No lifestyle step can promise that pancreatitis or gallstones will never happen on Mounjaro.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.113

04 · Dose pace and titration

Why the dose ladder matters

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1

The dose then rises in steps of 2.5mg at a minimum of 4-week intervals, up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber will guide the right pace.1

Do not alter doses yourself

Altering doses outside the licence means unknown dosing accuracy and no safety data.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

05 · Food and meal habits

Eating in a way that sits well with treatment

No food is banned on Mounjaro, but some choices make treatment easier. Prioritise protein at each meal, vegetables and fibre, and regular fluids. Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly. Smaller, slower meals work better.1

Meal habits worth building

  • Eat slowly and stop when you feel comfortably full, because the stomach empties more slowly on treatment.
  • Choose grilled, baked or steamed protein over deep-fried options.
  • Keep portions modest rather than saving up for one large meal.
  • Avoid very sugary food and drinks that leave you feeling unwell.
  • Watch high-calorie coffee drinks, which are an easy place for the calorie deficit to leak.

Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.1

What about coffee?

There is no interaction between Mounjaro and caffeine, so coffee and tea are fine. Strong coffee on an empty, slower-emptying stomach can aggravate nausea or reflux for some people. Rules about coffee circulating on social media have no clinical basis.1

06 · Alcohol and pancreatitis

Alcohol: keep it moderate or leave it out

There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual. Alcohol also adds calories that work against your goals. Moderation is the sensible rule, and many people find their desire for alcohol drops on treatment.1

If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. Your doses of those medicines may need adjusting while on Mounjaro, so speak to your prescriber.1

07 · Hydration and vomiting

Staying hydrated and handling vomiting

Drinking enough matters more on Mounjaro than usual. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly.1

Keep water within reach through the day. Treat inability to keep fluids down as a contact-your-clinical-team issue, not something to push through.1

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Persistent vomiting or severe, persistent stomach pain is different from those mild effects: get medical help straight away.

08 · Weight loss pace and gallstones

Gallstones, weight loss pace and what is known

Gallstones are a common side effect in the weight-management trials, affecting between 1 in 100 and 1 in 10 people, and acute gallbladder problems were linked to the amount of weight lost. Inflammation of the gallbladder was uncommon, affecting between 1 in 1,000 and 1 in 100 people.

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month, and trials measured results at 72 weeks. Think in months, not days.1

Protect muscle while you lose weight

Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The two levers are adequate protein and resistance exercise two or more times a week.1 Regular moderate activity such as brisk walking is part of the licence, not an optional extra.1

If you have had gallstones before

Say so in your consultation. A past gallbladder problem is exactly the kind of history the prescriber needs to weigh, and it is better disclosed than discovered later.

Pain in the upper right of the abdomen, fever or yellowing of the skin or eyes could be a gallbladder problem: get medical help straight away.

  • Mild, short-lived queasiness after an injection is common and usually settles.
  • Severe pain, pain that does not ease, or pain alongside repeated vomiting: get medical help straight away.
  • If you cannot keep fluids down, get medical help straight away.

You can report any suspected side effect through the MHRA Yellow Card scheme, which helps regulators monitor safety.19 Reporting a new symptom does not replace getting medical advice.

10 · Who should talk first

Who should speak to a prescriber before starting

Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber.11 That review is where your history gets weighed properly.

Mention in particular:

  • any previous pancreatitis or gallbladder problem;
  • other medicines, especially insulin or sulfonylureas;
  • persistent digestive symptoms you already have.

Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Repeat orders re-check suitability, and nothing ships automatically.

If you are weighing up where to be treated, note that Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first.4 Many people who meet the licensed criteria choose regulated private treatment rather than waiting. Either route should involve the same honest conversation about your history.
