04 · What tirzepatide is

What tirzepatide is and how it works

Tirzepatide is the active ingredient in Mounjaro, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

The generic name appears on the carton and the leaflet, while Mounjaro is the brand a UK pharmacy dispenses. A pharmacy that talks about tirzepatide without naming a licensed product, a strength or a prescriber is worth questioning.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1

Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That matters if you ever pause treatment: the medicine does not vanish the day after your last injection.

Each pen holds four weekly doses, so one pen is a 4-week supply. The pen strength on your order should match what your prescriber has chosen for you.

05 · Who can get it

Who can be prescribed tirzepatide privately

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples of those conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1

So the answer to whether it is hard to get approved is: not if you meet the licensed criteria and a prescriber finds it suitable. A prescriber assesses your BMI, medical history and current medicines before deciding.11

Can you get tirzepatide without going to a doctor? Not legitimately. It is a prescription-only medicine, supplied only after a consultation reviewed by a GPhC-registered prescriber. You do not need a face-to-face GP appointment, because the consultation is online, but you do need a clinical review. Anyone offering to supply without a consultation or prescription is not a safe source.

How to check any pharmacy before you order

Check the GPhC register first. Every pharmacy legally supplying prescription medicines in Great Britain, including online ones, appears on the public register of pharmacies held by the General Pharmaceutical Council.12 You can search by registration number, and ours is 9012878. If a seller will not give a number, or the number does not match the business name, walk away.

Second, confirm that a prescriber reviews you. A genuine service asks about your health, current medicines and history, then has a registered prescriber decide. A site that sells tirzepatide on request, with no questions, is a warning sign.

Third, ask what is actually being supplied. UK-licensed tirzepatide is the Mounjaro KwikPen, which comes in six strengths from 2.5mg to 15mg. Unbranded vials, powders or kits described as tirzepatide are not the licensed Mounjaro product.

07 · NHS or private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6

Full rollout is expected to take years. That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. It is a personal decision, and for some people the NHS route will be the right one as it widens.

If you think you may qualify on the NHS, speak to your GP. The NHS has its own guidance on the medicine and who is offered it.5 Eligibility on the NHS is narrower than the licence, so many adults who are suitable for private prescribing will not yet be offered it through their GP.

Private treatment is a paid route to the same licensed medicine, supplied by a registered pharmacy after a prescriber review. The medicine is the same branded KwikPen. What differs is who pays, who decides priority and how long you wait.

Choosing private does not stop you from going back to NHS services later. Tell your GP about any medicine you take, and keep your treatment record handy. Your prescriber can also advise on how the two routes might fit together as NHS access broadens.

08 · What results look like

What results look like in the trials

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks. The comparison group, using lifestyle changes alone, lost 3.1%.2

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

To make the percentage concrete: for someone starting at 100 kg, 20.9% is about 21 kg. That is simple arithmetic on the trial average, not a forecast for you, and it describes the 15mg group over 72 weeks.

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures: in SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure — the dose you respond to may still be ahead of you.

Treat any pharmacy page that promises a specific loss by a specific date with caution. A prescriber can discuss realistic expectations with you based on your starting point, dose and lifestyle. 09 · Year cost planning

If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices, before discounts. A new customer's first order is £40 less, and every later order is £25 less.

You spend at least 4 weeks on each dose, depending on how you respond, and settle at the dose your prescriber considers right for you. A first year (13 pens) costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. Those are standard prices before discounts.

10 · Safety and side effects

Safety, side effects and reporting

People often ask what the bad side of tirzepatide is. The honest answer is that it can cause side effects, as any medicine can. Most are digestive: nausea, diarrhoea, vomiting, constipation and abdominal discomfort affect more than 1 in 10 people. Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13

Read it before your first dose and again when your dose increases. If something worries you, contact your prescriber. Our aftercare includes side-effect support and monthly reviews, so you are not left to manage new symptoms alone.

If you notice a new or lasting symptom, tell your prescriber. You can report suspected side effects through the MHRA Yellow Card scheme.19 Reports help regulators monitor medicines across the whole population.

Prescribers check your health history and current medicines before treatment, because suitability depends on you. Tell them about every medicine you take, any conditions, and anything that changes during treatment. A prescriber can adjust or stop treatment where needed.

Counterfeit and unverified products are a separate risk. The MHRA publishes alerts about counterfeit medicines, and the safest protection is to buy from a registered pharmacy that supplies genuine, licensed pens.7 Be wary of social media sellers, unusually low prices, or anyone offering to supply without a consultation or prescription.

Do not use any pen that looks damaged, has been stored incorrectly, or arrives from an unclear source. Contact the pharmacy and, if you suspect a counterfeit, report it through the MHRA. For dispensed prescription medicines, faulty, damaged or wrong items can be reported to us within 3 days of delivery.

One final point: dose changes are decisions for your prescriber. Never extend, split or double doses yourself. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose and take the next one on your normal day, and never take two doses within 3 days of each other.

Storing your pen and receiving delivery

Store unopened pens in the fridge until first use, and they keep until the printed expiry date. For any question about how long an unopened pen can be out of the fridge, check the patient information leaflet and ask the pharmacy rather than guessing.13

Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen, and keep the pen cap on to protect it from light.13 Follow the pen's instructions for injecting, and never share a pen or needle.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
