Tirzepatide has been tested in trials built to look at heart and circulation outcomes, and your prescriber can advise what they mean for you. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises.

Mounjaro is the brand name for tirzepatide, a once-weekly injection made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes.1 It activates two natural gut-hormone receptors at once, GIP and GLP-1.1 Its weight-loss results in trials are covered in the SURMOUNT-1 and SURMOUNT-5 trials.

It matters most if you have type 2 diabetes, existing heart or artery disease, high blood pressure, or a strong family history. The trial populations differ from person to person, so the right question is not "what did the average participant gain?" but "what does my prescriber know about my risk?"

A weight-loss result cannot be stretched into a heart-protection claim. Each design answers its own question, and each deserves its own evidence.

04 · What outcomes trials measure

What a cardiovascular outcomes trial measures

A cardiovascular outcomes trial follows a large group of people for long enough to count events. The usual headline is a composite of the most serious events, such as heart attack, stroke and death from cardiovascular causes. Other outcomes, including hospital admission for heart failure or death from any cause, are often reported separately.

These trials are designed in stages. The first question is usually safety: does the new medicine avoid increasing cardiovascular risk? Only after that is shown can the trial ask whether the medicine does better than the comparator. Reporting that something is "non-inferior" is therefore a different statement from saying it is "superior".

This matters when you read a summary. A line such as "met its primary goal" can mean the medicine was shown to be no worse than another treatment. It does not automatically mean it prevents events better. When you see a headline figure, check which of these two questions it answers.

Trials also differ in who takes part. Results in people with type 2 diabetes and established artery disease do not transfer neatly to a healthy adult with obesity. 05 · Named tirzepatide trials

Tirzepatide trials in heart health

SURPASS-CVOT is the cardiovascular outcomes trial in adults with type 2 diabetes and established cardiovascular disease. SURMOUNT-MMO is the outcomes programme aimed at people with obesity, looking at morbidity and mortality. Because it concerns obesity rather than diabetes, it is the one most relevant to people seeking weight management.

Trials in heart failure with preserved ejection fraction and obesity also exist. They look at heart failure events rather than heart attack and stroke.

06 · Verified weight-loss results

What the weight-loss trials show

Weight is a major driver of cardiometabolic health, which is why people ask about heart outcomes in the first place.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

For a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. Your own result depends on dose, adherence, diet and activity.

07 · Tirzepatide versus semaglutide

How does tirzepatide compare with semaglutide?

For weight loss, the direct comparison is SURMOUNT-5, the first large head-to-head trial, which compared tirzepatide with semaglutide 2.4mg, the dose used in Wegovy. It did not test Ozempic. In SURMOUNT-5, 751 adults took tirzepatide or semaglutide 2.4mg for 72 weeks.15

At least 10% of body weight was lost by 81.6% on tirzepatide vs 60.5% on semaglutide; at least 15% by 64.6% vs 40.1%; at least 20% by 48.4% vs 27.3%; at least 25% by 31.6% vs 16.1%. Average loss was 22.8 kg vs 15.0 kg.15

That is a weight comparison. It tells you nothing about heart events. Do not assume a better weight result means a better cardiovascular result, and do not assume the reverse. Both medicines are licensed treatments, and which suits you depends on your health profile, tolerance and prescriber advice.

08 · Mechanism and drug class

What class of medicine is tirzepatide?

Tirzepatide belongs to the class of medicines that activate gut-hormone receptors. It is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Insulin and sulfonylureas may need dose adjustments because of the risk of low blood sugar, so tell your prescriber if you take either.11

09 · Safety and stopping

Side effects, stopping and long-term use

The full list of side effects for tirzepatide is in the patient information leaflet, and it is the document to read before starting.13 If you notice a new symptom, tell your prescriber, and you can report it through the Yellow Card scheme.19 Serious side effects are uncommon but need urgent attention: for severe, persistent stomach pain, yellowing of the skin or eyes, or swelling of the face or throat with difficulty breathing, get medical help straight away.

If you stop, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Whether to continue long term is a decision to make with your prescriber at regular reviews, taking your heart health, weight and tolerance into account.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

How people in the UK access tirzepatide

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
