Who is eligible: the criteria that actually matter

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1 A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.11

Sex, age bracket, job and postcode are not part of that test. Two people with the same BMI and health profile are assessed on the same clinical basis, whatever group a survey would place them in. That is why demographic trends tell you little about whether a jab is right for you personally.

Cost is one of the biggest practical filters on who uses private weight loss jabs. The Mounjaro pen has a standard price of £169.99 per month at the 2.5mg starting dose, and £218.99 at 5mg. 05 · Why the numbers differ

Why demographic estimates disagree

Estimates of UK weight loss jab use disagree because each study counts a different population in a different way. A national survey asks a sample of adults whether they have ever used or would consider using a jab. A pharmacy reports its own customers. The NHS counts prescriptions written in primary care or specialist services. None of these is the whole picture, and combining them without care double counts some people and misses others entirely.

Survey wording matters too. "Have you used a weight loss injection?" captures people on licensed medicines, but may also capture people who bought unregulated products or used someone else's pen. "Would you consider using one?" measures interest, not use. A headline that blends interest and actual use can overstate how many people are currently injecting.

Timing is another trap. This market has moved quickly. The NHS primary-care rollout of Mounjaro began in June 2025, and Eli Lilly's UK list-price change took effect on 1 September 2025.10 A figure gathered before those events may not describe the market today. When you see a statistic, check three things: who was asked, what exactly was counted, and when.

Private pharmacy data carries its own bias. A provider's customers are people who chose that provider, and those who pay privately are by definition people who either do not qualify for NHS treatment yet or chose not to wait. So private figures describe a self-selected group, not the UK population. Likewise, NHS figures describe the people the current rollout prioritises, who are the highest-need groups first.

The honest conclusion is that you can say who is licensed and who is eligible with confidence, but claims about exactly who is using jabs, down to a decimal point, deserve caution. 06 · Age and the licence

Age: what the licence says and what it doesn't

Mounjaro is licensed for adults.1 Within adulthood, the licence does not set different rules for people in their thirties, forties or sixties. Eligibility turns on BMI and weight-related conditions, assessed by a prescriber who reviews your history and current medicines.11

Surveys often split respondents into age bands, and it is easy to read a pattern as a rule. A band showing more interest does not mean other bands are less suitable. Interest in weight management can reflect life stage, health concerns, social media exposure, or how recently someone has spoken to a doctor about weight. None of that changes how a prescriber decides.

Some health factors become more common with age, such as high blood pressure, high cholesterol and prediabetes. Because those conditions appear in the licensed criteria, they can matter when someone has a BMI between 27 and 30. A person in that range with one of them can be eligible, while a person in the same range with none is not.1 That is a clinical test, not an age test.

Younger adults can feel they are under-represented in medical conversations about weight, and older adults can feel the opposite, that treatment is not meant for them. Neither impression comes from the licence. What a prescriber will want to know is your medical history, any other medicines you take, and whether Mounjaro is appropriate alongside them.

Medicines interact differently with different health backgrounds, and a prescriber reviewing your questionnaire is looking at exactly that. If you take insulin or sulfonylureas, for example, doses may need adjusting because of the risk of low blood sugar, and your prescriber can advise. If you use an oral contraceptive, the medicine can reduce its absorption because stomach emptying slows.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

So rather than asking "is my age group using jabs?", ask "do I meet the criteria, and is this suitable for me?" The consultation exists to answer exactly that, free and without obligation, reviewed by a GPhC-registered prescriber.

07 · Sex and weight-related conditions

Sex, gender and weight-related conditions

You may have seen claims that weight loss jabs are used more by one sex than the other. The licensing criteria for Mounjaro apply to adults by BMI and health conditions, regardless of sex.1

Differences in who seeks treatment can have many causes: who sees a GP about weight, who is comfortable discussing it, who is targeted by advertising, and who can pay privately. Those are access and attitude questions, not clinical ones. An accurate headline would say "more women than men in this sample reported using a jab", not "jabs work better for women".

On effectiveness, the trial evidence is a population average. In SURMOUNT-1, 2,539 adults taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

Where sex does matter clinically is in individual circumstances. Anyone who could become pregnant should discuss contraception with the prescriber, since the medicine can reduce the absorption of oral contraceptives because stomach emptying slows.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. That is a safety conversation for a prescriber, not a statistic.

