Who should think about what
If you have type 2 diabetes and existing heart or circulation disease, this comparison is directly relevant to you, and the decision should be made with your diabetes team. They know your kidney function, medicines and cardiac history; we do not see that full picture from an online questionnaire alone.
If you have type 2 diabetes but no heart disease, the trial is context rather than a verdict. Day-to-day factors usually matter more: blood sugar control, side effects, how you tolerate injections, and what your prescriber recommends.
If you are researching Mounjaro for weight loss rather than diabetes, a diabetes cardiovascular trial does not answer your question.
04 · Reading the trial paper
How to read a cardiovascular outcomes trial
A cardiovascular outcomes trial asks about events such as heart attack or stroke, not just lab numbers. That makes it a different kind of evidence from a trial measuring HbA1c or weight.
When you open the paper, look at four things. First, who was enrolled: the participants' health profile tells you whether you resemble them. Second, the primary endpoint: which events were counted together. Third, whether the aim was to show the medicine was no worse than the comparator or better than it. Fourth, the confidence interval around the result, which shows how precise the estimate is.
Those questions matter more than a single headline number. A result that shows a medicine is no worse than another is not the same claim as a result showing it is better, and summaries often blur the two. If an article you read does not make the distinction clear, go back to the paper.
Finally, a trial average does not predict your own result. Individual risk depends on age, blood pressure, cholesterol, smoking, kidney health and much else that your clinician will weigh.
05 · Diabetes and the heart
Can diabetes affect your heart?
Heart health is a routine part of type 2 diabetes care, which is why cardiovascular trials exist for diabetes medicines at all. Your diabetes team will normally review heart-related risk alongside blood sugar, and can explain your personal picture.
Mounjaro is licensed for type 2 diabetes, where it improves blood sugar control and supports weight loss.1 If you have diabetes, treatment and monitoring are coordinated differently from weight-management care, so tell your prescriber and your usual diabetes team about everything you take so care stays joined up.

06 · How tirzepatide works
How tirzepatide helps in type 2 diabetes
Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.
This dual action distinguishes it from single-action GLP-1 medicines such as semaglutide. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. For a broader look at the drug class, see GLP-1 medicines in type 2 diabetes.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.
07 · Weight-loss trials, separately
Weight-loss trial results are a separate question
Heart-outcome trials and weight-loss trials answer different questions, and their numbers should not be mixed.
In SURMOUNT-1, 2,539 adults on tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages and individual results vary.
In the first large head-to-head trial (SURMOUNT-5), adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15 Both are effective licensed treatments. SURMOUNT-5 compared tirzepatide with semaglutide, not dulaglutide, so it says nothing about dulaglutide.
08 · Other heart drugs
SGLT2 inhibitors, plaque and heart failure
Which medicine suits a person with heart failure depends on their diagnosis, kidney function and other treatments, so it is a decision for a cardiologist or diabetes specialist. Nobody should start or stop a heart medicine without advice from their specialist.

Report any suspected side effect through the MHRA Yellow Card scheme.19 Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.
