Off-label Mounjaro means a prescriber or patient uses tirzepatide in a way the product licence does not describe. A licence sets out the approved conditions, patient groups and dosing for a medicine. The label is the written version of that: the product information that comes with the pen.

In the UK, Mounjaro is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose KwikPen in strengths of 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Use that stays inside those terms is on-label. Use that steps outside them, in purpose, dose or who it is given to, is off-label.

Off-label does not automatically mean illegal or dangerous. Doctors can legitimately prescribe licensed medicines off-label when they judge it appropriate and take responsibility for that decision. The phrase becomes a problem when it is used as a loophole: a way to describe a routine that skips the safeguards the licence was built around. 04 · Off-label vs unlicensed

Off-label vs unlicensed vs counterfeit

These three terms describe different situations, and mixing them up is where most confusion starts.

Off-label describes a medicine that holds a UK licence but is used outside what that licence says. The product itself is genuine and regulated. Mounjaro's licensed uses are weight management in adults and type 2 diabetes.1

Unlicensed describes a medicine that has no UK marketing authorisation for the use in question, or at all. Medicines still in development are in this group. They cannot be sold as a normal treatment.

Counterfeit describes a fake product made to look like the real thing. The medicine may contain the wrong amount of active ingredient, or none, or something harmful. This is a supply problem and has nothing to do with licensing categories.

A page, forum post or seller that calls a product "off-label Mounjaro" is not telling you where it came from. Genuine, regulated supply comes through a registered pharmacy after a prescriber review. You can check any UK pharmacy on the GPhC register; ours is registered under GPhC 9012878.

05 · Non-diabetic use

What happens if a non-diabetic takes Mounjaro?

Taking Mounjaro without diabetes is not automatically off-label. Mounjaro is licensed for weight management in adults as its own indication, separate from type 2 diabetes.1 A person without diabetes who is assessed and prescribed it for weight management is using it on-label.

What matters is the assessment. A prescriber checks that treatment is suitable for you and that you meet the criteria, and then supports you through dose changes. That is very different from taking someone else's pen or buying from an unregulated seller.

In the headline SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

For someone starting at 100 kg, 20.9% is about 21 kg.

People who do not meet the criteria are the ones most likely to be drawn to off-label routes. For them, the risk-benefit balance is different, and the treatment has not been tested in that situation. That is exactly why an individual clinical review matters.

06 · Appetite suppressant claims

Is Mounjaro an off-label appetite suppressant?

Mounjaro is not licensed or sold as an appetite suppressant. It is a prescription medicine for weight management and type 2 diabetes, and it works through gut-hormone signalling rather than as a stimulant.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

That effect on appetite is part of how the licensed treatment works. Treatment is built around a gradual climb through set doses, with reviews along the way.

Beware of products sold as an "off-label appetite suppressant" or as "nature's" versions of prescription injections. Low-cost supplements are available without assessment and carry no clinical oversight. Their claims of dramatic weight loss are not backed by the kind of trial evidence the licensed medicines have.

07 · Licensed alternatives

Licensed routes and alternatives

If off-label looks attractive because of cost or access, it helps to know the licensed options first. There are several, and each has its own rules.

NHS. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Wegovy. Wegovy (semaglutide) is another licensed weight-management injection. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 A first Wegovy pen costs £99.99 at the first-order price. NHS access to Wegovy runs through specialist weight-management services; ask your GP.

Liraglutide. Liraglutide (Saxenda, and products such as Erly) is supplied at 6mg per millilitre. It is a daily injection, unlike once-weekly Mounjaro and Wegovy.

08 · Risks and reporting

Safety, side effects and reporting

Off-label routines carry the same medicine-related risks as licensed use, plus extra ones that come from the lack of supervision and evidence.

Tirzepatide slows how quickly the stomach empties. That is part of how it works, and it is also why the medicine can reduce absorption of oral contraceptives.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you use oral contraception, ask your prescriber about this. For people taking insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. That is something to be managed by a clinician, not guessed at.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That matters if you try something and want to stop: the effects do not end the moment you stop.

Store pens in the fridge at 2–8°C until first use. Unopened pens keep until the printed expiry date. For anything beyond that, such as how long an unopened pen can be out of the fridge, check the patient information leaflet and ask the pharmacy.13

Any new symptom should be discussed with the prescriber. You can report suspected side effects through the MHRA Yellow Card scheme.19 If you have bought a pen from a source you are not sure about, do not use it. MHRA also publishes counterfeit medicine alerts.7

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