Weight-related conditions also cut across any single demographic. High blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes appear in the licensed criteria for people with a BMI between 27 and 30.1 Whether you have one is a matter for your own health record. 08 · Income and the NHS divide

Income, region and the NHS versus private divide

Access to weight loss jabs depends heavily on route, and the route you can use depends partly on money and partly on where you stand in the NHS queue. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions.46 Full rollout is expected to take years.5

That means many people who meet the licensed criteria, for instance a BMI of 30 to 35, do not fall inside the early NHS groups. For them, regulated private treatment is the available route today. Private access is the part of the market where income matters most, because the prescription cost falls on the patient. NHS access to Wegovy runs through specialist weight-management services; ask your GP.

Paying monthly for a medicine is easier for some households than others. Where you live may also affect how quickly you can see a GP or reach an NHS service. The licence is national, but local service capacity can vary.

If you are in the NHS-eligible group, the NHS route can be the right first step. The NHS prescription charge in England is £9.90 per item for those who pay, but weight management medicines through the NHS follow their own service arrangements, so ask your GP what applies to you. If you are not yet eligible, or prefer not to wait, private treatment is a legitimate alternative, provided it comes from a GPhC-registered pharmacy after a prescriber has reviewed your health.

09 · What trials show

What the trial evidence says about who benefits

The trial evidence describes average results in a group of adults, not how a particular demographic will respond. The largest trial here is SURMOUNT-1, which followed 2,539 adults over 72 weeks. People taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg, compared with 3.1% with lifestyle changes alone.2

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.2 As a worked example only, for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

A question like "do jabs work for people my age or sex?" is not answered by the headline figure. What the evidence supports is that these medicines can be effective for adults who meet the licensing criteria and use them as directed.

The mechanism helps explain why the effect is not tied to a single group. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. The effect comes from physiology that adults share, not from a demographic label.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. Nothing needs doing while it clears.1 That is useful to know if you are weighing up a commitment: stopping is not instant, and the effects of a dose fade gradually.

10 · Costs over a year

Planning the cost over a year

If cost is what separates interest from use, then planning a full year is more helpful than looking at one pen. Mounjaro is a once-weekly injection supplied as a multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each pen lasts four weeks, so a year is 13 pens.

If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. Over a full first year, the cost is £2,711.88 if you stay on 5mg after your 2.5mg starting pen, rising to £3,599.88 if you step up through every dose to 15mg. 11 · Safe access

Safe access: buying from a regulated source

Whatever your demographic, the safest way to use a weight loss jab is through a regulated pharmacy after a prescriber has reviewed your health. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity, and individual results vary.

Growing demand attracts counterfeit and unregulated sellers. The MHRA publishes counterfeit medicine alerts, and anyone offering to supply a prescription-only weight loss medicine without a consultation or prescription is a red flag.7 Cheap social media offers, unfamiliar sellers and "no questions asked" supply are all reasons to stop. Like all medicines, Mounjaro can cause side effects, and most are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. How to read demographic claims critically

A good demographic statistic tells you who was counted, how, and when. A poor one gives a percentage with no method. Before relying on a headline about who uses weight loss jabs, run it through a short checklist.

  • Who was asked? A sample of the general population, a pharmacy's customers or NHS patients are three different groups.
  • What was measured? Current use, past use, and interest in using are not the same thing.
  • When was it gathered? The UK market changed with the NHS primary-care rollout in June 2025 and the September 2025 price change, so older data can mislead.
  • Which product? Licensed medicines, tablets, older daily injections and unregulated products are sometimes lumped together.
  • Who funded it? Commercial bodies, charities and universities have different motives and methods.

Be wary of anything that turns a correlation into advice. "Women in their forties use jabs most" does not mean you should or should not. Equally, a claim about a social media trend among young adults does not tell you whether a particular person is eligible.

Why do different sources give different numbers for weight loss jab users?
Each source counts something different. A survey samples adults and may measure interest as well as use, a pharmacy counts its own customers, and the NHS counts prescriptions in its services. Timing matters too, since the NHS primary-care rollout began in June 2025 and list prices changed on 1 September 2025. Check who was counted, what was measured and when before trusting a figure.
