Where the "triple G" name comes from

"Triple G" is informal shorthand, not a medical term, and you will not find it in any UK prescribing reference. It spread through social media and forum posts as a way of referring to retatrutide, the investigational medicine meant to act on three hormone pathways.

Informal names create a problem for patients. A nickname makes an unlicensed product sound like a finished one, and it turns a research programme into something that appears to be on a shelf. It is not. A licensed medicine has a name registered with the regulator, approved patient information, and a clear route for reporting side effects. None of those exist for retatrutide in the UK.

Compare that with the licensed injections you can actually be prescribed. When you read about a medicine, check whether a UK summary of product characteristics exists. If it does not, you are reading about research or marketing, not about something you can safely take.

Other nicknames follow the same pattern. Phrases such as "nature's Ozempic" or "poor man's Ozempic" are marketing labels, usually for low-cost supplements with no clinical oversight. They imply an equivalence that does not exist. A supplement is not a licensed medicine, its dosing is not standardised, and it has not been through the assessment that a prescription weight-loss injection has.

"Triple therapy weight loss medication" is also easily confused with combination products. A genuine triple-action design is a single molecule acting on several pathways. It is not the same as stacking three separate products, which is an unlicensed practice we do not support. The practical lesson: treat any nickname as a prompt to ask "what is the real name, and is it licensed in the UK?" For "triple G" the answers are retatrutide, and no. That one check removes most of the confusion and a lot of the risk.

04 · Licensed versus investigational

Licensed versus investigational: what the difference means

An investigational medicine is one still being tested to establish whether it is safe and effective; a licensed medicine has completed that process and been approved by the regulator for specific uses. The gap between them is not a formality. It is the difference between a product with defined strengths, manufacturing standards and published safety guidance, and a product with none of those for your use.

For Mounjaro, the UK approval defines who it is for, what strengths exist and how it should be used alongside a reduced-calorie diet and increased physical activity.1 Prescribers can look up tirzepatide dosing and safety information in the BNF.11 Nothing comparable exists for retatrutide, so no UK prescriber has an approved dosing schedule to follow.

This is why the claims you see online should be read with caution. Research results describe what happened to volunteers in a controlled study, under supervision, at particular doses. They do not tell a person at home what a vial from an unknown seller contains or what it will do.

It is also why comparisons that line up a research drug against a licensed one can mislead. Different studies enrol different people, last different lengths of time and measure different things. Only a trial that compares two medicines directly can support a head-to-head claim, and no such trial supports a ranking of retatrutide against anything on the UK market.

  • Licensed: approved indication, approved strengths, published leaflet, UK reporting route.
  • Investigational: no approved UK indication, no approved strengths, no UK leaflet.
  • Unregulated "research" products: no assurance of identity, purity or dose.

The main risk is that you cannot know what you are injecting. A product sold as retatrutide by a website, social account or messaging group has not been through UK medicines regulation, and nobody who has reviewed your health has decided it is appropriate for you. A regulated route works differently. A prescription medicine is supplied only after a consultation reviewed by a GPhC-registered prescriber. Anyone offering to supply a weight-loss injection without a consultation or prescription is operating outside that safeguard, whatever the product is called. The MHRA publishes guidance on counterfeit medicines and runs the Yellow Card scheme for reporting problems.7

Specific problems to be aware of with unregulated supply include:

  • No assurance the contents match the label, or that the product is sterile.
  • No tested storage or cold-chain handling during postage.
  • No prescriber checking your medicines, conditions or contraception.
  • No aftercare if you feel unwell, and nowhere obvious to report a reaction.
  • Dramatic weight-loss claims used to sell the product, with no evidence you can check.

Medicines also interact. Licensed information for tirzepatide notes that slowing of stomach emptying can affect how quickly some oral medicines are absorbed, and prescribers need to know what you take, particularly warfarin or other narrow-margin medicines, insulin or sulfonylureas.1 A seller of unlicensed product will not carry out that check.

