05 · What tirzepatide is

What tirzepatide is and how it works

Tirzepatide is the active ingredient in Mounjaro, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each pen contains four weekly doses, so one pen covers one month of treatment.

Tirzepatide is the generic name, and Mounjaro is the brand. In the UK, the brand product from Eli Lilly is the licensed route, supplied against a prescription after a consultation.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

That dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, sold as Wegovy and Ozempic. Tirzepatide works differently from semaglutide, through two receptors rather than one. A prescriber can talk you through the choice.

It is worth being clear about what the injection is not. It is not a replacement for eating well or moving more. The licence says it must be used alongside a reduced-calorie diet and increased physical activity. The pen does the appetite-regulating work, while the habits you build around it decide how much of that effect is useful in daily life.

If anything in it worries you, raise it with your prescriber before you start rather than after.

06 · Who can use it

Who can use it: UK eligibility

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.

That is the licensed position for private prescribing. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you. The decision is clinical and individual. Someone who meets the BMI numbers may still be advised against treatment, and the review exists to catch that.

You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.13 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. NHS versus private

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 or above with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

The NHS threshold is narrower than the licence. If you sit at a BMI of 30 to 35, or between 27 and 30 with a weight-related condition, you may meet the licensed criteria without being in an early NHS group. Ask your GP where you fall in the current rollout.

Why the consultation matters

The consultation is where eligibility is actually decided. It is free, with no obligation, and payment is taken when you place the order. If the prescriber decides Mounjaro is not suitable, the order is refunded automatically in full. That means you do not pay for treatment the prescriber decides is not suitable.

07 · Doses and titration

The dose ladder and first weeks

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals, through 7.5mg, 10mg and 12.5mg, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace. Settling at a dose that works for you is a clinical decision, not a race to the top.

If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. That total is before discounts: a new customer's first order is £40 less, and every later order is £25 less.

What the first weeks feel like

The 2.5mg starting weeks are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. The early effects are the ones to expect and plan around. 08 · Results to expect

What results to expect

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Trial averages do not predict your own result, so treat them as a picture of what the study found, not a forecast.

A worked example

To make the percentages concrete, for someone starting at 100 kg, 20.9% is about 21 kg, and 15% is about 15 kg. "How much will I lose in 2 months?"

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1 Because the dose ladder moves up at a minimum of 4-week steps, two months usually sits within the early part of treatment. Think in months, not days, and judge progress by trend, not individual weigh-ins.

When your prescriber reviews progress

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

Practical habits make the review more useful. Weigh at the same time of day, record the trend rather than reacting to single readings, and note how your appetite and side effects change as the dose moves. Finally, remember that Mounjaro is licensed for use with a reduced-calorie diet and increased physical activity. The trial comparison group, who had lifestyle changes alone, lost 3.1% on average. The medicine moves the odds in your favour, but the daily habits are still part of the treatment.

10 · Injecting and storage

Injecting, missed doses and storage

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1

There is no "best" time of day. Mounjaro works the same morning or evening. What matters is the same day each week. Needles, a sharps bin and injection guidance come with every order, and the pen's own instructions are the guide to the injection itself. Follow them closely, and ask our team if anything is unclear.

Setting the dose

The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

If you miss a dose

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1 If you want to move your regular day, you can, provided at least 3 days (72 hours) have passed since your last dose.

Storage

Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK label is the one that applies to a UK pen.

Travelling

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly, and that includes the United States if you are heading to California.

11 · Stopping and duration

How long you can take it and what happens after

The right duration is a clinical decision made with your prescriber at regular reviews. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Our aftercare includes ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team.

Trials measured results at 72 weeks, but that is the length of a study, not a limit on treatment. Whether you continue, hold your dose or stop is something you decide with your prescriber, based on how you are doing, what you tolerate, and what you want.

Does it wear off after 5 days?

No. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 A half-life is the time for the amount in your body to fall by half, not the time for it to disappear. So after five days, about half is still there. That slow clearance is also one reason the once-weekly schedule works.

What happens after you stop

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Discuss your plan with your prescriber before you stop, not after, so you have a diet and activity routine ready that you can keep up without the injection.

If you stop for a while and want to restart, tell your prescriber how long you have been off. Restarting is a clinical decision, and your prescriber will decide which dose to begin at.

Switching or pausing safely

If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms. Always use one pen at a time, and throw it away 30 days after first use, or sooner if it has been above 30°C.

Is tirzepatide stronger than Ozempic?
Tirzepatide works differently rather than being simply stronger. It activates two gut-hormone receptors, GIP and GLP-1, while semaglutide, sold as Wegovy and Ozempic, is a single-action GLP-1 medicine. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine; the licensed weight-management options are Mounjaro and Wegovy, and which suits you depends on your health profile, tolerance and prescriber advice. A prescriber can assess which is suitable for you after reviewing your medical history and current medicines.
What is the bad side of tirzepatide?
The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and these usually settle. The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Seek immediate medical help for severe, persistent stomach pain that may spread to your back (possible pancreatitis), pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction. Tell your prescriber about any symptom that worries you, and report suspected side effects through the Yellow Card scheme.
Who should avoid tirzepatide?
Mounjaro is licensed for adults with a BMI of 30 or above, or between 27 and 30 with a weight-related condition, alongside a reduced-calorie diet and more activity. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Answer the consultation questions fully, because that review is how unsuitable use is caught, and unsuitable orders are refunded automatically.
How long can you take tirzepatide?
The decision sits with you and your prescriber at regular reviews. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right. What happens after you stop taking tirzepatide?
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear your system. There is no way to speed this up, and nothing needs doing while it clears. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average. Talk to your prescriber before stopping so you have a diet and activity plan ready.
Does tirzepatide wear off completely after 5 days?
No. Five days is the half-life, meaning about half the medicine is still in your body at that point. After a final dose, it takes roughly 4 to 5 weeks to clear the system, and nothing speeds this up. That slow clearance is why a once-weekly injection works. If you miss a dose, take it within 4 days (96 hours) of the scheduled time, otherwise skip it.
What is the success rate of tirzepatide?
In SURMOUNT-1, which enrolled 2,539 adults, 85% of people on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight at 72 weeks. Half of those on 10mg and 57% on 15mg lost at least 20%. These are trial averages alongside diet and exercise, and trial averages do not predict your own result.
