How do weight loss injections work?
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, sold as Wegovy and Ozempic.1 That does not make one right for everyone. The choice depends on your medical history, current medicines, how you tolerate treatment and what you can afford, which is why a prescriber reviews each order.
The medicine does not replace the basics. It is used alongside a reduced-calorie diet and increased physical activity, and the licence is written that way. Think of the pen as a tool that makes it easier to eat less, not as a substitute for changing what and how you eat.
You inject once a week, on the same day each week, using the pen supplied. Treatment starts at the lowest strength, 2.5mg, and your prescriber steps the dose up over time according to how you respond and tolerate it. Once a pen is in use, the UK KwikPen is licensed for up to 30 days (not above 30°C) out of the fridge. Before first use, store pens in the fridge. Unopened pens keep until the printed expiry date. For anything beyond that, such as an unopened pen that has been left out, check the patient information leaflet and ask the pharmacy rather than guessing.13
If you are weighing an injection against tablets, the main practical difference is frequency and format. Mounjaro and Wegovy are once-weekly injections. Liraglutide, sold as Saxenda and in products such as Erly, is a daily injection supplied at 6mg per millilitre.
05 · Who is eligible
Who can get a weight loss drug online?
Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.111 It must be used alongside a reduced-calorie diet and increased physical activity.
Meeting a BMI threshold is necessary, not sufficient. A prescriber assesses your BMI, medical history and current medicines before deciding whether the medicine is suitable for you.1 That is the point of the consultation, and it is why a reputable online pharmacy will never supply a weight loss drug on a tick-box alone.

These are the kinds of details a prescriber works through with you, not reasons to guess for yourself.

Treatment is for adults. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. If you are breastfeeding, discuss it with your prescriber so the decision reflects your circumstances. 06 · Results to expect
What results do the trials show?
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
To make the percentages concrete, take a worked example. For someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. Your own starting weight and response will differ, so treat this as arithmetic, not a forecast.
The trial also reports how many people reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
Two points deserve emphasis. First, these figures describe groups, not individuals, and trial averages do not predict your own result. Second, the 72-week timescale is part of the result.
What you can say is that results build over months, they depend on dose, diet, activity and how well you tolerate the medicine, and they vary from person to person.

07 · NHS or private
NHS or private: which route suits you?
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years.
That phasing is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. The two routes are not rivals. - You may be in an early NHS group. Ask your GP about current local access. The NHS prescription charge in England is £9.90 per item where it applies.
- You meet the licensed criteria but not the NHS ones. Examples are a BMI of 30 to 35, or a BMI of 27 to 30 with a weight-related condition. Private prescribing follows the licence rather than the NHS rollout stage.
- You want to start sooner. A private pharmacy can assess you through a free consultation and dispense after prescriber approval.
NHS access to Wegovy runs through specialist weight-management services; ask your GP about it.

Most people do not know their maintenance dose before they start. The prescriber steps the dose up and reviews how you are doing at each stage, so the first few months are about finding the strength that suits you. That is a clinical decision, not a budget one, but knowing the range lets you plan.
Three practical things keep the cost predictable:
- Know the range. Standard monthly pens run from £169.99 at 2.5mg to £329.99 at 15mg.
- Use the discounts. The new-customer discount applies to the first order at any dose, and the reorder discount applies automatically to every later order.
- Spread the cost if it helps. Klarna and pay-monthly options are available, and there is no subscription, so you only order when you are ready.
It is also worth deciding what you will do if you stop. Tirzepatide has a half-life of about five days, so it takes roughly 4 to 5 weeks to clear the system after a final dose. 09 · Other options
Wegovy, tablets, liraglutide and orlistat compared
Mounjaro is not the only prescription route, and the honest answer is that the right drug depends on the person.
Wegovy (semaglutide) is a once-weekly injection. The pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg. Titration steps up after at least 4 weeks, reaching the 2.4mg maintenance dose from week 17, and 2.4mg is the licensed maintenance dose for weight management.18 Its first pen costs £99.99 as a first-order price. Mounjaro and Wegovy act differently. Tirzepatide activates GIP and GLP-1 receptors, while semaglutide is a single-action GLP-1 medicine.1 Compare the Mounjaro first-order starting pen (£129.99) with the Wegovy first pen (£99.99), then look at the maintenance dose you are likely to need.
Weight loss tablets. Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) are prescription-only oral GLP-1 medicines with their own product information.2021 Their trial results come from separate studies and should not be compared with Mounjaro's as if they were head to head. Liraglutide (Saxenda, and products such as Erly) is supplied at 6mg per millilitre and is a daily injection, unlike the once-weekly Mounjaro and Wegovy.
Orlistat is another licensed option, but figures vary, and your prescriber can advise on whether it suits you.
Supplements. So-called natural alternatives are low-cost supplements with no clinical oversight. They are not an established substitute for licensed medicines, and dramatic weight-loss claims on their packaging should be treated with caution.
Do weight loss tablets actually work? Licensed prescription tablets have their own published trials, and your prescriber can explain how they apply to you. Unlicensed pills sold online with dramatic claims are another matter entirely. If a product needs no consultation, it is not the same thing as a licensed medicine, whatever the label says.
The practical advice is to start with the consultation. A prescriber can look at your history and discuss which treatment is appropriate rather than leaving you to compare marketing.
10 · Safety and side effects
Is it safe? Side effects, reports and counterfeits
Mounjaro is a licensed prescription medicine, and it is safest when prescribed after a proper assessment and used as directed. Most side effects are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and they are most likely when starting treatment or moving up a dose, usually easing within days to a few weeks.
If you notice a new symptom, speak to your prescriber. You can also report a suspected side effect through the MHRA Yellow Card scheme.19 If you notice a symptom that is not listed in the patient leaflet, you should still tell your prescriber so they can assess it.
Medicines that slow stomach emptying can affect other medicines. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.7 Tell the prescriber about insulin or sulfonylureas too, as their doses may need adjusting because of the risk of low blood sugar.
Counterfeit medicines are a real hazard for anyone buying weight loss drugs online. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme.7 To protect yourself:
- Check the register. A legitimate pharmacy has a GPhC registration you can verify on the public register.12
- Expect a consultation. Anyone offering to supply without a consultation or prescription is not operating legitimately.
- Beware of social media sellers. Marketplaces, social posts and messaging-app dealers are not regulated pharmacies.

12 · Starting and stopping
Starting, storing and stopping treatment
Treatment starts at 2.5mg and is stepped up by your prescriber, so you will not begin at the dose people quote from trials. Your first pen is a four-week supply.
That is one reason you do not skip ahead or change strength on your own.
Storage is simple but important. Keep unopened pens in the fridge until first use; they keep until the printed expiry date. Once a pen is in use, UK KwikPens are licensed for up to 30 days (not above 30°C) out of the fridge.13 If a pen has been left out of the fridge before it was opened, read the patient information leaflet and ask the pharmacy rather than guessing. Outside the tested window, sterility and potency are no longer assured.

What about stopping? Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