If you have already used an unregulated product and feel unwell, get medical help straight away; for severe, persistent stomach pain, chest pain, swelling of the face, lips, tongue or throat, or difficulty breathing, get medical help straight away. Report the suspected side effect through the Yellow Card scheme.19 Reporting helps the regulator spot problems even for products that should not be on the market.

Mounjaro is the licensed once-weekly injection that targets two hormone receptors, and it is available in the UK now. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 If you are drawn to "triple" because you want more than a single-pathway medicine, the dual-action licensed option is the nearest real-world step.

Mounjaro comes in six strengths, so treatment can be stepped up under prescriber supervision. The starting pen is 2.5mg, and your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99). Every later order is £25 off the standard price (code DOSE25).

Mounjaro versus semaglutide: how to compare fairly

Mounjaro acts on GIP and GLP-1, while semaglutide, sold as Wegovy for weight management and as Ozempic for type 2 diabetes only, is a single-action GLP-1 medicine.1 Both are effective licensed treatments, and neither is a superior choice for everyone.

For a direct comparison in obesity, the relevant study is the head-to-head trial of tirzepatide against semaglutide.15 That is the only kind of evidence that supports a side-by-side claim.

Practical differences matter as much as headline results:

  • Both are once-weekly injections given with a pre-filled pen.
  • Mounjaro steps through 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, with the dose decided by your prescriber.1
  • Tolerability and personal response differ, so the right choice is individual.

Tablets, liquids and other options people ask about

If injections are the sticking point, licensed tablets now exist alongside the injectables. Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) both have UK product information, and each is described in its own summary of product characteristics.2021 Their trial programmes were run separately, so their results should never be lined up against Mounjaro's as though they were tested head to head.2223

There is no single best weight loss pill for everyone. The right option is the licensed one that suits your health, tolerance and goals, chosen with a prescriber. Be wary of any page that names a single winner for everyone.

Liraglutide, supplied as Saxenda and in products such as Erly, is a daily injection at a concentration of 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy.

Supplements deserve a separate warning. Berberine and similar products are marketed as a "poor man's Ozempic" or "nature's Ozempic". They are low-cost supplements with no clinical oversight, their dosing is not standardised, and they are not equivalent to any prescription medicine. When researching any option, ask four questions:

  • Is it licensed in the UK for weight management?
  • Does it come with a UK patient leaflet?
  • Will a prescriber review my health before supply?
  • Do I know how to report a side effect?

If the answer to any of these is no, treat the product as unproven. A regulated pharmacy will always be able to say yes to all four.

09 · NHS and private access

NHS and private routes to treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Neither route offers retatrutide. It is not part of NHS guidance, and no private UK clinic can lawfully prescribe it as a licensed treatment. If you read that someone is offering it through a prescriber, ask which licence covers it; there is none.

Where private treatment fits depends on your circumstances. Some people qualify for NHS support but face a wait. Others do not meet the current NHS criteria but are within the licence. 10 · Safety, storage and starting

Safety, storage and what to expect when you start

Before starting any licensed injection, tell your prescriber about everything you take. Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose.1 Mention warfarin or other narrow-margin medicines, insulin, and sulfonylureas, where doses may need reducing to avoid low blood sugar. No adjustment is needed for paracetamol.1

The medicine can also reduce absorption of oral contraceptives because stomach emptying slows, so ask your prescriber about contraception.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Side effects are listed in the patient information leaflet, and you should report any suspected side effect through the Yellow Card scheme.1319 Regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

Storage matters for the pen. Keep unopened pens in the fridge at 2–8°C until first use, and they keep until the printed expiry date. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.13 For anything else about storage, check the leaflet or ask the pharmacy. For the injection itself, follow the pen's instructions. Needles, a sharps bin and injection guidance come with every order.

If you stop, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system; there is no way to speed this up, and nothing needs doing while it clears.1

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Is retatrutide better than Ozempic?
Nobody can say so reliably, because retatrutide is still an investigational medicine with no UK licence and no approved UK product information. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine — and its safety and dosing are documented. A claim that one is better needs a head-to-head trial and regulatory approval, and neither exists here. What are the risks of taking retatrutide?
