follow-up. It still needs a proper clinical assessment. Injecting, storing and travelling with your pen

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1

Choosing a day and time

There is no "best" time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off, and then stick to it.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Storage

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Travelling

You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.13

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 Severe, persistent stomach pain, with or without being sick, could be pancreatitis: stop using Mounjaro and get medical help straight away.
