In practice the realistic prescription options for someone considering medication are Mounjaro, Wegovy and liraglutide. Oral GLP-1 tablets such as Wegovy tablets and Foundayo are a newer route, and your prescriber can tell you what is suitable. Supplements sold as natural alternatives are cheap and easy to buy, but they come with no clinical oversight and are not comparable to licensed medicines.

Most readers face one of two decisions: whether a prescription medicine is relevant to them, and if so which one to discuss with a prescriber. Relevance depends on your BMI and health history. UK licensing for weight management generally starts at a BMI of 30 kg/m², or 27 kg/m² with a weight-related condition, and your prescriber assesses the detail.

03 · Why no padded top ten
Why a top ten does not exist
A genuine top ten of weight loss medications does not exist in the UK, because only a short list of prescription medicines is licensed for weight management. Pages that reach ten usually do it by adding diabetes drugs, supplements, old stimulant products or unlicensed imports. Those entries sound impressive but carry very different evidence and very different risks.
The licensed injections are the established centre of the field. Oral GLP-1 tablets are the newest addition.
When you read any ranked list, check four things. First, is each medicine licensed in the UK for weight management? Second, are the results quoted from named trials, and are they compared fairly? Third, does the page mention side effects and who should not take the medicine? Fourth, does it tell you a prescriber must assess you? A list that skips these is selling, not informing.
Be especially careful with anything described as a natural version of a prescription medicine. Supplements are available cheaply without assessment, and nothing about them has been shown to behave like a licensed GLP-1 medicine. Seller claims of dramatic weight loss should be treated as marketing. If a seller offers to supply a prescription medicine without a consultation or prescription, walk away, because that is a route to counterfeit products. The MHRA publishes counterfeit alerts and runs the Yellow Card scheme for reporting side effects or suspect medicines.7
Finally, a medicine is only one part of weight management. 04 · Mounjaro explained
Mounjaro (tirzepatide): what it is and how it works
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1
Treatment starts at the lowest strength and steps up, rather than starting at a higher dose. Your prescriber reviews you as you move through the strengths and decides where you settle.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
Storage is simple. Keep unopened pens in the fridge at 2–8°C until first use; they keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.

05 · Wegovy and semaglutide
Wegovy (semaglutide): the other weekly injection
Wegovy is a once-weekly semaglutide injection and the other main licensed option in this class for weight management. Where Mounjaro activates both GIP and GLP-1 receptors, semaglutide is a single-action GLP-1 medicine, and that difference in mechanism is the clearest factual contrast between them.1
Wegovy pens come in a ladder of strengths: 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. The dose steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.18
Practical differences matter as much as mechanism. Think about how the medicine is supplied, how your prescriber handles titration, what you tolerate, and what is available when you order. For Wegovy, check availability at the time of order. 06 · Liraglutide and tablets
Liraglutide, oral tablets and other options
Liraglutide is a daily injection, unlike once-weekly Mounjaro and Wegovy, and it is supplied at a concentration of 6mg per millilitre. It is sold under brands including Saxenda, and products such as Erly use the same concentration. The daily schedule is the main practical difference from the weekly injections, and some people find a daily routine harder to maintain.
Oral GLP-1 tablets are the newest development. Wegovy tablets contain oral semaglutide, and Foundayo is a separate oral GLP-1 tablet. Tablets are attractive to people who dislike needles, but they are different medicines from injections, and your prescriber can advise on suitability.
You may also see questions about whether there is a tablet equivalent to Mounjaro. Mounjaro's mechanism, activating both GIP and GLP-1 receptors, is described as the first of its kind, and we do not equate any tablet with it. Tablets and injections are different medicines with different evidence, so treat any claim of equivalence with caution.
Orlistat is a long-established capsule option; figures vary and your prescriber can advise. Stimulant-style or unlicensed pills sold online are a different matter: avoid products offered without a consultation or prescription, and report suspected side effects or suspect medicines through the Yellow Card scheme.19

07 · Diabetes medicines and weight
Diabetes medications and weight loss
Diabetes medicines are prescribed to control blood sugar, and any weight change is a secondary effect that differs from drug to drug. Some diabetes treatments are associated with weight loss, others are neutral and some can lead to weight gain, which is why your diabetes team chooses on several factors, not weight alone.
Mounjaro sits in both worlds. It is also licensed for type 2 diabetes, where it improves blood sugar control (HbA1c) and supports weight loss. If you have type 2 diabetes, treatment and monitoring are coordinated differently, so tell your prescriber and your usual diabetes team so care stays joined up.1
Do not start or stop a diabetes medicine to lose weight without speaking to the clinician who manages it. Changing treatment can destabilise blood sugar. If you take insulin or sulfonylureas, doses may need reducing to avoid low blood sugar when a GLP-1-based medicine is added, which is a decision for your prescriber and diabetes team, not something to adjust yourself.1
It is also worth separating two groups. People with type 2 diabetes who want help with weight should raise it at their diabetes review. People without diabetes who are considering a weight management medicine follow a different pathway, assessed against weight-management criteria. Both routes involve a clinician looking at the whole picture.

08 · NHS versus private
NHS access versus private treatment
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46
So, do you qualify for Mounjaro on the NHS? The honest answer is that it depends on where you sit in the phased rollout and on your health conditions. Your GP is the right person to ask, and the NHS guide to tirzepatide explains the medicine in plain language.5 Do not assume you are excluded or included from a headline alone.
If you are asking how to lose weight quickly on the NHS, for example a specific amount in a set number of weeks, we would gently reframe the question. No medicine or programme can promise a particular outcome in a particular timeframe, and chasing a rapid target is often counterproductive. NHS services focus on steady, sustainable change, supported by diet, activity and, where eligible, medication.
Private treatment is a choice some adults make when they meet the licensed criteria but cannot yet access NHS supply. It involves a free online consultation, review by a GPhC-registered prescriber, and ongoing monthly reviews. 09 · Safety and interactions
Safety, interactions and who should be careful
Every prescription weight loss medicine needs a clinician's review before you start, because safety depends on your health, your other medicines and your circumstances. Mounjaro (tirzepatide) is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.1 Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose. Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin, or sulfonylureas (where doses may need reducing to avoid low blood sugar). No adjustment is needed for paracetamol.1
Contraception deserves a specific mention. The medicine can reduce absorption of oral contraceptives, because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Ask your prescriber what protection to use, and read the MHRA guidance.7
Like all medicines, Mounjaro can cause side effects, and most are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Do not rely on a ranked list or a forum for that detail. If you notice a new or worrying symptom, speak to your prescriber, and report suspected side effects through the Yellow Card scheme.1319
Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review. Any big change in weight, appetite and routine can affect how you feel: tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17

In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

A Mounjaro pen is one four-week supply, and the standard price rises with the dose. The 2.5mg pen is £169.99, 5mg is £218.99, 7.5mg is £279.99, 10mg is £290.99, 12.5mg is £319.99 and 15mg is £329.99, all per month. Prices were checked 11 October 2026.
If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year of 13 pens costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. Movement matters for general health, strength and mood, not only for the scale. Start with what you enjoy and build gradually. Resistance exercise is a helpful part of a plan because it supports muscle, though your prescriber can tailor advice to your health.
Finally, think about what happens long term. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. 12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
