A quick overview

What to know
before you start.

  • Serious side effects such as pancreatitis are uncommon, but they need urgent attention if they happen.
  • Tell your prescriber about any history of pancreatitis or gallbladder problems before treatment starts.
  • Severe, persistent stomach pain that may spread to your back, with or without vomiting, is a reason to seek immediate medical help.
  • Do not wait to see whether the pain settles, and do not use food or home remedies to treat suspected pancreatitis.
  • Report suspected side effects through the MHRA Yellow Card scheme.
In this guide

01 · The core answer

How big is the risk of pancreatitis with Mounjaro?

The risk is small for most people, but it is real, and that is why it appears in the product information. The Mounjaro patient leaflet lists possible pancreatitis among the serious side effects to act on, and serious side effects as a group are uncommon.13

Pancreatitis is uncommon, and recognising the warning sign early matters. Knowing the warning sign matters more than memorising a statistic.

02 · Your decision

Is this relevant to me, and what should I do next?

It is most relevant if you have ever had pancreatitis, gallbladder problems or severe digestive disease. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.13 That is why the consultation asks about your full medical history and medicine list.

Your situationWhat to do
Previous pancreatitisSay so in your consultation. Your prescriber will weigh it, and may advise against treatment.
Gallbladder problemsDeclare them. This is another reason for extra prescriber care.
Severe, persistent stomach pain spreading to your backSeek immediate medical help. Do not wait for it to pass.
Mild nausea or a heavy feeling after mealsCommon early on. Smaller, slower meals often help, and you can raise it at your monthly review.
Unable to keep fluids downContact your clinical team — dehydration needs prompt attention.

If you are still deciding whether treatment suits you, you can start a free consultation for the Mounjaro pen, where a GPhC-registered prescriber reviews your history. Our wider weight loss clinic page explains the full pathway.

03 · Warning signs to act on

Early warning signs of pancreatitis on Mounjaro

The sign to act on is severe, persistent stomach pain that may spread to your back, with or without vomiting: stop using Mounjaro and get medical help straight away.13

The pain is what separates this from ordinary medicine side effects. Mild nausea, a full feeling or a bout of indigestion in the first weeks of treatment are common and usually settle. A severe pain that keeps going, particularly one that moves towards your back, is a different picture.

If that happens, seek immediate medical help. You can tell your prescribing team afterwards.

Please do not self-diagnose from a symptom list and do not delay because you are unsure. Our page on Mounjaro pancreatitis symptoms goes into how to describe your symptoms when you seek help.

Suspected side effects can also be reported through the MHRA Yellow Card scheme, which helps regulators monitor safety.19

04 · Who needs extra care

Who needs extra care before starting

People with a history of pancreatitis, gallbladder problems, severe gut or digestive disease or diabetic retinopathy need extra prescriber care.13 It does not mean all of these people are barred, but it does mean the decision is individual.

This is the reason a medical questionnaire sits at the start of the process. Our consultation is free and is reviewed by a GPhC-registered prescriber, not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

Gallbladder trouble is worth separating from pancreatitis. Upper right abdominal pain, fever or yellowing of the skin or eyes may signal possible gallbladder problems, and also needs medical help straight away.13 Seek immediate medical help for signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.

05 · Reducing your risk

How to reduce the risk of pancreatitis on Mounjaro

There is no proven way to prevent pancreatitis while taking Mounjaro or gallbladder problems. Before you start, tell your prescriber if you have ever had pancreatitis, gallstones or gallbladder disease, or severe problems with your digestion.

Sensible general habits for comfort on treatment include:

  • Share your full medical history and medicine list during consultation.13
  • Eat smaller, slower meals. Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly.
  • Keep fluids up, especially if you have vomiting or diarrhoea.
  • Keep alcohol moderate.
  • Do not push through severe, persistent abdominal pain; get medical help straight away.

They make treatment more comfortable and keep you safe in general. They are not a proven shield against pancreatitis. For a practical walk-through, read how to reduce the risk of pancreatitis on Mounjaro.

06 · Weight loss and pancreatitis

Can sudden weight loss cause pancreatitis?

Acute gallbladder problems on Mounjaro were linked to the amount of weight lost; if you are worried about pancreatitis, speak to your prescriber. Some of any rapid weight loss comes from lean tissue, which is why protein at each meal and resistance exercise matter, especially as the dose rises and appetite falls.

In the major obesity trial, people on tirzepatide with diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

Treatment is licensed alongside a reduced-calorie diet and increased physical activity. A steady, sustainable approach is both the licensed approach and the more comfortable one. If you are eating very little, struggling with persistent vomiting or feeling unwell, tell your clinical team rather than assuming it is normal.

07 · If you suspect pancreatitis

What to do if you suspect pancreatitis

If you have severe, persistent stomach pain that may spread to your back, with or without being sick, stop using Mounjaro, get medical help straight away and tell the clinician you are taking Mounjaro.13 Take your pen or its packaging if you can, so the dose and timing are clear.

People often ask which foods calm pancreatitis. Suspected pancreatitis is not a condition to manage with diet at home. Treatment decisions belong with the doctors assessing you, and they will tell you what to eat, drink and take.

Once you have been assessed, let your prescribing team know. They will decide with you what happens to your treatment. Do not restart or adjust doses by yourself. You can also submit a report through the Yellow Card scheme.19

Because tirzepatide takes roughly four to five weeks to clear the system and nothing speeds this up, medical teams may want to know about your most recent dose.1 For a broader overview, see what to expect with Mounjaro and pancreatitis.

08 · Putting the risk in perspective

Putting the risk in perspective

Pancreatitis is a genuine reason for care, not a reason for panic. Most people on Mounjaro never experience it, yet it is serious enough that the product information tells you exactly what to look for.13

Our overview of pancreatitis risk across this medicine class sets out how the picture looks more widely. If your own circumstances make you uncertain, the right step is a conversation with a prescriber, not a search forum. Our page on whether you can get pancreatitis on Mounjaro answers the question directly.

What foods calm down pancreatitis?

Should I stop Mounjaro if I think I have pancreatitis?
Yes — stop using Mounjaro and get medical help straight away, and tell the clinician you take Mounjaro. Then contact your prescribing team so they can guide what happens to your treatment. Do not restart, change dose or carry on injecting on your own. Tirzepatide takes roughly four to five weeks to clear, and nothing speeds that up.
