Mounjaro comes with a patient information leaflet and a summary of product characteristics113. Both are written for you and your prescriber to read before starting. Reading them with your oncology nurse or pharmacist, with tamoxifen in front of you, is a sensible step.

04 · How Mounjaro works

How Mounjaro works, and why it matters to the conversation

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-11. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

Because a prescriber thinking about your safety wants to know what the medicine does, not just its name. A treatment that slows stomach emptying is relevant to anything else you take by mouth, and a treatment that changes appetite is relevant if you are already managing nausea or low appetite from another cause. Those are questions for your prescriber.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic)1. Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg1. Treatment normally begins at the lowest strength, with a prescriber deciding about steps up. That gradual approach is part of why monthly reviews matter, particularly when you are on other long-term treatment.

05 · Weight changes on tamoxifen

Weight changes during tamoxifen: the honest picture

Many people on tamoxifen ask why weight seems harder to manage, whether it is hard to lose weight on tamoxifen, and what the average gain might be.

Appetite, activity, sleep, mood, other medicines, menopausal status and the effects of a cancer diagnosis itself all play a part. Separating them is a job for someone who can examine you and look at your records. If you feel your weight has shifted in a way that worries you, raise it at your next review rather than waiting.

A general approach is the one all licensed weight management medicines are built around: a reduced-calorie diet and increased physical activity1. Physical activity during or after cancer treatment should be pitched to your own situation, so ask your oncology team or a cancer rehabilitation service what is suitable.

If you have stopped tamoxifen and weight has not shifted, that is also something to discuss with your GP. Weight can stay put for reasons that have nothing to do with the drug, and a review may uncover something treatable. Nothing here should make you feel you have failed; a stubborn scale is a common experience and a legitimate reason to ask for support.

06 · Questions to bring

Questions to bring to your oncology team and prescriber

Going into the conversation prepared makes it shorter and more useful. These are the questions we would want a patient to ask, in rough order of importance.

  • Is there any reason, in my diagnosis or treatment plan, to avoid a weight loss medicine at this point?
  • Would you prefer I wait until a particular stage of treatment has finished?
  • Which of my current medicines should my prescriber know about, and should they contact you directly?
  • If I begin a weekly injection, which symptoms should I report promptly, and to whom?
  • Are there non-drug options, such as dietitian or exercise support, that you would suggest first or alongside?
  • How will we review whether the plan is working, and when would we stop?

Write down the answers. If your oncology team asks for information from the prescriber, ask for it to be shared in writing so that everyone is working from the same facts.

It also helps to bring a complete list of what you take: prescriptions, over-the-counter products and supplements. Supplements sold as natural weight loss aids are available cheaply without assessment, and their dosing is not standardised. They are not a substitute for a medicine reviewed by a prescriber, and they are worth declaring to your oncology team all the same.

07 · Other weight loss options

Wegovy, liraglutide and other options

Mounjaro is not the only licensed route, and your team may prefer a different one. Wegovy (semaglutide) is a once-weekly injection with a dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, with the 2.4mg maintenance dose starting from week 1718. Liraglutide (Saxenda, and products such as Erly) is supplied at 6mg/ml and is a daily injection, unlike the weekly options.

If you are asking whether you can take Wegovy while on tamoxifen, the answer is the same as for Mounjaro. It depends on your individual review.

08 · NHS access

NHS access to weight loss injections

Mounjaro is available on the NHS, but access is being phased in over several years46. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years.

If you are under hospital or specialist care for breast cancer, your team may be able to tell you whether an NHS route is realistic for you and whether a specialist should oversee it. For some people that is a better first conversation than a private consultation, because the people who manage your cancer care are already in the loop.

Others choose regulated private treatment rather than waiting. That is a legitimate choice, as long as the prescriber is told about tamoxifen and your oncology team is aware. A private route does not remove the need for coordination; if anything it makes it more important.

Wegovy access on the NHS runs through specialist weight-management services; ask your GP.

09 · Stopping tamoxifen

Stopping tamoxifen and how long things take to settle

You should not stop tamoxifen to make room for a weight loss medicine, or because you feel unwell, without speaking to your oncology team first. It is prescribed for a reason, and any decision to change it is theirs to make with you.

Raise any lingering symptoms with your GP or oncology nurse; they can tell you what to expect for your situation and whether anything else needs checking.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system1. There is no way to speed this up, and nothing needs doing while it clears. If your oncology team ever asks you to pause a weight loss injection, that is the period they will be thinking about.

Keep your prescriber informed of any change to your cancer treatment, in either direction. A start, stop or switch of any medicine can change what is suitable for you.

10 · Side effects and reporting

Side effects, nausea and reporting a problem

The patient information leaflet for each medicine lists the recognised effects, and your pharmacist can help you read them side by side.

If you start anything new and feel worse, contact your prescriber and your oncology team rather than waiting it out. The same goes for any symptom that is new to you. If you have severe, persistent stomach pain, chest pain, a racing heart, fainting, swelling of the face, lips, tongue or throat, difficulty breathing, or yellowing of the skin or eyes, get medical help straight away.

You can report suspected side effects of any medicine through the MHRA Yellow Card scheme19. Reports help regulators spot patterns, and you do not need to be certain that a medicine caused the problem to submit one.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
